I got sick twice and ended up in intensive care one of the times. I was not offered the opportunity to receive a Covid-19 immunization shot, either from the facility doctor, or for my caretaker to transport me to my doctor’s office.
The service was not very good. I spent most of my time back and forth from the hospitals, and my quarantine was restarted back to zero each time I returned.
I would not recommend any one to go here. They do not return phone messages or update patients on their condition.

Very disappointed in the facility. It was great when it was Holy Family and Religious women worked there. It has really gone downhill since becoming symphony. I am looking for a new facility. It is no longer as clean, the nurses hang around the station chatting with each other when residents are screaming for help, and the food leaves a lot to be desired. I had to interrupt the nursing staff hen fest to get them to reposition a patient who was screaming in pain and all twisted in his wheelchair. Unacceptable! They serve crusty hard to cut pieces of breaded chicken (mostly breading) that is too difficult for the residents to cut so they don’s eat as much. No longer impressed and residents are not getting what they are paying for. They need supervisors who know what the staff is doing and not doing, and nurses who care. So sad.

My 92 year old mom passed away at this facility 9 months ago On january 18th 2021. She had lost a good portion of her eyesight within 10 months prior to entering this facility. She then had something similar to a stroke. Unable to move for the most part and declined each day. Due to covid I was not able to see her. Trying to reach someone on her floor to either talk to my mom or a staff member was a struggle. The ONLY nurse i was able to reach and had compassion was louis. The floor manager a short young girl had NO COMPASSION at all. When they told us they were putting mom in hospice they told us we could make an appointment for 2 days later to see her. All the floor manager cared about was if mom was still their after 5 more days we would have to pay. We gave her our decision to leave mom at the facility vs risking mom passing away in an ambulance if we brought her home. After we told her our decision all she did was walk away from us. Maybe she will be in the same situation some day. And I hope some day I will see her so I can tell her how I feel to her face. She is a disgrace to that facility.

You can be fed in your room, or you can go to the dining area and sit at a table. Behind the kitchen is the rehab facilities. The staff spoiled him. They were friendly, and it was like home. The nurses would visit, sit, and chat with him. He could play cards, watch TV, and sit in the screened porch when the weather was decent. I have a family member that’s in manor care that’s blind and needs assistance with almost everything. She was admitted on February 13, 2020 and on Saturday, February 15th they did get her up and dressed. On Sunday, February 16th I noticed she still had the same outfit on so I asked if she was going to get cleaned up and changed, well of course the nurse that was on staff said of course. I came again on Tuesday, February 18th and she was still in the same clothing, I was furious, well again I was told they was going to change her. I came back on Wednesday, February 19th and there she was with the same outfit on. Finally I told them she was not leaving for dialysis until someone cleaned her up and change her clothing, finally two CNA’s took her in the bathroom and washed her up. This is terrible how she’s being treated. This past Sunday, March 1, 2020 I asked for a manager, finally someone came in and stated she was a manager her name was Shannon, I mentioned to her that they patient needed a shower asap because she’s been a patient at manor care since February 13th and no one thought it would be nice to take her and give her a good shower, her body is very itchy and nasty looking, she assured me that she make a note and have the next shift give her a shower. Time went by so I asked the nurse she assured me that she would get a shower if not that night first thing the next morning which was Monday, March 2nd. She is being neglected just because she can’t see or do anything for herself. I told them she loves playing bingo but need assistance, I was told no problem someone will assist. No one ever, ever, ever came to get her, all they do is leave her sitting in the lobby ALL DAY LONG. She love sitting in front of her tv listening to her programs. She’s totally IGNORED. If you have a love one that cannot do for themselves DO NOT SEND THEM HERE. This place is only for patients that can take care of themselves. Managers do not return phone calls at all. The only person that was nice, truthful and showed concern was the REHAB DOCTOR!

My mother in law was transferred there December 3, from munster community hospital. The plan was for her to be somewhere she can have therapy to get her back up at least able to get in a wheelchair. ( with in house dialysis) My family called for three days . I was told someone would contact me with a care plan. Nothing on the forth day I asked to speak to the director. That’s when the person who answered the phone wanted to be of assistance. They wasn’t calling us. They claimed they called her husband who passed away in 2017 phone. And other contacts from a contact list from a visit in 2016 mind you her contacts was updated at the hospital. It’s been 14 days she has been in the bed no therapy we still can’t get a real care plan. I was told by the nurse director she has been in the bed because she was on the covid unit. I understand that but she has been off the unit 3 days. I went to go pick up her dirty laundry. Her clothes are still packed and cleaned from the day my niece dropped them off. They never even touched the bag. I called and asked what’s the procedure and was told by the person that answered the phone it wasn’t their job to check or count clothes. We was told the activity director would be calling so we can have a zoom. Haven’t spoken to her yet but I’m sure she has a few voice-mail. I called to speak with a dialysis nurse. She couldn’t tell me anything about her dialysis said I had to talk to someone from day shift but day shift told me to contact her. At this point I am at my wits end. They are using covid as an excuse to not do their job the social worker wants to know her care plan for home but it seems like they have no plan solid plan or solution their. They can’t tell me a food plan. I don’t know what the rooms look like because we are not allowed in because of covid. I am really upset I think I’m going to get my lawyer involved because this is really got me baffled.

That’s when the person who answered the phone wanted to be of assistance. They wasn’t calling us. They claimed they called her husband who passed away in 2017 phone. And other contacts from a contact list from a visit in 2016 mind you her contacts was updated at the hospital. It’s been 14 days she has been in the bed no therapy we still can’t get a real care plan. I was told by the nurse director she has been in the bed because she was on the covid unit. I understand that but she has been off the unit 3 days. I went to go pick up her dirty laundry. Her clothes are still packed and cleaned from the day my niece dropped them off. They never even touched the bag. I called and asked what’s the procedure and was told by the person that answered the phone it wasn’t their job to check or count clothes. We was told the activity director would be calling so we can have a zoom. Haven’t spoken to her yet but I’m sure she has a few voice-mail. I called to speak with a dialysis nurse. She couldn’t tell me anything about her dialysis said I had to talk to someone from day shift but day shift told me to contact her. At this point I am at my wits end. They are using covid as an excuse to not do their job the social worker wants to know her care plan for home but it seems like they have no plan solid plan or solution their. They can’t tell me a food plan. I don’t know what the rooms look like because we are not allowed in because of covid. I am really upset I think I’m going to get my lawyer involved because this is really got me baffled.

If I can give zero stars I would. I don’t know where to begin. My father was a patient at this facility for only a week. In a matter of a week. This facility did not give him his required meds, misplaced inhalers, bruising on his eye and a lump on the head (which may required stitches), arm bruised due to blood work (which was not authorized) dehydration and lack of exercise. The nursing staff failed to properly insert a catheter. Patients only leave their beds for dining purposes only, maybe if there is an activity going on. According to the staff and I swear you cannot make this up! Patients were in bed due to quarantine. Yet they were all eating in the dining area at the same time!! They did not change my father’s clothes maybe twice in a week. They even sent another man’s boxers in his suitcase!!!! Again you cannot make this up! I will be contacting the VA and put in a complaint through them. This facility is contracted through the VA so I will go through them. I also have pictures I will be submitting as well.

This is hands down the worst rehabilitation/nursing facility you could ever have you loved one spend time in. Patients wander the halls aimlessly because there is rarely any activities for them, the staff is friendly but over worked and the director of nurses is a very smug woman who had an issue with being addressed about her nursing staff. Food is passable but not the best. Rooms are cramped with very poor lighting. Patients have no tv or cell phones unless they are provided by the family. The facility itself constantly reaks of human excrement and its just disgusting.

I filed a complaints( proven). In addition to, they discharged my family member after all of my complaints and kept all of his belongings. The employees were rude, and unprofessional. I would never recommend this place for your love one. Keep searching and read all of the reviews for this place.
