My mother was in rehab at Signature HealthCARE of Primacy for six weeks. Everybody was very nice and very accommodating. I didn't have any issues with anything. She was taken care of. They always kept me informed on what's going on, her treatments or medications, and even her activities. She was in a semi-private room. It had two living areas, two TVs, two dressers, and two closets. They kept it clean and sanitized. They were strict about COVID restrictions and masks. Her insurance and Medicare took care of everything. They took her to doctors' appointments. I would recommend that facility.

Rooms are small as expected. Sometimes you get an aggravating roommate, but just have to deal with it. PT Staff VERY caring ! This was the 2nd time I've been in this facility (rehab from falls in my home) Be aware, once your Insurance runs out... you're OUT THE DOOR. ha

This was by far the worst nursing home. My dad was accepted and admitted to this SNF/Rehab in early to mid-October 2021. Three weeks later he died. He was sent to the hospital several times in the small amount of time he was there. Every time he went to the hospital, he got better but upon return to Signature, he was sent back for a medical reason, each different. My father ended up with double pneumonia due to the appointed staff not turning him and moving him as they were obligated to do. Please do research thoroughly on all nursing homes before sending your loved one there.

I do NOT recommend. Worse experience ever. If you don’t care that your loved ones are left in dirty/wet diapers for hours, that end up resulting in a staph infection in the wound on their hip, or that the rehab folks don’t read the notes from the hospital so their recovery is worse then when they got there, or that their belongings are missing, well, this is the place for you. The staff level is actually pretty helpful and friendly. But when you try to bring issues to the attention of a leader she was extremely rude, talks over you, doesn’t listen to your issues and doesn’t seem to care about trying to make matters better or provide better care. We have filed a formal grievance and are working to get our loved one out of this location as fast as possible for the sake of her own safety and health.

My sister moved to Signature HealthCARE of Primacy a month ago. They were friendly and all, but you just can't see anything. Because of the virus, I guess we had to see residents there through the window from outside. Sometimes we have to talk to them out in the rain. We can't even go into the building. It was a rehab, but it's transitioning now to a nursing facility. Yesterday, we got a chance to see my sister for the first time. They obviously stick to their COVID rules; they're really strict with that, so you just can't do anything. Right now, she has a bedsore. She's in long-term care and sometimes it seems like they don't quite understand her or maybe I don't understand. She doesn't get to eat. She's being fed with a tube. I think they probably have activities, but she just gets therapy now. The facility looks average, but it looks clean. The hospital is around the corner; it's on the same street.

I was in Signature HealthCARE of Primacy for 18 months. They're very good. The staff members were very nice, very friendly, and easy to get along with. My room was kept very clean. Food was good. They had everything you could want to do.
