"Be absolutely sure the doctor writes ALL of your in-house orders and discharge orders prior to his disappearance. They are good at leaving a lot out. Also, I was on 2L of oxygen while there; they discharged me to home without any. Said I didn't need it according to what was recorded in OT chart. Said it was recorded above 88%. Either they falsified medical records or lied because my O2 saturation was 81 - 85% while walking. They also marked on my discharge paperwork that I didn't participate in therapy, which I did. It was the primary purpose why I was sent there. Now my insurance doesn't want to pay for it. WATCH BUSINESS / MANAGEMENTS EVERY MOVE. Nurses and some nurse aides are fantastic. Therapist are pretty fantastic. Main cleaning lady is good. Meals are ok; could be a bit more filling. Aides keep up with ice water in your room; which mattered to me. My bed was comfy; room was kept clean. Bathroom is shared between 2 rooms / 4 people. Climate control could be a touchy situation if you are in the bed by the window where the heating /cooling unit is."

"The rooms are large and staff is friendly. The skilled nursing floor is clean and welcoming. The nurses really are awesome and the aids are wonderful. Any requests I had on behalf of my family member were met as best and possible. Not much in the way of amenities but the staff more than makes up for that. "

"Haven Crest in Monongahela, PA gets four out of five stars from me! My grandfather was alone and always a pretty independent guy, but eventually he needed to give up his house and get some 24 hour help. He didn't want to live with us, instead preferring to go to a nursing home where he could be looked after by professionals. Fortunately, the place is only about ten minutes away from us, so we were able to come by as many times as we wanted. And overall, the people in my family were pleased with Haven Crest. I would say that this facility is pretty good. The staff has always been friendly and upbeat with me and my family when we would visit, and they were also very nice to my grandfather during his stay here. The place itself was a bit rundown, but just small cosmetic things like needing a new lick of paint here and there. Even so, the facility was quite clean and the folks who lived there all seemed to be happy and well taken care of. "

"My dad has been in WeCare at Waynesburg for a while now. The people are really on the ball. They seem to take really good care of my dad. I don't really have anything negative to say about them. They seem to be on top of things. His room is nice and pleasant. The place seems to be pretty well kept. They are helping my dad pretty much with everything, like feeding, medications, and personal care. I have not tried their food, but it had been delivered when I was there, and it smelled wonderful. The facility seems to be clean any time I've been there, and they seem to have a lot of activities and programs. My dad is to the point where he really isn't able to participate too much, but they are available. He seems to be getting pretty good care there."

"Care is provided by an excellent. Food is abundant and generally good quality. The rehab staff are quite good. It is a secure facility and kept clean...no odors detected. Rooms are a little small, but well kept. They provide entertainment and social activities. Live music is sometimes available. They do have an activities room that was also clean."

"I spent time at Mapleshire both as an acute care patient and as an outpatient for physical, occupational, and speech therapy after suffering a stroke and heart failure. The nursing staff were all highly skilled, attentive, and compassionate. The therapists were also all highly skilled, and determined to achieve as much rehabilitation as I was capable of. They provided an individualized vigorous and challenging workout while at the same time maintaining a kind and at times humorous environment. The rooms were comfortable and quiet. Everyone from administration, front desk staff, nutritionist, kitchen staff to cleaning personnel all appeared focused on providing as comfortable and healing an experience as possible. "

"I was too there for 3 months as a patient. The people on staff were always friendly and helpful. The therapy staff is excellent. Food was fair. There were some missteps with my discharge protocol and paperwork that could be handled better."

"I was in rehab at Encompass Health Rehabilitation Hospital of Morgantown. I was there for about a month. The care was spot on, and I had very good caregivers. My PT was the primary person assigned to me, and she really advocated for me when I had an issue with something. She really kept me going, attitude-wise, because it was a bad leg break. The food was also excellent. They have outdoor spaces, and the gym is the physical therapy room. We had three hours of therapy a day, and it was very difficult, so mostly, I was just resting when I wasn't in those three hours of therapy and eating. The day was just kind of taken up between therapy, which started at eight or nine, and around meals that could go till four or four thirty, so it was a pretty full day. I chose this facility for myself because my dad had also been there, and I liked the way they were with him. The therapy team kind of pulled him into the routine, and they did a good job with him."

"My mom was a resident at this facility. The staff treated her with dignity and respect for the entire two years she was there. Mom had multiple disabilities and was confined to a wheelchair. She was always clean and never had an oder about her which was no easy task as she could not self toilet. I never heard the staff raise their voice and no task was to much. I strongly recommend this facility. "

"My father was recently a patient here. He had terminal cancer & was there for end of life care. Every employee we encountered was absolutely fantastic, not only with my father but also with us, his family. They were more than attentive & were there for anything he needed but also whatever it was we needed. They went above & beyond for him & us & they made the heartbreaking situation of losing our father a little easier knowing he was in such amazing hands. There are no words to express our gratitude for what they did. You guys are amazing & again from the bottom of our hearts, we thank you. We will never forget what you did for him most importantly, but also for us."

"Transitions Healthcare North Huntingdon was fine. It seemed friendly, clean, and well organized. My dad talks very highly of the staff and say he likes the food. When I was there one day, they came in and asked him what he liked and what he didn’t like; he liked that because he’s a very picky eater. My mother is in and out of nursing homes and I would say Transitions Healthcare North Huntingdon is probably the best one I’ve ever been in. "

"Rehabilitation and Nursing Center was a very clean community and they did their best to take care of all their patients. They're just understaffed and overwhelmed. The grounds and location are very nice. My mother's room was nice and big enough. They have activities for the seniors as well."

"I was so upset at first sight of this "highly rated" facility as we brought my loved one in. There is not enough staffing, and the aides are worked too hard. There are caring individuals, but almost 95% of the time your loved one is left to wander alone. No one can be taken to the bathroom while dinner is being served, while they take them to dinner, or bring them back. Elderly people must WAIT till staff is ready for them to go. Not the cleanest place either. Not enough liquids given for proper hydration. WAY too many people in the building- no personal care for anyone! Anyone who has made progress on recovery from a stroke or heart attack or injury will backslide rather than progress with healing. There is NO future in this place! Very Depressing and truly difficult to remove a loved one from once they are there. "

"The rooms could use a paint job to brighten them up. The staff is always kind and accommodating when I need assistance with my family member. It is always festive looking during all the holidays for the resistant's and they definitely make an effort to try to improve the place."
