My mother was at Forest View Center for Rehabilitation and Nursing for two days only. She wasn't put on the right floor. She was supposed to be put on the first or second floor, but she was put in a room with someone who was very, very angry and had some mental issues because she was very hostile. Some of the staff were not nice. They told my mother one time to shut up when she was there at the facility. The meals weren't coordinated based on my mother's health. They would give her things like eggs for a heart patient. It's not every day, and I can understand it sometimes. On Saturdays, they only gave her cold food because they said it was a Jewish holiday. They never explained that to us before her going there. They wouldn't describe what the food is on the plate. You would have to guess what it was. I also noticed that when you came in, the receptionist was very, very rude. It was a man and he was extremely rude.

My husband is in Forest View Center for Rehabilitation and Nursing. We got Medicaid, so we can put him in the residents' quarters above and hopefully get him to walk to help his memory. I've gone every day since he's been in there and they've been great with me. They get him out of bed, they wash him, and they put him out in the hallway in his wheelchair. They've been wonderful with him. They are so kind and very patient. I took him to his primary last Friday in a van that they arranged for me to get. I find them very helpful and very positive. That's one of the best ones that he's been in. The staff is very patient and very kind to my husband. When they walk by him in the chair, they always kibitz with him and he loves that. They try to get the person to come out of their shell and to do what they need to do, but also take care and enjoy. He always eats his meals. They are well balanced, with soup, and an appetizer of some kind, and then they always check and see if they want hot tea or coffee when they get their meal or if they want a Coke or something. Even when they're in the hallways, they always check to see if they want something to drink, whether it's iced water or anything. I find them very caring. He's right near the nurses' station and they keep an eye on him, which is great, to make sure that he's OK. The staff has a good sense of humor too. If a woman was singing, they encouraged her to sing more and she had a great time and she probably felt like a million dollars. When I came to visit and he was still in bed, they were very patient with him getting him dressed. They came back to make sure he was washed up and dressed and everything. They're very kind. They know everybody's name. They check on them. Cleanliness has been great. While I've been there, a couple of cleaning people have come by and cleaned the room and everything, and they make the beds when people are out of the room like they don't like to bother any patients or anything with it. They're very considerate, very kind and very funny. The nurses knew their stuff because they were helping me with things with my husband and his physical therapy. They've been helping him and encouraging and they even let me stay a couple of times in the physical therapy room to be able to watch and cheer him on. They're very good. They've been excellent. They're very nice about it and they let me bring the fruits that he loves. They'll put him right out in the hallway with a towel around his chin in case he drops anything. He's been eating his watermelon and grapes and all the things he loves. They make sure that if somebody doesn't get something they like for a meal, they'll change it right on the spot and send it to the kitchen. I'm really happy. They helped me get Medicaid and we just got it, so the price was like halved. I think you get your money's worth. It's 24-hour care. If I'm five minutes over or something at night, they don't rush you out. They've been very good with him and they also check his meds to make sure he's taking them. They've done little things like put them in ice cream or pudding, so that he gets them without really feeling that he's being opposed. They've done some creative things with the nursing and the care and everything, which no other place I've known that we've been in has done. The staff is excellent.

Forest View Center for Rehabilitation and Nursing was a nice facility. It was very good. They took very good care of my husband. It was like a little hospital. It was immaculately clean. The food I did not think was as good as the other place, but it certainly was nourishing enough. He got excellent care, and the staff was wonderful and so was the management. He was on Medicare when he was in there, so it worked very well financially. The value for money was quite good.

DO NOT PLACE A LOVED ONE HERE. Here's my family's story: On Monday, November 20th, I exited Forest View with my elderly relative who has been a patient there for several months. When I arrived at the facility, my grandmother’s body was cold and she was completely unresponsive, something that should have been immediately recognized by staff had they taken the time to observe residents rather than leave them in bed or scattered across the corridor. Her demeanor was a serious departure from what I’ve observed during my previous visitations. My uncle was also at the facility and alerted the nurse to his concerns. The contracted EMT (Senior Care Emergency Medical Services) took so long to arrive that I called 911 myself and the EMTs I phoned arrived 5 minutes after the EMTs summoned by FV arrived. I have no idea why it would take 45 minutes for emergency medical professionals to arrive. Upon examination by the EMT, the gentlemen stated that her skin was cold to the touch. When we arrived at the hospital 10 minutes later, her temperature was recorded to be 90° and her blood pressure was 71/42- surely they did not both drop this low in transit. In conversations with the EMT, they alerted me that when they inquired about her vitals to the medical staff at FV, the staff was not aware and had not checked her glucose levels solely because she was not diabetic, which was alarming to them due to her age. At the hospital, doctors confirmed that she arrived in septic shock and would be treated for sepsis due to lack of proper hygiene and cleaning at FV. In fact, the urine culture found bacteria in her urine that is only found in fecal matter. She is currently being treated with two different antibiotics as the bacteria is known to be resistant. Prior to her transport via ambulance, I visited her on Saturday and advocated for her diaper to be changed when she signaled to me that it was wet. When I approached both the Asian nurse on the sixth floor and a CNA named Gloria, I was told that she would have to wait until after she eats dinner as it is difficult to feed her in bed. It appears that she is only changed in the morning and in the evening when being put to bed for the night. It was only 3:30 pm when I made the initial request. They wanted a 92-years old woman to sit in a wet diaper until at least 6pm, which is unequivocally and wholly unacceptable, inhumane and unsanitary. I, along with her medical team at the hospital, am certain that unhygienic practices like this led to her current hospitalization and diagnosis of sepsis. On another occasion, my mother had a similar incident when she told a CNA that my grandmother had defecated in her diaper and the CNA’s response was she is about to change her for bed. My grandmother was not changed for bed until 7 pm that evening. She ate dinner while sitting in her feces. While at FV during prepping for bed, we offered assistance for my grandmother. We are always asked to leave the room and now we know why. Additionally, the hospital’s medical team permitted us to see the condition of a bedsore during her examination- the bedsore appears raw and untreated. Why was this stage 4 wound not treated by FV? Adding further insult to this matter, no one at FV reached out to communicate with my family out of care or concern for my grandmother since her transfer to the hospital last Monday, illustrating a lack of empathy, diligence and compassion. Simply put, this lack of communication and overall incident is horrifying and erodes the trust we’ve attempted to build with this organization over the past few months. Had it not been for me and my uncle’s immediate response, our Christmas this year would have looked even more different than the one we just experienced. We had planned to spend the day with her at FV, instead we spent the day at the hospital with her. Without our assiduousness and care, it is likely that we would have spent the holiday grieving her due to FV's dearth of care and responsibility. AVOID AT ALL COSTS!

this facility not able to manage pt. Please read my complain. My mother was admitted to Forest View Rehab center on March 9th, because she had a fracture in her T10 vertebrae and needed to recover. She was moved there in the beginning of the COVID-19 pandemic and the center had locked down the entire building so that none of the family could visit or check in on their family. My mother did not speak English and I tried my hardest to speak to her as often as I could, but we could tell she was being neglected. When we would call her she would tell us stories of things that did occur and when i would ask the nurses or caretakers if they were checking in on her they said that everything was fine with her and she was doing great. My mother did not have a cellphone and did not know how to use one, which is fair for a 78 year old woman. So whenever we tried to get in contact through the office no one would be able to transfer us half of the time and when we were able to finally speak to her she would be begging for water or some other necessity and tell me that no one was listening to her or giving her the basic essentials. When I would call them and tell them that she needed help they would tell me that I was wrong and that she was fine or nurses are caring for other resident. One night I was on phone 4-5 hours but unable to reach anyone from facility, I believe they had no staff, but never told us about it. One day I called my mom and a woman was begging for water and they could not even give that to her when she wanted it. When my mother became fully delusional and lethargic they kept telling me that everything was okay: "she is sleeping", but I knew they were wrong. I wanted her released to my family so that we could watch over her but it was a lots of thinks to do to get her home and i was working on it, her discharge was another think and did take to long, because all needed equipment were taking time to get together. Among all of this I even asked them to check her for COVID-19 because they told us her oxygen was dropping and they refused saying that their facility was quarantined and that there was no chance of her getting it. About 2 days after they told me that it was all okay, I got a call telling me she was being sent to the hospital because she was sick with the virus and in very poor condition. 3 days after she was hospitalized she died. This facility neglected all care for my mother. No one ever tried to help her and every single time my family tried to check on her they would tell us she was okay and then tell us we could not speak to her at the moment. She became slow and lethargic and her physical state worsened every single day that she was there. They neglected my mother and I blame them for her death.

I would highly recommend this rehab center for the care of a loved one. The people there really care about the details that are so often overlooked, and took really good care of me and my family member
