Residents' health and cognitive functioning is first assessed by our RN. However, it is a team approach as our Generations and Assisted Living team members receive annual dementia training to recognize early warning signs of memory impairments. In collaboration with our residents, their physicians, specialists and family members, we discuss and update the resident's care plan in accordance with their needs. Our Generations residents are re-assessed on a quarterly basis, when there is a change in status or discharge from a medical facility and return to the community.
Scheduled local transportation for medical appointments, shopping excursions and field trips. Our bus can transport 14 residents including space for 2 wheelchairs.
Our Generations Memory Care Program is designed to engage, stimulate, and empower our Generation residents. First, our Culinary Creations signature programs focuses on preparing recipes from the past. From the color of our dishware to special weighted utensils, if needed, to the presentation of our dishes, each one entices the sight, smell and palate of our residents. Healthy, chef-prepared entrees, made from scratch, are adapted to meet the dietary needs of our residents without compromising appearance or taste. Utilizing molds, modified diets are provided to our residents resembling the food they are eating. Of course, person-centered reminders and redirection, when necessary, are techniques our team utilizes to insure our residents maintain their health and wellness through nutrition.
Much like we do for our Assisted Living residents, we utilize all forms of communication with our residents' families, from phone calls, in-person meetings, text messages, emails and mail via the USPS. In addition, we may FaceTime or do video chats with family members. We also capture videos or pictures of residents to share with their families. Supplementary to formal modes of communication for individual resident updates, we share community updates on our website and social media pages, in addition to a community newsletter.
Heartis Yardley is nestled a residential setting of Yardley, Lower Makefield Township. We are close to several shopping centers and restaurants. Major transportation arteries of I-95 & US Route 1, located nearby, offer convenience for visits from our residents' families.
Heartis Yardley is a pet-friendly community. We have indoor courtyards and walking paths. In addition, Yardley and Lower Makefield Township has connecting walking paths including one our residents may join at the entrance of our community. The Executive Director volunteers with animal rescue organizations and welcomes visits with our furry residents. Treats will always be on-hand!
All team members receiving training on how to compassionately support the day-to-day needs of residents, including those with memory impairments while also finding ways to engage with them. Direct care staff members, by regulation, receive at least 16 hours of annual training related to their specific job duties. In addition, administrative staff, direct care staff persons, ancillary staff persons, substitute personnel and volunteers receive at least 4 hours of dementia-specific training within 30 days of hire and at least 2 hours of dementia-specific training annually thereafter.
Our Generations Memory Care residents and programs reside in a secured neighborhood, with a magnetically-secured entrance accessed from our assisted living area by authorized personnel. Guests are escorted into our Generations Neighborhood. Our courtyard is secured as well. Residents identified as an elopement risk receive a wander-guard device as well.

I have a relative at this facility. The facility is large and lovely. The caregivers that we've experienced have all been kind and concerned individuals. The management has changed several times so we don't know who is in charge. They need to provide the family with names and numbers of those running the facility. Beware of the food situation, the meals are meager, the quality of the food is poor and they do not provide for anyone that is Gluten Free. The menu is boring and residents have been complaining for months about the poor quality and variety of the food. This situation has not improved even though the residents pay a high amount to live there. One of the main issues with this place is the menu for dinner. For the amount of money that these residents pay, they should be getting filet at least once a week, not chicken 3 times a week.

I am writing to laud the work of several care givers: Patricia, John, Dahyana, Amelia, Doreen, Nykia, Olivia, Cameron, Thaj, Monica W. serving in the Memory Care Unit. When visiting with my wife I always receive a greeting from each one and all seem to be attentive to their respective tasks. It can be very frustrating for me to see my wife in her current mental state, but these caregivers do their best to bring some light into my dark (at times)situation.

It was fabulous. We were going there, but Dad didn't qualify. They're wonderful. They were attentive. They were terrific. The rooms were light, airy, modern, and comfortable. The dining area was beautiful, but we did not experience the food. They have everything. The only thing they didn't have was a pool.

We went to visit. It was very new and the salesperson we dealt with was very caring, and not pushy. It was just a beautiful place. The rooms had a full-sized fridge, a nice sink, and a microwave. There's no oven but you don't need it since you have all the food right there, but there's a small kitchenette in the room. It was nice. They provide three meals a day, and they have a little snack or bistro area, and it was lovely. The tour was nice. The person who gave the tour was Patrice, and she was sweet, very caring, and very energetic. She talked to my mom mostly and understood our questions. It was a nice experience. They offered us to come back and talk to the nurse and have lunch the next weekend. The facility itself was so pretty. It's brand new, the couches are nice, it doesn't have any odors, the carpets are clean and beautiful, and the windows are big and sunny.

Excellent caring facilities. The staff goes above and beyond to assist residents and family members are kept informed. The dining service and food are outstanding, and the physical therapy service is excellent

It seemed very nice. It was due to open the next week. It was not yet open when we did the tour. The rooms were beautiful. It was easy to get around in. The memory care was very similar to the other place where it is very secure. They had a nice courtyard in the middle for the residents to walk around in and spend time outside. It's a beautiful place.
