unhappy resident 2022

I am/was a resident of this facility

I wrote a better review earlier today. But I asked for my pain medicine at 410pm that I am permitted to get every six hours. I should have received it at 425pm. It is now 630 and has come in three times but the nurse is MIA. If they can not handle the work get additional help. I don't believe they discount the cost of care when they fail at their duties.

I visited this facility

My grandmother was a resident here. All the staff are caring , and go above and beyond. It looks much better on the inside then from the street. Clean, great food, amazing activities like singing, theater, painting, music, bingo, wii, trivia, and more, There therapy dept is wonderful. The nurses are always available and knowledgeable. I would recommend this facility as the best in the area !!!

My mother recently stayed at Greenleaf Nursing and Rehab Center for IV medication and rehab services, for a total of almost a month 1/2. It is the best rehab facility we've experienced. When I visited my mother there was always so much activity all around with patients in therapy department and/or family visiting them. This would put my mind at ease and was great to experience. The Director of therapy has an open door policy-which was great!! Overall the staff at Greenleaf were welcoming, helpful, and attentive. There were a select few CNAs who were not as cheerful at times, but they were still respectful of my mother’s needs. All the nurses my mother had were awesome and few went beyond their call of duty. As for the rehab, the therapists were excellent!! All... the food was not the best, sadly. However, some meals were tasty, but it's the typical “nursing home” type of food, and mom didn't seem to mind. The rooms in the facility seemed to be all semi-private & my mom’s roommate was very nice. Overall my experience was positive one and I would recommend Greenleaf to anyone who asked.

My mother was admitted to Greenleaf Nursing and Rehabilitation/Skilled Nursing Facility after suffering a stroke this past February. She was unable to walk, talk, or even feed herself but 3 weeks later she was not only talking and walking but was our old mom again. She was very depressed and didn't hold out much hope for recovery but the care she received here was comprehensive and the nurses, therapist, and staff was top notch. The room we a semi-private, which was great for mom, because she had someone to keep her company. I wholeheartedly recommend Greenleaf, they exceeded our expectations.

I am/was a resident of this facility

I recently had surgery on my leg and needed extensive wound care. I spent about 4 weeks at Greenleaf Nursing and Rehabilitation Center . Overall, I had an extremely positive experience. The nurses, aides, and physical therapists were all well-trained, professional, caring, and went out of their way to be attentive and accommodating. Everyone I met was extraordinarily nice. The facility is an older building, but don't let the interior fool you, because their care was A+!! Overall a great facility that I would highly recommend to anyone.

I spent a couple of months at Greenleaf and they did a wonderful job with helping me through my rehabilitation and recovery. The staff was knowledgeable, friendly and instrumental in my recovery. The DON was excellent!! He was very knowledgeable and was always there to answer my questions. Thank you for everything you do!! 2 thumbs up!!

We’ve made a decision and decided to move my father here at Greenleaf Nursing Rehabilitation Center, because they offered a wide range of medical services, activities and long -term care options. The staff has been extremely caring and they really take the time t to know you. However, Its an older building, but we didn't mind, because at the end of the day, its the care that matters!! Our father is extremely happy and has a lot of close friends. The building is well maintained, and close to where we currently live. I am very happy overall and I am happy we choose Greenleaf!!

My mother was recently sent to Greenleaf for severe COPD and multiple medical issues. From the PT's, OT's, nurses, docs and social workers, she could not have received better care, professionalism, attentiveness, compassion and enthusiasm. She developed a great bond with her caregivers which gave me peace of mind. I have no agenda to support this place for my own or someone else's advantage, just to offer appreciation for a job well done. I recognize this is challenging work and good medical staff does not get enough appreciation for their tireless efforts. Thanks to everyone in Greenleaf, I appreciate you all for your patience, your skills and your positive attitude with my loved one.

Our father was recently at Greenleaf for short-term rehab and our family has found the facility, staff, and grounds to be everything advertised. The staff is attentive and caring, and we appreciate the fact that they regularly kept us advised of any changes, events, etc. whether they were critical in nature or not. The caring nature of the staff, from her nurses and aides to the administrative personnel and the people who coordinate and lead the various activities.

My parents moved to Greenleaf 2 years ago and they are really happy. The staff are caring, knowledgeable and professional. The facility is beautifully renovated, close to parks, town, grocery stores, and they also provide transportation.

We choose Greenleaf for my dad because it is near our home and heard they recently made some great changes. The food is great. The support staff, all the nurses, CNAs, everyone has really been helpful to me as far as paper work goes. Everyone is just great here, and I really like it. They have entertainment, games during the day, movies, and monthly birthday parties. They had bingo last night and it was great seeing everyone involved. I would recommend Greenleaf to anyone who asks me about them

I recently had a bad wound infection and couldn't be sent home. I was offered a couple of facilities in the area, that would accept my insurance. Greenleaf was one of them. I started to search the Internet and they had a really good website. But, I was skeptical reading some of the comments, so I figured I would still give them a chance. Since, I heard they are under new management and had really good wound care. When I was entering the building, I noticed they have been doing a lot of improvements, like painting, new carpets, new therapy gym, and it was really clean. You can tell the new management is really turning the place around. Because, everyone greeted me with smiles an were there for me, when I needed them. The wound care nurse was awesome and spent a lot of time with me. In addition, the food was really good!! I stayed for a while, till my wounds healed. All I have to say is wonderful things about the facility. Hopefully, people after me will see all the positive changes, as I did!! Thank you Greenleaf!!

I visited this facility

The administrator is incompetent....not able to assist relatives in times of need....the facility is not clean, floors always dirty and the staff, especially the CNA's are always outside smoking or not paying attention to the patient's needs... I would not send my worst enemy to this facility!!!!!!!!!!!!!!!!

This is one of the worst places. It stinks of urine and is very depressing. Small rooms and terrible food, the care level is not great at all. My mother was here for rehab and my grandmother died here about 20 years ago. It was terrible then and just as bad now.

I am a friend or relative of a resident

My relative was at Greenleaf, a long-term living place. The building should have been torn down and replaced years ago. It's an old, dilapidated building. The medical staff and the therapy staff were very good. I Can't complain about them. The nurses on the floor were all registered nurses so they're good. The aides were also good. But the place was just a depressing, obsolete place, and he was stuck in a really small room with another man. Also, somebody in there stole my iPhone, and it was not a patient because it was late at night and there was only one aide on the floor. I went to the desk and I asked them if somebody turned in an iPhone and the guy there didn't even turn around to look at me. He just gave me a really curt "No." I called the cops, and I talked to the manager the next day. It was a very dismal place.

My friend is currently at Greenleaf (Greenleaf Nursing and Convalescent) for rehab. There are two occupants in a room. His roommate is a permanent resident with an emotional problem and plays the TV really loud and won’t turn it down. The roommate is not very considerate. What my friend doesn’t like about this place is that they don’t have a permanent staff. They contract out, so the staff could be different every day. My friend has to explain himself and what he needs to a new person a lot. They do have activities, but he doesn’t participate. They have a beauty/barber shop where somebody comes in once a week to cut hair and things like that. My friend has just had his leg amputated, so he spends time every day in physical therapy. One other thing that he mentioned is that sometimes it is hard to coordinate with them. He actually leaves the building three days a week to go to dialysis at another facility and sometimes the coordination with bringing him a lunch to go and having all his meds given to him and everything before he leaves has been problematic.
