I can't say enough on how great the staff took care of me. I could barely stand when I arrived but the wonderful Occupational therapist and Physical therapy worked with me, patiently and now I walk distance with a cane or sometimes without. The CNA's played a major part in my success rooting me on and the food was delicious with great cooks. Two thumbs up!!👍👍

I went in on Wednesday the 13th and was given my picture of water. They put me in a room up against the wall. I said hello to the roommate and I was like boy. This is a small space and they said it was gonna be too loud in this room and put me in a different room right next door in a small space again, we measured it and it ended up being 7 1/2 feet long and 4 feet wide . For the 52 hours there I was miserable every time I called for the nurse. It took forever for some staff member to come and just basically walked me to the bathroom. That’s all they had to do was follow me to the bathroom. I finally just had to give up and start going by myself in great pain since I broke my pelvis in my tailbone. It was a terrible experience loud TVs going at all hours of the day. I had no control over my light switch since it was broken so they would just turn on the light whenever and leave it on and leave the door open. The food was awful My roommate left her TV on all night and they came in twice in the middle of the night both nights because they need to bathe her and put medicine on her open wounds Almost 2 times taking myself to the bathroom I had to ask for more water on Friday luckily, my friends brought me water . I found that hardly anyone had common sense and no one seem to care except for two nurses that I barely ever saw. I was scared to death to go to sleep, and even if I did go to sleep, I’d be woken up by loud noise, terribly and said that this room was terrible, I only went there because I wanted to do rehab because I really don’t have help at home, but I had to find help even though I need more help. I’d rather be at home. At least I get to sleep never clean my room until I requested it on Friday morning , and all they did was sweep up some trash and they didn’t even get it all . A terrible experience. I know if I stayed there any longer mental health would go down and I would not get better and not get any rest for a long period of time. PT and OT staff was excellent . I truly did not belong there and I truly would’ve gotten severely depressed. If I had to stay any longer, it was very sad and depressing.. CNA did not want to be there at all. They were just there.for a check . It was just a trauma of an experience

The employees are very rude and laugh and joke about the situation they just sit residents in small rooms and do not care for there patients as expected very unprofessional there were some lady’s there that was very nice and professional but for the most part I wouldn’t recommend that facility for any family memebers

I pray for anyone thinking of using this facility for their loved ones to read this review. My mom had multiple strokes and we put her in Sardis oaks for rehab. Our initial visit before placing her was really good however once in their care her health deteriorated to the point we had to place her back in the emergency room for dehydration and other bleeding complications. She couldn’t swallow when she went to the facility but rather than implementing an IV for feeding and fluids they just left her food tray in front of her and never really tried to ensure she was eating or remaining hydrated. Additionally we would constantly have to tell the staff to change her as she would sit in her waste for hours. Finally she was suppose to receive 120 hours daily of PT/OT/ST but I never saw her actually doing any real physical therapy or swallowing test for her throat problems. I’m not really sure how this facility has maintained a 4.0 overall rating and it really needs to be re-evaluated. No loved one should be treated in this manner and my prayer is true exposure to this facility and the personnel that work here.

My friend's mom was there for rehab. I visited several times and it was always clean and she seemed well taken care of. She did great in rehab and went home after a couple of weeks.

My family member was living at Sardis Oaks for 8 months. She was in the long term care part of the facility. They are understaffed or the nusrses and aides don't respond when you press the help/call button. Average wait time for assistance is 30 to 45 min. A friend or relative must stay on top of the patient's care or they will be left for hours on end with minimal care. My relative was unable to feed herself at all and they repeatedly left trays of solid food on a table and left the room. They did not give her water on a regular basis - they would leave a pitcher by the bed which she was unable to pour into a cup on her own. They have activities for those that are able to participate, but for those patients that are bedridden they do not actively take care of them.

My husband was recently at Sardis Oaks for a bad knee injury. He shared a room and they are small, but the care he got made up for the small room. The rehabilitation staff and facility is excellent and that was our main reason for going there. The nursing staff is caring and competent. The dietary staff helped deal with his not wanting to eat. People were in constantly to check on him, cutting up with him, and encouraging him. He was near the nurses station and sometimes it got a little louder than he would have liked. I would not hesitate to go back there, small room and all!

I fell and broke both bones in my lower right leg. After spending 20 days at Pineville Rehabilitation Hospital, an acute care facility, I spent 40 plus days at Sardis Oaks, a sub-acute facility. I could not be more pleased or happier with my choice of facility. The staff at Sardis Oaks is wonderful. The nurses (RNs, LPNs, NAs) were excellent and so caring. The therapists (physical and occupational) were exceptional. Everyone from the administration through the health care individuals to food service and custodial could not have done a better job for me. I live alone and Sardis Oaks prepared me to come home and again do things for myself. I sincerely feel that the professionals at Sardis Oaks are more like family and close friends than simply people that I met at a health care facility. I simply can't recommend Sardis Oaks enough.

when to see a friend 2 weeks ago, she was in a very small room with another woman. she was In the room where she could hardly move or get up to go to the Bathroom!! she had troubles with money so i think that why the people/ nurses did not help her more!! they took her outside in her wheelchair and left her when she had an issue and had to get back inside, and did nothing! this place needs to get help and be a whole overhaul!!! I am so very mad and writing this for her also, worst place ever

I recently rehabbed at Sardis Oaks following a hip replacement. The rehabilitation staff and facility are excellent. The nursing staff is competent and communicative. The administration and caseworkers went above and beyond to ensure that I received the time required for rehabilitation and that I had a good next step upon discharge. The food was typically institutional but options are available and the dietary staff made certain to deal with my unusual food allergy. The is a great place to go after your surgery.

My wife was at Sardis Oaks for a little while. It was more like a skilled nursing facility and wasn't really long-term. The care depended on what shift you were on. It had some great people there, especially the therapists who were awesome, but some of the nursing could have needed some more training. The cleanliness was good. The food was OK. My issue was with the staff since some of the CNAs and nurses were not very attentive. I think they were understaffed.

I was referred to Sardis Oaks by the hospital and by other people that had patients there. The facility was adequate, fine as a nursing home, and the ratio of nurses to patient was much higher. The staff was fine, and the food was OK.

If I could give no stars I would. This place is horrible and for the first time in my life I am afraid to get old and end up in place like this. I would not let my dog stay here. My mother had to wait sometimes 45 minutes or more to go the bathroom, pain medication was given 2+ hours late, staff was mean and rude and on the week-ends the place is dangerously understaffed with one temp nurse and one assistant covering 2 halls. My mother luckily was able to call me and I could call up and demand someone take her to the bathroom or give her her medicine. But she was still traumatized by the place and said she would rather die than go back. Nice right? Awful management, under-staffed, rude and incompetent staff who couldn't perform basic care...I am filing a formal complaint.

If you care about your family member absolutely do not send them here. last year my mother stayed at this place and it was a wonderful place. This year is a totally different story. they are so slack there. my mother went to the hospital because they could not get her nose to stop bleeding. she was there for four hours at the emergency room. When she returned her bed was still soiled with blood. the ambulance driver changed her bed and put new sheets on. yesterday they let her lay in respiratory distress for hours. I had to call the front desk and tell them she needed to go to the emergency room. My mother has now been admitted to the hospital. PLEASE !!!! if you love your love ones do not let them stay here.

My family member was sent there for short-term rehab from hospital. We arrived later in afternoon to complete disfunction. She was out in a semi-private room by transport company. We were told to wait because admin, PA, And head nurse needed to meet with us. We waited over 2hrs, asking every 30min is someone was coming, and being told they were. After 2 hrs, we were to the Dr., head nurse, and admin went home. The other staff had no IDEA what meds my parent needed and when they were to be given. They were confused and blamed the discharge office from hospital. They were lucky because I had the entire medical record, which I paid for before leaving hospital. We also told staff that my parent had MRSA and wanted to know why universal precautions were not being used. They claimed they were not told my hospital, therefore, they would call to verify. My parent stayed in that semi-private room all night without anyone using mask, gowns, or gloves. The next day, I received a call from PA stating the infectious control was called in and she should not be in a room with another patient. PA also stated that med order was very confusing and they probably missed medicine overnight because of confusion. I personally walked around the facility several times while waiting for 2 hours and I saw nurses sitting, patients falling, patients yelling for help, patients laying bed in diapers, and patients sitting in halls star gazing. I personally was not impressed with this facility and I am glad my parent will only be there for 2 week for rehab. If you need long term care, go there and see it for yourself, at night!

If you have a loved one needing nursing care DO NOT PUT THEM IN SARDIS OAKS!!! The care is awful. My mother has been left all day without being changed over and over again. The response time when she calls for assistance is outrageous. She called for help bc she was falling out of the bed. 35 minutes later someone showed up and then rudely asked "What do you want?" Most all of the staff are rude, uncaring and don't follow standards. For example: This is a NO LIFT facility where patients are to be moved by use of a slide board. Time and time again she is lifted off of and onto the bed. She is to have access to certain items at all times; such as her phone and "grabber". These are invariably on the other side of the room. A good number of times she has not even gotten her lunch until 2:00 in the afternoon or later. She has even had to call to ask if she could have food! Many of the staff are rude and uncaring. There is ONE worker who has been very nice. And the therapists are fine. Oh! Another thing! She was not even gotten up out of the bed until 1:00 on Sat. and consequently did not have lunch until 2:00 p.m. Many times, when the therapist come to get her for her therapy session, she has not been bathed, changed or dressed. They have to take the time to do all threee before she can go to therapy. This takes away from their valuable time where they could be working with my mother and other patients! The halls always smell of feces. This place is HORRIBLE!!! Complaint after complaint has been made to the management with no results except for the same sorriness!

She was not happy with her situation during her cognitive moments, but it was about not being at her home rather than an issue with her level of care. Sardis was a clean nice facility with caring staff. Visits with my grandmother were easy. We could come and go as often as we liked and we could also check my grandmother out to stay with us in our homes. She had her own private room that we were able to decorate with her personal items. In the main center of the floor she stayed on was an open living area for the residents where they could have visitors or just enjoy each others company. We were able to have a hair stylist come to her room and fix her hair like she liked it which always seemed to brighten her spirits. As well as being knowledgeable staff was also empathetic as her condition got progressively worse. They kept a vigil watch over her and when she developed a high fever beyond their level of care they transported her quickly to a hospital that could better treat her. They were a true comfort to our family in her final days.

Everyone was incredibly loving and sweet- my little brother and sister were welcomed, despite their young age, and many of the residents doted on them. In addition, animals were kept at the facility and there were beautiful gardens, which I thought was a great thing.

I visited Sardis Oaks when I was assisting a family in locating a facility for their mother who could no longer remain independent in her home due to health issues. Sardis Oaks is a clean facility and the staff are friendly. My immediate impression however, was that it's an older facility and could use some updating. All of the rooms I saw were double occupancy and I think for a longer term stay single occupancy rooms would allow for a more private and at home feel. In addition, the wing we visited had a shared bathroom (so not even an individual bathroom for each double occupancy room), another downfall especially for elderly who might have difficulty walking out of their room and down a hall to get to the restroom in the middle of the evening. The dining room is clean and friendly and I saw a couple of residents enjoying a meal with their friends or family members. There are also a good amount of activities available (at least one daily - from arts and crafts to chair exercise). I noticed a nice little courtyard area at this facility and a few residents enjoying some outdoor time. The location for this facility is well situated for outdoor time as it sits back off the road and seems to take up several acres of land. In addition, the facility offers a hair salon and on the day I was there one resident seemed to be enjoying her time with a very friendly hair dresser. Ultimately the family I was working with decided to go with a different facility because of the lack of private rooms available at this facility. This one did make about the top five in their list though because there were also some strong positives (nice dining room, friendly staff, good activities schedule). Overall, I would recommend this facility but also suggest that some others in the area might be a better "fit" for some residents.

This place is DISGUSTING. Nurses are rude and ruff with the patients. They leave patients sitting in a wheelchair for hours w/o checking on them, changing them, and feeding them. If you are not by the patients side you never know how they treat them. Patients in up in worse condition or even passing away for things they could have prevented. They should be shut down. We are in the process of removing our family member. There SICK
