My mom is doing rehab at Forest Creek Village in Indianapolis. Overall, the experience is very positive and we are very much pleased with my mother's progress. When she was at another facility, she did not have this kind of progress. Here, she is making connections and getting involved so I like how they're taking care of her. They have some issues but overall, it's been very positive. From the outside, it's nice but it's an older building and they need to work on their housing maintenance. Like closets that don't close and little stuff that needs to be worked on. I think they allow that to go a little too long before it's fixed. The housekeeping needs to keep her room clean and swept. It's not that bad, they eventually caught up with it, but I think they're shorthanded. Overall, I would say they're still a pretty clean facility. Sometimes they run out of things too, like one time I saw the staff running out of trash bags and they didn't have the solution to clean the tables. Little stuff like that, they need to stay on top of that. They have a good staff there. I haven't had any bad experiences with the staff. They're all caring and they all take care of her. I just don't like the maintenance part and the building upkeep. That's a problem with me. They also need to work on their employee's morale up because they have really good employees. The rehab part is great though. The staff is attentive and I found quite a few people who are very, very good staff. The administrator, Tom, is also very attentive. He listens to our problems, as well as the social worker. They have an advocate there too, her name is Jessica. I know that if we have a serious problem, we always know who to go to. If we need help, they usually respond. They just need to have more people. My mother loved their food because she goes back for seconds. If they have, she will go and they will give her some more. One day she cleaned up her plate so I would say the food is really good.

I would never send your family or friends here, not even your enemies. Not only did they not pay attention when my mom said something was wrong, they also didn't do what they were supposed to when she went into cardiac arrest. With two nurses in the room when it happened, they delayed CPR by minutes which proved fatal. Now, 6 weeks later, they are trying to bill us when I know for a fact her insurance covers everything. They have absolutely no compassion. Please, by all that is good in the world, do not go to this establishment.

We visited Forest Creek. The staff was friendly and caring. The rooms were small for sharing.

Dad almost died of malnutrition and neglect. It is dishearthening to see a place that suppose to care for someone who truly need help and NOT get it. All that we ask for is care and compassion for our love one. The actions of the staff showed otherwise. Just remember KARMA will get to you. We appreciate all that you've done for dad all the time he was a resident here until now. We learn a great deal from this experienced. We will make sure he will be in a place that truly care.

My wife's great-uncle was a resident at this facility for a little over one year. We had him transferred to this facility after a drug overdose almost proved fatal at the first one he was a resident at. The staff was amazing, the nurses were all extremely kind to both his family and Uncle, who was the stereotypical grumpy old man. He would yell at the nurses and make a mess on purpose and they just laughed it off and provided amazing care regardless. They encouraged us to visit whenever we wanted at any time during visiting hours and even let us stay after visiting hours had ended. He was allowed to leave to be with family on holidays when he was able, and was even encouraged to do so. After his passing, the facility did everything they could to help the family through the grieving process. The facility itself was always pristine and very modern, and each room had a television, which the residents seemed to really enjoy.
