"My father went to Heartland after falling out of his bed at Wheeling Hospital and having his ankle bone dislocated. He was supposed to get physical therapy and go home. If your family member needs PT to return home, this is not the place to send them. Their PT department is useless. Also, if your parent is over 80, they basically make them wheel-chair bound. They do not want "elderly" patients up and walking around. My father walked into Heartland and within a six months, was in a wheel chair and never up walking again. They took an independent man, and because of his age, kept him down. Now as for the care by the doctor, nurses and aides, here was how it went. The doctor at Heartland NEVER touched my father; did not visit with him, NEVER examined him; he only came into the facility to read thru the charts so he could bill Medicare. ALL of his care was from the nursed and aides. I was at Heartland one to two times a day and spent at least 4 hours each day of the weekend and was very involved in his care. My father was outgoing and joked and talked to everyone. We made some good friends at Heartland, and I include the nurses and aides in that list. My father died at Heartland and the nurses were wonderful to my father and my family thru the last week when things got tuff. Heartland is good if you plan on being there on a consistent basis, asking questions and making sure your loved one is always a priority. "

"I toured the facility because I was debating over two assisted living facilities. Country Club was our choice, and we are glad we did. They are very patient oriented. When we toured, they actually had karaoke going on. Everyone was having a good time. We would definitely recommend this facility."

"I visited this facility a number of times a few years ago. I thought that the staff members were very helpful and made thoughtful decisions pertaining to the residents' needs. The patient support staff were proficient in their skills and were able to meets the needs of the residents in a timely manner. The amenities were adequate in that the facility seemed to be a pleasant place for a resident to live. There is a sitting room/game room where residents can congregate as well. The food that was provided to the residents appeared to be nutritious and balanced. From what I could tell, the residents by and large seemed to enjoy the facility. Residents that could not care for themselves sufficiently were assisted in a manner that provided safety and supported physiological integrity. Additionally, other aspects of Maslow's Hierarchy of Needs were supported as well. These include love/belonging, esteem, and self-actualization. The staff made an effort to make residents feel comfortable, and they provided activities to boost achievement and creativity. "

"My wife spent the last few months of her life at this fascility and the care was much better than one could expect. Both the aides and nurses went out of their way to make her comportable as possble spending time to talk to her. It is my opinion the put quality of life into her final days and I will forever be grateful."

"Mound View Health Care in Moundsville provided us with a comfortable, affordable, and homely environment for our aunt. The staff was constantly looking for ways to make her not just live there, but enjoy herself while she did. On several occasions our large family would crowd into her room only to listen to her rant and rave about the people she was interacting with, from the staff to fellow residents. Our family would be lost without Mound view Health Care and their professional manner. I would recommend this facility to anyone looking for a place to move a loved one that can provide a friendly, homely experience. The only negative side of our experience was trying to get into the facility initially because of the great demand that they experience. Spots fill up quick because people need a place that they know they'll be cared for and that's what you get at Mound View Health Care. "

"When you walk in the staff is very friendly and ask if you need any help. They also will show you to the resident you are visiting and check up on them. The bed sheets are always clean and the rooms very tidy. There seems to be plenty of activities for the residents to do, and my grandmother was very happy there. I am very satisfied with the service my grandmother is getting and would recommend Beacon House to others. "

"I visited this facility many times to see my father and everytime I went there the staff of nurses and doctors were always polite and courteous. Anytime I ever had a question for them about my fathers care they would answer and keep me updated about his progress. One time I spent the whole days there visiting and I was amazed at the quality care that was given to my father. Anytime he needed something they were right there to help him. If I ever needed to go to a nursing home for care I would pick this one. I rate it very highly. "

"At Valley Haven Geriatric Center, the workers are very good with my dad. They know how he is, his routine, and he is used to them. We haven't had any problems. The facility is very clean. His room is very clean and he doesn't have a roommate at the moment, so he's very happy about that. He seems to be alright with the food. They do have things to do; he just doesn't participate because he doesn't want to. There are just some issues with communication with the social worker that they need to improve on. "

"We moved my in-law to Brightwood Center- West Virgina because it's close, and we figured that they have good rating, and friends and family can go see her. The staff have all been nice and answered all my questions. I call anytime, and gets an update on her. They are very professional. She is very happy where she's at. The community is very nice and clean. They have an activity place where everybody could go, and they have bingo. She may not be participating a lot because of the dementia and the Alzheimer's that she has but at least she is participating at something, and at least her mind is moving. She has not been there that long but we are really pleased with everything that we have seen."

"My wife (age 72) has been a resident at Lancia Villa for over a year mostly with physicql problems and some .slowly developing dementia, Prior to this she had been a patient at various other long-term care facilities for rehab. in Ohio and West Virginia. One in particular, in Ohio, she had been a rehabilitaion patient on four different occasions. Based on these past experiences she and I are very happy that we chose Lancia Villa for a permanent nuirsing home. The facility itself is clean and well maintained. I can not say enough about the wonderful staff of nurses and aides. The quality of care here is far and above any other place she had been..I can talk freely with the nurses and administrative staff. We are all on first name basis.. At the other places, I was ignored or they were too busy to answer questions. Due to this wonderful ability of interaction and total lack of interaction elsewhere, I am proud to give Lancia Villa Royale Nursing Home a FIVE (5) star rating..!!"

"My sister in law lived at carriage inn for the last few weeks of her life. I can highly recommend the end of life care that she was given by the team of nurses, nurses aids, cleaing staff and doctor. She was always clean. Her room was always clean. There was no smell like most nursing homes have the moment you walk in the door or the room of residents. All that attended to her needs showed a great deal of compassion and knowledge. The food was okay (nothing to write home about) but they were always willing to change the menu and address foods concerns. I highly recommend this home for those that have short term needs. I don't have any interaction or experience with longer term care to comment."

"On behalf of the Hughes family, I would like to thank the staff at Gables for the care given to my mother. After a short hospital stay, we decided on Gables as the skilled facility to get the care she needed and the therapy to regain her strength, endurance, and mobility to get her back home. Although a few health issues arose, she fought thru them and was able to continue with her therapies. I cannot thank Erica M., Danielle W, and Amy P. enough for never giving up on my mom and seeing that she received the best therapies! Also, her special aide Ava, who saw her thru some tough evenings. The kindness and compassion the staff at Gables had shown my mom was wonderful. She is now home and much stronger than before her hospital stay! The facility itself was very nice. We enjoyed the ice cream parlor and hearing a few residents play the piano."

"The rooms are moderately sized and kept relatively clean. Though the bathrooms have an odor unless an air freshener is used often. The kitchen staff are friendly and work with what they have which isn’t much. Don’t go here if needing a special diet like gluten free/diabetic. The food choices are extremely limited by corporate and the Kitchen staff don’t have much leeway. Though hardworking, Housekeeping and Maintenance staff is barely skeleton crew and issues get missed because of this. The biggest problem lies in the nursing staff and administrators. Some of the nurses are excellent at their jobs and seem to care while others ignore call lights and are rarely consistent in times giving meds. The meds are changed without warning and not conveyed to the patient even if the nursing staff is made aware. Most of the Aides are friendly and helpful. However the turnover rate for the good workers is very high. If the patient sees the Doctor once a month, they are lucky. The administration seems more likely to want the patient out unless already paying them. Don’t bother going to the social worker for anything. Patients must do it themselves if they can or go over their head if needing anything done. If there is any choice do NOT go or send loved ones here."

"This facility is often understaffed. Patients wait over 20 minutes for call lights to be answered. And, then help is still often delayed. Medications are not well supervised. Major corrections are temporarily "fixed" when the institution gets a heads up that the state is about to visit. Many of the "amenities" are a farce .Activities are often cancelled or are really fictitious. Transportation to appointments is often an issue. "

"The level of basic care behind locked doors in the Behavioral Unit of Victoria Way is consistently minimal. Often my mother has no water cup for drinking from the bedside water pitcher. On admission, my mother went many hours until we notified staff she was without any water, water pitcher, and drinking cup. Even now, we find mother left in a chair in the same position for several hours. She has developed the beginning of a bedsore from being left up all day in same position. She entered the nursing home ambulating with a walker, taking herself to the bathroom, pulling her pants down and up. Now she has fallen recently. She has no strength to walk to the bathroom. Mom requires assistance to walk to the bathroom, and someone must change her and treat her bedsore. She cannot ambulate without assistance from an aide or family at her side with a walker. And she cannot walk down a hall without getting so tired she must immediately sit down. We have found Mom with no Depends on her and no undergarments. My mother loves to sleep in flannel nightgowns, but the staff does not regularly attend to my mother's bathroom needs, so she now wets herself during the night. and they will only put open back patient gowns on her. I could go on and on. We are looking for another place as fast as we can find one to admit mother, she has dementia and Crohn's Disease. She is 86."

"My mom is in The Grove at Washington for rehab. She likes it, she said they take good care of her, they are very nice to her, and she has no complaints whatsoever (other than we can't see her, but obviously that is the case at most places with COVID). They brought her in a wheelchair to the front window so that my dad could give her some Valentine's Day gifts through the window, but that has been all. She said the food is good. From the outside it doesn't look that great. It's an older building, but it seemed clean from what I can see when I drop things off. When she was in the hospital for ten days, the hospital never told us she had a bedsore and a stroke. When she went to The Grove they found it. Now the bedsore is gone, and they said the stroke has affected that side of her brain -- the walking part. She's standing now; they didn't want to rush her. They keep in contact with my sister."
