I am a friend or relative of a current/past resident

Within 48 hours of admission, my brother was found lying in his own feces...not once, but twice. He called the nurses' station for walking assistance with no response. He was promised physical therapy 5x a week to become stronger, which never happened. He was unable to sit up in bed because the bed was broken and its adjustments failed to work. He was then moved to another bed after a heated and unprofessional argument in front of him by the nurses ensued. The placement and care at SB was soul-destroying for him; it robbed him of his dignity and his health declined bc of it and he died just a few days later well before his time. Do NOT send your loved one here!

f there was an option below one star I would be checking that. My 90 year old mother was transitioned here for short term care after a hospital stay for a cardiac event. She had mild to moderate dementia upon arrival and after a 7 day stay at Sarah Brayton declined into severe dementia. The staff was mostly uncaring and very rude, although there was one or two that were decent. While in a confused state on day 2, she was put into a recliner by the nurses station to be watched while in an agitated state and had a fall and was transported back to the hospital. Thankfully, no broken bones. But this is their idea of supervised care. I found her in bed on occasion naked from the waist down and wet. There was never the appropriate things needed in the room, specifically diapers that would fit her and I had to chase someone down to get these every time. She did not know to use the call button and thankfully her roommate watched out for her and called when she tried to get up for the bathroom or I am sure there would have been many more falls.The food, by the way, was of low quality, not nutritious, and served cold. After day 2 of her stay here I tried to get her discharged into the care of our family but was told that if I did that it would be AMA (against medical advice) and we would not receive any home services (which she is entitled to). Turns out this was not true according to the social worker who I was finally able to track down after 4 days. She was efficient, knowledgeable and caring. I believe she is totally overworked. We never had an admissions meeting so when I did track the social worker down, it was both an admissions and discharge meeting The nursing facility's doctor is a sports medicine physician and came in to see my mother once - very rushed and largely useless. The facility is old. My mom's room was very small, dark, and the TV did not work well enough to be watched. In addition to all of this there was a Covid outbreak and I counted at least 8 patients with signs on their doors. Covid protocols were lax and patients are not tested upon arrival (or at least my mom wasn't). Physical therapy was by no means consistent and she was never visited by an occupational therapist or speech/language pathologist to determine swallowing function. Medications were never administered on time and I had to go find nurses to get her pain medications and lidocaine patch which should have been given regularly, There was no routine in place for personal hygiene and I had to ask that she be washed. Never cared for dentures. We were told they were doing a routine chest X-Ray but were never told she was diagnosed with bi-lateral pneumonia. We learned this after getting her home and going through her meds- she was sent home with 2 heavy-duty antibiotics which made her really sick. Although then we took her to the hospital again and the hospital said she did not have pneumonia. So I do think the facility mixed up her records. In summary, this is a totally sub-par facility and I hope this review will help other families make an informed decision about their loved one's care.

My mother is in this facility after a stroke. Some staff is amazing while others have a horrible attitude! My mother is threatened that there will be consequences if she leaves her wheel chair yet they won't help her get into bed. She told them all day she was feeling sick and was left with vomit on herself till 1 am when a nurse got in shift and called 911! Horrible staff just there for a paycheck and do not care about your loved ones!

My husband was in a nursing home at Sarah Brayton Nursing Care Center. We've always been very happy with the services there. My husband enjoyed being there. He liked the service. The food wasn't bad. The nurses and aides were very pleasant. The staff was excellent. The facility was old, but it was clean, so that was the important part to me. They have therapy rooms, and they have accommodations for therapy.

I am/was a resident of this facility

While the majority of staff -- nurses, aides, PT and OT -- were very nice, the overall management was horrendous. There was no cohesion between the departments at all. One hand did not know what the other was doing. It was quite clear that the staff was overworked, there simply were too few nurses trying to care for too many patients. I'd ask for my pain medication and sometimes would not receive it for over an hour later. Again, the nurses simply had too many patients that they were trying to deal with. There were many other issues as well. The mismanagement went all the way down to the kitchen leaving certain food items, that were supposed to be included in the meal, off the tray when it was brought in. I could go on but you should be warned: THIS IS A VERY POORLY MANAGED INSTITUTION!!!!

I worked as a volunteer early summer. I was welcomed by staff and patients. My interaction with patients was excellent. Many above and beyond services for the patients we’re totally professional

I am very upset, being the fact that my Aunt has Congestive Heart Failure and the head of the beds only adjust to a limited level due to the fact their sooooo old!!! I was informed my aunt's bed was broken and it was less than 24 hours from her arrival!!! Why was she even put in it then?? In addition, whenever I required assistance regarding my terminally ill aunt, its been on average at least 15 minutes or longer before an employee would enter the room. Basic articles such as a basin were not provided upon my aunt's arrival. All require individual requests when you find the article is needed and not available upon arrival. I did experience expressed compassion from a few of the employees thank god.

Do not use this facility.. There is no case management to review patient goals and objectives. The NP does not communicate and provide any data to help the family in any way. It appesrs the PT and OT only use their checklists to evaluate and do not listen to any other objective. Discharge was not based on safety of the patient but on other reasons.

I am a friend or relative of resident

They almost killed my uncle...do NOT use this place! They canceled the ambulance to bring him to the hospital, because they have their own. He had to wait 45 minutes to get to the hospital where they found he had a fever and double pneumonia. They told us he did not have a fever and told him his was being a baby for complaining about pain. DO NOT USE THIS PLACE!

I am a friend or relative of a resident

I like the nursing care at Sarah S. Brayton Nursing Center best. They have RNs, LPNs, and doctors on the staff the majority of the time. My mother's room has a bathroom and it is a shared facility: two people to a room. The food is very good. They have their own chef and everything. They have a dining area and a community area. She is going through physical therapy. I like their attentive nursing staff the best.

My experience was good.The nurse's were all very helpful and caring.They served fairly decent meals every day, with good sized portions.They had games and activitys for them to keep busy during the days.they treated him very well compared to other places he has been in.They had transportation vehicles to take him to his appointments when need be. visiting hours were very good, i could go and see him as i pleased..The only negative thing was they kept moving him from room to room and he couldnt just settle in one room. I would recommend this nursing home to others in the future as they did a very good job overall with the patient and his family.. All in all i give sarah brayton a thumbs up.
