"I was at Diversicare of Bessemer for rehab. Their CNAs were great but I had problems with the nurses there. There was a medication mix-up on the nursing side. Everything else was alright. The physical therapy was excellent. Some days the meals were not good, other days were pretty good. It was very clean but looks like it needs to be remodeled."

"My father was in Columbiana Health and Rehabilitation, Llc for three weeks. The people were very friendly and looked after the patients well. The staff was efficient and very kind. It was more of a family setting than a nursing home or facility setting. He thought the food was good. They had some outdoor spaces. They had multiple rooms and multiple things going on. It was a nice place to be. It was highly recommended to us for his situation."

"My father in law was a resident here in 2003 and passed away there. Since I have kept in contact with their activities director "Denise" and make things for the residents at Christmas. I have been doing this since 2003 and I get more out of them allowing me to do this and the surprise expressions I get from the residents than I do all year long. It is so worth it for them as well as myself. I think Plantation provides excellent care , it's clean and Denise seems to be very involved with their well being and concerned about keeping them entertained. She does a wonderful job as well as the rest of the staff. Recently I noticed it had been painted and looks very nice and happier. The nurses seem to be very patience and kind to the residents. I am very impressed with Denise overall because she really loves her job and wants to make a difference and I believe she does. "

"I was sent to HealthSouth Lakeshore Rehab and they were quite good. They have several places where you can enjoy a meal with friends or with other patients. The physical therapists are excellent. The food was good and more than adequate. The room is just basic, a bed and a couple of extra chairs, built in dresser and a closet. It is very well maintained and more than adequate in size. "

The staff was very nice, explained things, and was very courteous. She took us to the physical therapy area. Everything was in place. The clients all seemed clean and dressed."

"We had my 74-year old sister transferred to Greenbrier from another facility that was not taking care of her and seemed to have little interest in rehabilitating her so she could return home. The facilities are very nice and the staff was always helpful, more importantly the medical staff was actually interested in helping her progress from assisted living to independent living in her own home with a great rehabilitation program. The physical and nutritional therapy helped get her back to a balanced state where she could take care of herself again. It was a great transformation from someone who was losing the will to live, to once again caring for herself and other family members. She has successfully moved back to her house, and we enjoy the company of our sister once again."

"My experience with Self-Nursing Home was great. It was quiet and clean. They served breakfast, lunch and dinner for my mom and dad. The staff was great. There was one person per room and the room was good. I had one look at two places and this one was clean, it was quiet, and it didn't have people sitting or hanging outside. "Self Health Care and Rehab Center is wonderful. So far we have been really happy with it. It’s centrally located between my three siblings, and it’s one of the reasons that we got it. The biggest thing was that we were able to get her into a private room. The room is nice and large, and we have her set up with the nurse, but then we have been able to put our furniture in there. She has a living room area also. The staff is absolutely great. We are very happy with the nurse that takes care of her on a daily basis, and then the caregiver that works with mother with her personal care is very good. "
