"I was in a rehab facility for three months. Everyone was extremely kind to me. They all practically cried when I left. The whole stay there was very, very enjoyable for me. The staff are very attentive and very good to me. The food they serve was great. I would recommend Four Seasons. "

"I have a close friend who recently underwent a rehab stay at this facility to deal with a fractured pelvis. Within a week this facility demonstrated all the strengths and weaknesses of the for-profit American health care "system", beginning with a staff that's largely trying to be very caring but too often is simply rather overwhelmed. Compared to the staffing average in California, the ratio of licensed nurses to patients is rather light, with LVNs and CNAs pretty much handling all the hands-on nursing; based on this patient’s experience, don't expect to ever see an RN in your room. When things get busy, a patient's call light can sometimes not get answered for a very long time (I know, this is not uncommon pretty much everywhere, but it's still far from ideal). There's no doctor on staff; a physician's assistant comes through once a day and reports back to an off-site doctor (neither of them actually works for Windsor, but for Regal Medical Group, which has a very cozy relationship with Windsor). The PT/OT staff, also provided by a 3rd party vendor, are warm and friendly, but in the instance of this patient a mistake was made early on when a modality inappropriate for a pelvic fracture was tried, resulting in severe pain that didn't subside to pre-admission levels for days. I will say the place is clean, kitchen staff delightful (food is nursing-home serviceable), the valet for the too-small lot in back goes the extra mile with a winning grin, and everyone we asked for help with something was helpful even when in the middle of juggling other duties. There are two interior garden courtyards and a small dining/rec room as well as a good-sized main dining room for mobile patients; movies are regularly run on the large TV there. Each bed has a small TV on a boom with headphones to go with it, allowing for patients to be able to not have to hear their roommate's choice of show - much appreciated. There is a full calendar of activities for those who can participate. But expect to see, and hear, long-term inhabitants along the halls much of the day."

"My husband is now at Country Villa Wilshire Healthcare Center. It is not far away from my house, and I was really impressed. I went to visit it, and I liked it. They really take good care of my husband. If you need something, you just can ask them. They will do it for you. I'm very happy for him over there. The appearance is very clean and very nice. The nurses are very nice, and they help you. So far, so good. I'm happy. They have activities. They have a nice and beautiful patio. They have all the equipment. Even for someone who cannot get up, they can lift them to shower and to get them out. They are really nice for the people that cannot walk. My husband is bedridden, but I know they have activities. He has only been there for three weeks. From what I have seen with them, I'm happy with it."

"While this facility is not in the best neighborhood, nor have the best curb appeal, I could not be much happier about the location my grandmother has been living for over the past five years (and actually it is probably closer to 10 years at this point). She has always been well taken care of in her time there. Suffering from Alzheimer's, she cannot necessarily let us know the level of care, but during all visits, she is clean, her wounds are taken care of, and her needs are being met. The staff has been very helpful and kind. They are respectful of family wishes concerning her care, and also communicate any health concerns as they arise. This facility has been a blessing for my family. Not only is my grandmother well cared for, but it has not taken every penny the family has collectively to ensure such excellent care. I would recommend it to friends and family. "

"Grand Valley looks a little crowded and it's kind of fast-paced. The staff does what they can. The rooms are all shared. The food was pretty decent. They have a list of activities but I don't see them really pursuing all of it. My mother is there mainly for physical and occupational therapy."

"I'm very happy with The Care Center on Hazeltine. They're taking very good care of my mother. It was recommended from the in-home hospice care that she had. It's very clean. The staff is very friendly and very helpful. They have her on a liquid diet that my mom's not crazy about. For other people on regular diets, the food is a 5. They take her every day to an activity room. She doesn't participate in anything, but she sits there and watches. When I go visit her, there are always a lot of people in there, either playing cards, watching TV, or playing checkers. I'm very happy about that. They have somebody that comes and take care of my mom's hair. She's had somebody come to put nail polish on her. She's not active in anything, but for people that want it, they show movies and they do outings. Right now, we're not paying for it, it's covered by Medicare. After this weekend, Medicare won't pay anymore, and it's just under $195 a day after Medicare stops paying, so I believe that's a little on the high side."

"It's a clean center and there's no smell. My mother had been in a different rehab center before but Burbank Healthcare has no urine smell at all. It's very clean and the walls are shiny. There's wallpaper on the walls and it's just comfortable. The staff are very nice. My mom is happier with the place compared to the previous one. I haven't eaten there yet but the food looks good also. "

"I had a friend at Sharon Care Center, and they weren't very good. It's a rehab center. The people were very nice. Maybe my friend was just demanding, but it was kind of hard for her to get their attention sometimes or to get therapy. Therapy was really an issue. She was there for a while, and she wasn't under any therapy for a long time. She didn't like the food, so other friends of mine started bringing her food, or she ordered food. My friends would bring her fruits and the snacks that she liked. She would send food back. I think the chef might have gotten a little upset because she would be sending everything back. Her room held three beds. She had to go to the hospital, and when she came back, she had to go to a different room. When her old room became available, they put her in it again. The upkeep was pretty good, but the parking was terrible. You have to park on the street. There's a gym across the street, and on the side street is a residential area. It looks like it was permit-parking at certain hours. It looked like you could get blocked in, or you could block someone else in. During certain hours of the day, it was valet parking. It also wasn't leveled, so it was kind of on a little hill. It was not user-friendly at all. I didn't see much activity going on while I was there. I also didn't see too much interaction. It could be because of my friend, who wasn't engaging in anything. I remember they had a COVID outbreak there, too, and it didn't seem like they contained it enough. I thought that instead of the little barriers, they could have had more plastic around, or they could have let everybody who was coming to visit know that there was an outbreak."

"I am currently at this rehab center and apparently between 2010 and now a lot has changed. I was sent here after a botched surgery which then required emergency surgery to correct and put me in a 3 week coma. I lost my ability to walk, write, my speech was slurred and I lost 45 pounds. This is also a senior care hospital. Their rehab center is excellent and is helping me to recover my walking, speech etc. As for the rest, my nurses are friendly, very attentive and quick with care. A friend who visited who has done volunteer work in a number of senior citizens homes said this is one of the nicest he's seen so clearly things have improved since the previous review. "

"My husband was a resident of Berkley Valley Post-Acute. They were neglectful of my husband. He had many things going on and they didn't pay attention. The physical therapy people were the only ones that were good. The rest of the staff, I had to bring their attention to everything that was happening. My husband wouldn't eat their food, so I had to go shopping and bring him food, and he ordered food. The food was not good. They did have what they called a rec room where they played bingo or whatever. As far as an exercise room, they had a physical therapy room for the people who were there for rehab and physical therapy. The physical therapy rooms looked fine. They had equipment and I guess I'm going to say three on that. My husband was in a three-men's bedroom, which I thought was terrible. So I'm going to give the rooms a one. They're outrageous, from what I understand the insurance paid and then I had to put him on Medi-Cal, and it seems like I don't know what they charge today, but it's a lot and it seems way out of line. I believe it's $500 a day for a three-bed, for whatever they do, which is nothing. So I have nothing good to say about that facility."
