"My mother is going to be moving next week into assisted living at Rawlins House & Fall Creek Retirement Village. She lives in the nursing home part and she's moving to assisted living. It's not as modern as the other ones we visited. It needs some updates in general. The staff was very nice."

"We went there to visit with the residents. Some of them were ok, but most were kinda off. They sat out in the hall mostly, in wheelchairs or in beds. A lot of them were broken spirited.The whole place smelled of urine and feces. The staff was inattentive. At one point, I heard one of the residents screaming. I asked one of the nurses about it and he shrugged, "She does that all the time. She will stop soon."My visit to this facility was probably one of the most troubling experiences I've ever had. "

"We liked Morrison Woods, but they didn't have a room available at the time we were placing my mom. I like the fact that they had Medicaid there and I wouldn't have to move mom from one facility to another because they have continuing care. They also have an Alzheimer's unit. The room sizes were great. The staff there was very, very pleasant. I know someone who does the activities there; I know her personally. A lot of their activities don't reach my mom's standards because she is still able to walk. They had a buffet set up. It looked very, very nice. It's a very, very nice facility. "

"Westminster Village Muncie is out of our budget range but it's very nice. It's very clean, the nurses are very nice and they are helpful. They have a lot of things that you can do, and a beauty shop. And from what I saw, it was very nice. I had a meal there and it was very good. In terms of value for the money, it's way overpriced, they've used up all her money and so she has to move."

"If you have need of a long-term care facility, I highly recommend Liberty Village here in Muncie. Several years ago, it became obvious that my elderly aunt was in need of more care than my wife and I were able to give. I am sure there are several wonderful facilities in and around Muncie and we considered others before choosing Liberty Village. For us, as with most families looking to place a loved one, we wanted the best care possible for her. A few years prior, my aunt had a good friend living at Liberty Village and she was always telling my aunt how much she liked it there and how good they were to her. After meeting with some of the staff and touring the facility, my aunt was more than ready to move into her new home and begin meeting her new family. My aunt was immediately welcomed not only by the entire staff, but by residents as well, and in just a few weeks became part of the Liberty Village family with most everyone calling her by name. She would walk the hallways talking, laughing and joking with other residents and staff, became a regular winner at bingo and took part in most all activities offered. Every time we visited her, she eagerly shared what she had enjoyed that day. As the years passed, she became less active and not always her "friendly" self, sometimes complaining to the staff about something that to most of us would be a non-issue, yet the staff responded to her every request with a caring and positive attitude. In February 2010 my mother became virtually immobile and was no longer able to remain at home. After seeing the kind of love and care that my aunt was getting at Liberty Village, we didn't consider any other location and placed my mother there also. Unfortunately my mother's condition was much worse than anyone had previously thought and she passed within a few months, but not without getting the best possible care, attention and respect that anyone could receive. Offering their condolences with tears in their eyes and sorrow in their hearts, her caretakers from Liberty Village attended mother's calling. Even though she was there such a short time, the staff had grown to love her too. Shortly after mother passed, my aunt began to further decline. Seemingly the entire staff of Liberty Village was saddened and did even more to show kindness to her, knowing the end was near. Eventually she was provided in-house hospice care and was made as comfortable as possible during her last few weeks. She died two days before Christmas, peacefully, pain-free and with several of the heavy-hearted staff checking on her, and us, up until her passing. Over the years my wife and I have made many unscheduled visits to Liberty Village on different days and at different times of the day. On many occasions (unaware that they were being seen), we have noticed the staff, including the custodians, interacting with the residents in a manner that could only be seen as patient and kind, spending quality time with someone else's loved one. This type of loving attention to residents, as well as family members, seems to be what the entire staff at Liberty Village is all about and view as a normal day at work. They are all "special people" and, in our view, go above and beyond their professional job scope in truly CARING for their residents. Each of them treats the residents as if they were their own family. We, the family, are overwhelmingly amazed at the care given, as well as the compassion and professionalism shown, by the entire staff of Liberty Village. We wish to acknowledge and say a public "THANK YOU" for ALL that was done for our loved ones, and for us, as we watched them do for our family what we were not capable of doing. "

"We used Yorktown Manor for my dad's rehab. We also used the facility when both my mom and dad's health started to deteriorate since we were already accustomed to and familiar with it. It was only eight minutes from my home, so it was a perfect choice for us. The staff was extremely pleasant and helpful. The facility was fairly close to a hospital and local doctors. It was located in a residential area and had very pleasant surroundings. "

"I was brought here December 2022. I had been in another facility and had a very bad experience. From the first few days I was treated like a person. The level of care I receive is fantastic. I get three showers every week, the staff is excellent, everyone is at the minimum a CNA, even management, the nurses are amazing and very helpful. I have a great room with my own paintings on the walls. I am at home here. Even most of the food is good. I could live out my life here and I would be happy. Plus, I’m taking part in the activities several days a week. We play bingo, arts and crafts, church services, occasional outings, just so much to keep us busy and feeling fulfilled. Our physical and occupational therapy services are some of the best I’ve ever experienced. "

"My father-in-law is in Parker Health Care & Rehabilitation Center. They are awesome. Somebody is going in and out of his room regularly so they are checking on him and making sure that he's not falling. They're very cheerful too and the staff is just wonderful. They're busy and constantly working with the patients. You don't see the halls empty. They're even bringing some of the people up to the nurse's desk, those that tend to wander or fall, so they can interact with each other and they can see them. The first day we were there, a physical therapy person jumped in to help us. They happened to have a meeting that they only have once every 3 months, and we just happened to come in to get my father-in-law transferred. She jumped in and helped get him situated and everything. It wasn't even her job, but she did it with a cheerful attitude. They're just superb and I heard nothing but good things from other people that have someone in there too. It's a nursing home and also a rehab place, which is what my father-in-law is in for. I heard they have a very small assisted living area that's only like 9 people. One of them is double occupancy, so there are only 8 rooms. It's very small and they don't have any openings now for that. I didn't know about it until we put him in because they don't have openings. I noticed that they don't have as many rooms as they could, in terms of meeting areas and stuff like that. The hallways would be a little better if they were wider too but they're fine and good enough for what they're doing."

"If you don't mind underhanded staff, having to fix issues that they caused with Medicaid every couple of months, and dirty surroundings then this is the place for you! I am trying to move my dad oyt of there. The care is substandard. They don't call and let you know when your loved one fell, much less send them out to the hospital. My dad had to deal with pain over night. They called early the next evening to inform me that my dad was on his way to the hospital. After an er visit and xrays it was determined that he had dislocated his shoulder. He also chipped the upper arm bone. He had to go to a specialist for a while over it. He sat there in the nursing home for 24 hours in that condition. He was in pain. They only called when they had already sent him out in an ambulance. I there are two counties between me and my dad. I will not ever put him in a nursing home that lacks reviews. I will make surprise visits before I commit to the new place. Miller's of Middletown smells horrible and is anything but clean. Don't take my word for it. Go check for your self. You will learn that it is everything I claim and more. They have been investigated so many times by adult protective services that I am amazed that they are still in operation. "

"My mother was in Bethany Pointe Health Campus. She really enjoyed it, and especially she enjoyed their food. She said they had excellent food. She went in for rehab and she was in one section for COVID-19. She had occupational and physiotherapy. I think she improved as much as she could, but she's going to be close to 100, so we didn't think she was going to jump up and be running around the block, but she got stronger. I didn't taste the food, but she liked it so much, and she's pretty picky. I think once she got out of the isolation area, she had a private room. She was able to go down into the areas where people kind of congregated. She rolled herself to see people going up and down the hall and that sort of thing. She said she really liked the room. I think it's a pretty new area. It may have been renovated. She said it was very pretty and she enjoyed it. It's very comfortable. She liked looking out the windows and that kind of thing. They had a courtyard, but she couldn't go outside at the time. They had another area where people could sit and talk and kind of meet new people. That was all in their newer renovated areas. She was very pleased with it. She's legally blind, so she really didn't participate in puzzles or anything of that nature. She enjoyed the socialization part of it. The staff was fairly good. I called them for updates."

"This facility is completely changed from its former self of just 2 years ago. Glenn is the new administrator. He's considered by many of my friends in the medical field as the best in the county. He has completely turned the place around. From a one star rating, it will soon be a 4 or 5 star. Many of the workers are still there from the past, but many were replaced if they weren't doing their jobs. The food is great, the caring is there from everyone I've met. I highly recommend it. My 95-year old mom lives there. They are finishing up a half-million dollar remodeling of the entire facility. One wing of rooms is even single occupancy now. "

"Northview Health And Living is kept very well and the grounds are nice. The room is just kind of small, but everything else is fabulous. So far, the staff have been lovely. The aides are friendly and compassionate. The admissions people were very helpful and so far I would give them all high remarks. My mother is limited because she has macular degeneration, but they do therapy with her Monday through Friday and they have music activities. For the other residents, they have games, cards, and movies. She has participated in singing and piano playing. Today, I believe, they're doing a church service or a Bible study. My mother is gluten-free and they have tried very hard to accommodate her diet. I give them high marks there."

"My grandmother was a resident at Countryside and I cannot say enough, how great their staff is! She was a new dialysis patient and Countryside's therapy team did a great job working around her chair time as well as the care team providing support to our family. Countryside's team truly made my grandmother feel apart of their family.
