My grandpa was at Forum Parkway. It was really nice. They have game nights, they have speakers that come in and talk with them, they have little meetings to go over plans, and they have different activities to get out of the room to meet up with each other. He said the food was great; however, it was an older place. They had studio rooms. He had a roommate. There were two to each room and he didn't like that. He liked the staff members. It wasn't too expensive, but not too low either.

My parent was in the skilled nursing/rehabilitation wing. The rooms are spacious. The facility is clean and nicely appointed. The food is fair; but did not seem super nutritious. The CNAs/Nurses/Med Techs/Aides who helped my parent were kind and caring. The front desk staff very welcoming and friendly. While the other staff (Director of Nursing, Director of Rehabilitation, Social Worker, and Business Manager) was willing to meet with me while I was there; two did not respond to emails that I sent as follow-ups to in-person conversations. They are very very short staffed on the care side (CNA/Nurses/Aides). There could also be a structural problem regarding communication from the PT staff to the nursing staff and between the nursing shifts -- one shift had my parent trying to stand when they did not have the strength to do so. The Asssistant Director of Nursing did not to take responsibility for the poor communication and the inconsistent care. My parent regularly sat in a wet brief or a wet bed. There were not enough people on the floor to help do physical transfers to the wheelchair or assist them in the bathroom or with physical care (hair brushing, teeth brushing, getting dressed). My parent laid in bed more than they should have during their stay and as a result did not improve very much. On more than one occassion there was a single person on the wing to deliver and pick-up meals, assist patients, etc.... you cannot be angry at the worker because they are doing their best and working very hard without enough support or assistance. The PT studio was big and well-equipped. The PTs working with my parent were good; but there were not enough aides and CNAs to help reiforce skills and build strength and endurance when out of the PT Gym. For example, helping patients to transfer into the wheelchairs regularly or assist patients to transfer to toilets. Several times, I waited between 20-40 minutes after pushing the call button so we could request a transfer from the bed to the wheelchair. While Forum promoted their activities and dining areas when I toured the facility, there was not enough staff to assist my parent to take advantage of them. When my parent was transferred to in-patient rehabilitation center they had a stage one pressure sore on their food and a terrible skin rash in their private areas due to inadequate care due to being short staffed. They also had a UTI most likely due to inadequate cleaning. (The In-Patient Rehab nurse told me that Medicare will not pay a facility to treat pressure sores that occur in a facility because they are so preventable). Again, the nursing/CNA/aides were all kind and caring and I had no complaint except to feel bad that they did not have the support they needed to do their job in the best possible manner and in my parent's case it delayed their success in returning to their home.

I have been requesting a single or private room since I have been here and no one wants to help me. The roommate doesn’t understand that there are 2 of us and we have some issues

The care my relative received at this facility was terrible. The staff was not prepared for my relative's arrival. None of the required medication was available. When I inquired about this at 4:00 pm in the afternoon on the day of arrival I was informed that the Hospital had not submitted the medication orders. I called the hospital attending physician and found that the medication order had in fact been submitted at 1:30 in the afternoon. So my relative had no pain med for the entire night. At 4:30 am the next morning I removed my relative from this facility after overhearing a nurse/aide tell my relative that she could only change here depends every two hours. I was on the phone with my relative when the nurse/aide walked in to the room after my relative waited 50 minutes on someone to assist her. DO NOT TAKE YOUR LOVED ONE'S to this deplorable facility.

My father is at Forum Parkway Health & Rehabilitation. I don't like this place. They need probably more staff. The staff did not seem to be as attentive, and they were not as motivated to work with my father as much to get him back up from a PT standpoint to doing what he was before in my opinion. The food was OK. It wasn't great, but it was OK. It's pretty consistently the same type of stuff for three to four days. I would give the facilities a 3 because there were times when the rooms needed to be cleaned a little bit better. They just didn't seem to be very organized. It's located close to where my mother was staying so she could go back and forth easily because she was the one that was there. It is near the hospital he was at before going there.
