Welcome to Charlottesville Pointe Rehabilitation and Healthcare Center. We are a 180-bed skilled nursing facility that includes a 60-bed secure Alzheimer�s Unit. Our center provides a wide range of skilled nursing and rehabilitation services for both short stay recovery, as well as ongoing supportive services for individuals requiring long-term care. Our focus is on improving the quality of life for those we serve through our orthopedic, neurological, cardiac, pulmonary, wound care services as well as interventions for other medically complex diagnoses, admitting 24 hours a day, 7 days a week. Facility overall is ok. It was one two options available. Physical therapy staff were good, but the nursing care and billing were god awful. They would tell you one thing and do another. After the first few weeks, we had to make sure someone from our family was their to be our fathers advocate. Most of the staff was brand new, very inexperienced and probably under paid because of lack of attention to patients. There were no amenities, other than a room with puzzles and a tv. They had a dinning room that no one used and all meals were delivered to rooms. We consistently asking staff for medications, and help with everyday tasks.

My father was there for rehab for two months. The facility was not clean. My father was not kept clean. He was not properly cared for. I regret him ever being there.

HORRIBLE! Flies, lots of bugs crawling on the floor, no soap available... they are using POTTY chairs as wheelchairs!! When you call for your family member, the routine, robotic answer (if they answer the phone at all), is "They just took pain medication and they are sleeping. They will call you back." [hang up] How can they call back if they don't even know WHO was calling?? Thank fully, my father had his own cell phone. When I got this answer, I called him directly -- to make arrangements to get him out of there immediately. How are they still open???? I had to rate at least a 1-star to submit the review, but I'd never rate it even that high. Where is the Health Department??

Trinity Mission Health and Rehab was clean. I would not have let my wife go back the second time if it wasn't clean the first time. The food wasn't a problem. As far as therapy, she wasn't able to take therapy while she was still on IVs. I don't have any problem with them or I wouldn't have gone back. I went with the administrator one time, and I talked directly to him. He even put me up in a motel for 5 nights. Trinity is the only one I still trust and I have no one else to recommend besides them.

My mother stayed at this facility for around 2-1/2 years and they were the worst years of her life. The place smells like urine and poop!!! They do not help the patients eat or they are called and don't respond to your call. Mom was pushed three times by a patient. We complained and wanted this patient moved from where Mom was. They said they couldn't do it. Well, the third time sent Mom to the hospital with a fractured hip. I have seen patients with cuts on their body and face where they have fallen. All that place is, is for someone to spend the night, then who knows what occurs. Please, Please do NOT send a loved one to that facility. It should be shut down. I can't believe that facilities like this are kept in business. My Mom now resides in a great Nursing Home that really cares for her well being!!!!!

This facility is the worst in Charlottesville. The staff is uncaring and are particularly inept. The staff here has on more than one occasion caused fractured limbs, missed major falls and failed to perform CPR on patients. It's not really a "healthcare" facility, so much as it is a lockup for the old and poor.One aspect that will immediately strike a visitor is the smell. It's a dirty cleaner smell - the sort that occurs when dirty mop water is used. That is compounded by the normal nursing home smells and sights. The facility is outdated and the staff has trouble with the patients in wheelchairs.They also don't change the sheets on the beds with any regularity, or even when soiled. One patient reported going a week with a soiled sheet and repeatedly asking them to change it. This is not a good place to end up. I wish there were some positive comments to be made about this facility, but the staff have terrible attitudes!

Provides no stimulation for residents. Monthly birthday celebrations have been cancelled. Outings are twice a year, if that. They are always cancelled. They let their contract with Comcast TV elapse and now most residents stare at the wall for eight hours. It has gone downhill so unless you absolutely have no other place to go, or your family member is at the point of not understanding their surroundings, the go somewhere else.
