All the staff I encountered were very friendly, helpful, and appeared to enjoy their jobs. Whenever I visited, residents would greet me and many visited each other like they were family. Hard working staff.

My brother's room has been without heat for 7+ days. Current outside temp is 20°, 63° in his room and several other residents rooms in his area. I have called their main number, no one answers.

They have horrible staffing levels so call lights are on a long time. The food is terrible - ate a couple of meals with my friend and from appearance to taste they aren’t fit for human consumption! Cleaning is hit and Miss - tge smell is bad at times!

The staff is rude very unprofessional. The place smells of pee and feces. Staff could careless about patients just want the money. Will not send a enemy to this place. The rooms are dirty

My father was a resident of Mission Point Forest Hills late August, 2020 thru mid-October, 2020. He was admitted for rehab after surgery for a hip fracture. At 92 years of age, and an Alzheimer's patient, he was very active and able to provide general care for himself. He lived at home with an in-home care giver Monday thru Friday, and his children caring for him nights and weekends. We knew after his hip fracture, that full recovery was beyond comprehension, but our hope was that he would be able to regain the mobility he had prior to the injury. Being able to get himself dressed each day, using bathroom facilities on his own, able to move about the house with the assistance of his walker, was his and our goals. Day One of being admitted to Mission Point, should have been our first clue that this was not the right place for him. He was whisked away in his wheelchair upon arrival with barely an opportunity to explain the reasoning for him being there, and absolutely no information provided by staff at Mission Point as to what the next steps would be. There was not an Administrator on site the day of his admit. Understanding all of the restrictions of the Covid 19 pandemic, we knew our assistance in his rehab would be very limited. Day after day, call after call, request after request, and finally a face to face meeting with the Administrator, did nothing but add to the confusion and misinformation we were receiving. During my fathers first follow-up visit with the surgeon, he felt my dad had been given a muscle relaxer prior to his appointment. He was lethargic and barely able to communicate with me (his daughter), much less answer any questions the surgeon had. The records provided by Mission Point to the surgeon were sporadic with very little information provided as to the type of care/rehab he was receiving. The surgeons report back to Mission Point was to discontinue any and all muscle relaxers, to provide pain meds prior to therapy, to continue physical therapy, remove him from bed daily for meals and activities. None of this happened. Physical therapy was discontinued due to insurance no longer paying for it. We as his children, agreed to the out of pocket costs to continue therapy, but to no avail. He got weaker and weaker as time went on. Check up calls made on weekends were the worst. At times we were told he never had more than a few bites of food. Left in bed all day because he "seemed tired from his PT during the week." Physical therapy, or any therapy, was non-existent. In a call to the Administrator the next day, I relayed my feeling that they were charging my dad $370 a day, to let him die. Ms. Brady (Administrator) was offended by my statement. Phone messages left, and emails sent went unanswered. When talking with anyone at Mission Point, it as always one excuse after another. Short staffed, he seemed tired, etc., etc., etc. After his second follow up appt. with his surgeon, he advised us to remove him from Mission Point, immediately. He too felt that my dad was there to die. Extreme bed sores were discovered on his feet, heels, and ankles. The surgeon confirmed my thoughts, that my dad would never walk again. He barely had the strength to sit up in his wheelchair. His only means of communication was at a mere whisper, and the confusion was at an all time high. The surgeon recommended an appt. with a specialist for the bed sores. At that appointment, I was told there was nothing that could be done. He was too weak for treatment. The specialist felt that any form of treatment would result in amputation of his feet. She urged us to remove him from Mission Point and to seek the resources of Hospice Care. He left Mission Point on Oct. 16, 2020 and died on Nov. 2, 2020. My dad passed away 2.5 weeks after returning home. He barely ate. We, his children, and in-home care provider, attended to his every need, 24 hours a day, 7 days a week; including changing his diapers. With the help of the Veterans Administration, and hospital resources, we had every amenity made available for his care and comfort. But it was too little, too late to turn things around. He passed away peacefully in his sleep and we were all able to say goodbye. Would this have had a different outcome had we been able to assist with his rehab? I truly believe he would still be with us. Covid 19 has changed all of us, but Covid 19 can not, and should not, be used as an excuse for NEGLIGENCE. Negligence was the cause of my dads demise, and that is the fault of Mission Point. I have filed complaints with a State Ombudsman, and with the MI Dept. of Licensing and Regulatory Affairs. It won't bring my dad back, but if it can prevent another family from suffering with the negligence we did, we will rest easy.

Building is old and leaks, residents left unattended in the halls when in need. Everything about this facility is terrible. This place needs to be shut down immediately.

I walked into the facility and was immediately greeted by friendly staff. They directed me to my mother's room where she looked clean, the room was tidy and she was playing a game with another resident. My mother has been thriving here as a long-term patient where he has struggled at other places in the past. I am very pleased with her care. The only negative was that the building is old however kept up.

Upon visiting my friend at this facility, I found it to be dirty and smelly. I was very glad that my friend wasn't there very long as I don't feel like she got the care that she deserved at all.
