I am a friend or relative of a current/past resident

Don’t ever bring a loved one to this location. My grandma was here for 3 weeks after spending over a month in a hospital bed and showed no improvement in physical therapy at all. Patients and staff started getting covid so I opted to get my grandma out to which they made it nearly impossible. They refused to provide her hospital discharge papers, list of medication, which medication she had already taken that day, had us call an ambulance ourselves to which we had to pay out of pocket and she also needs a bipap machine at night to which they said they could not give us the one she was using or even help us acquire one because they did not approve the discharge and could be held “liable” however, without the machine she could die. When the ambulance arrived the rehab did not provide the EMS team with any paperwork or even the clothes she had there. I’m grateful to the EMS team who wrapped my grandma in blankets to keep her warm cause the rehab lost her clothes and sent her home in the gown she wore there in 30 degree weather! I have never seen a facility meant to care for their patients behave so inhumane and disgustingly. All this just to keep her check because her insurance was no longer going to cover her stay however, she herself said many times that she wanted to leave. Many patients and staff have covid yet they still allow visitation so despite all their hinderances I’m glad I got my grandma out.

I am a friend or relative of a current/past resident

I appreciate the way the Nurses, Doctors and Nurse's Aids care for my Mother. The staff always call me anytime day or night and keep in alert of change of Meds or Health issues. I appreciate all of them for their personal care. I visit often so I get to know every one on every Shift. Social Workers are the Best stay on time of things. The Staff do several activities with the Residents, I especially like the Rehabilitation they provided made Mom stronger after leaving the hospital. Thank you.

I visited this facility

Meadow Park Rehab is housed in a small building with some very small rooms. Patients are cramped in their rooms and most of the staff are unfriendly. However there are a few staff that appear to like their jobs. During my father's stay there they constantly lost his clothes. The food appears to be o.k and plentiful. The rooms and building needs a facelift. The facility smell like mold and mildew. Also the patient's room and bathroom aren't kept cleaned. If I didn't come in and tidy up my father's room staff wouldn't have clean it. At times I felt like coming in with pine, bleach and a mop and clean the room myself. My opinion is if you place your family members in this place you better keep your eyes on them. Also I do not recommend this place to anyone.
