My stay has been very good. The nursing staff and the Cna's go above and beyond. The pt's and ot's have been excellent. I have a pretty bad injury on my left foot and I do believe I'll leave here close to 100%. Rosita has been especially kind and caring, also Betsy has been very diligent on my health and well-being. Lauren does wonderful lymphoma rubdowns. Thank you to everyone that helps me on a daily basis.

As I was coming close to the dining area to visit my mother I observed a nurse speaking very rudely to my mother and when she asked a question the nursed marched off and commented nastily " Don't start with me" Several of my mother's belongings have gone missing including her winter coat and purse. I have made several inquiries with no answers

I visited The Guardian Nursing Facility recently. I found the Staff very friendly and helpful. The building is clean and taken care of. It is bright and cheerful. All my questions were answered and I felt very pleased to feel comfortable there.

Walking into your facility I was very negative looking for every reason to not place my loved one in a long term care. I walked out after our tour with a different view which was how could I get her into the Guardian Center .The building was beautiful and the staff we ran into had a pleasant greeting to us with a smile. The Administrator is amazing, she spoke to us as if we were family and all her kind words and we never felt rushed even with all of our question. One of our hardest days was that tour has turned into a relief seeing staff interacting and the way the administrator treated us. THANK YOU

My older friend seemed to be neglected and staff did not try to understand his needs. I found him the first time alone in a very hot room without A/C and without a phone, water, or anything within his reach. He was not able to walk at this point so he relied on what others placed near him. There was no calendar or white board to indicate when his therapy was and the names of his staff. He fell twice while at The Guardian and there were no signs of measures to identify people at risk of falling and to prevent that. One fall led to a rehospitalization. The second time I found him during the day in a dark room, alone, again with no calendar, and this time not even a clock, which is not acceptable for elderly people who can "sundown" and become confused without these. Several staff on the unit seemed uninterested in the patients, although at least one of them was attentive and tried to be helpful. This place gets only 1 out of 5 stars in the Medicare rating system of all nursing homes. Please try someplace else for anyone you care about.

This facility is dirty, staff are rude and during covid were not wearing masks. The food is barley eddable. I went there for rehab after major surgery and was left waiting in pain for my pain meds for HOURS. Gun shots outside my window at night. I chose to leave after 24 hours and they refused to give me my meds. I had to explain what happened to my dr and luckily they gave me new RXs but I had to pay out of pocket for them because the facility billed my ins for the ones they didn't give me. They NEVER came to check on me or see if I needed assistance. They are most definitely out of code in many areas and I have already filed with the appropriate authorities. Do NOT ever go there or send your loved ones there.

Nursing care was excellent they checked in with me and provided help 24/7 The therapy staff got me back on track in days !! I would recommend this nursing facility

My brother John had been in the Guardian Center in Brockton, Ma for 2 years. He was a young man with a severe obesity problem. John was over 600 pounds and this facility made him feel like he was 150 pounds. Never was he treated badly by any staff. At Holiday time the staff got presents for the residents and had a Holiday tree for them. He had a nurse named Karen and she was a doll. Because of His size the facility demanded that Medicare get him an appropriate bed. They got a king size hospital bed that raised and lowered and allowed him to get up without assistance. They also ordered him an extra wide wheelchair so as not to make him feel cramped or uncomfortable. He enjoyed his stay there and it really was a turning point in John going home to an apartment and living on his own again. Being a large Man in a built for small people world was very hard, yet the Guardian Center made John feel like he belonged.
