"I have a close friend who recently underwent a rehab stay at this facility to deal with a fractured pelvis. Within a week this facility demonstrated all the strengths and weaknesses of the for-profit American health care "system", beginning with a staff that's largely trying to be very caring but too often is simply rather overwhelmed. Compared to the staffing average in California, the ratio of licensed nurses to patients is rather light, with LVNs and CNAs pretty much handling all the hands-on nursing; based on this patient’s experience, don't expect to ever see an RN in your room. When things get busy, a patient's call light can sometimes not get answered for a very long time (I know, this is not uncommon pretty much everywhere, but it's still far from ideal). There's no doctor on staff; a physician's assistant comes through once a day and reports back to an off-site doctor (neither of them actually works for Windsor, but for Regal Medical Group, which has a very cozy relationship with Windsor). The PT/OT staff, also provided by a 3rd party vendor, are warm and friendly, but in the instance of this patient a mistake was made early on when a modality inappropriate for a pelvic fracture was tried, resulting in severe pain that didn't subside to pre-admission levels for days. I will say the place is clean, kitchen staff delightful (food is nursing-home serviceable), the valet for the too-small lot in back goes the extra mile with a winning grin, and everyone we asked for help with something was helpful even when in the middle of juggling other duties. There are two interior garden courtyards and a small dining/rec room as well as a good-sized main dining room for mobile patients; movies are regularly run on the large TV there. Each bed has a small TV on a boom with headphones to go with it, allowing for patients to be able to not have to hear their roommate's choice of show - much appreciated. There is a full calendar of activities for those who can participate. But expect to see, and hear, long-term inhabitants along the halls much of the day."

"Station 2 was the Station to be in! After the period of readjustment with the help of the admission staff who took it upon them self to make it easier, for someone like me who really did not understand WHY it was important for my recovery to be there,by taking extra steps for me to settle in and settle down. Station 2's Charge Nurse was able to educate me on what was expected of me in order for my CNA's and all my attendants, designated to my room (which I had to myself at the time) for a successful recovery. Side note, I was not the only patient under their care on any given day. They have a Physical Therapy Department which is vital to your recovery, unbeknownst to me. Having them PATIENTLY, work with you to execute exercises, educate you on the difference between Physical Therapy and Occupational Therapy and why both are vital for the recovery process.Nutrition and meals are a constant work in progress, you are given alternative meals than the regular menu, however if you take the time to speak to them regarding your preference on a more specific nutritional diet, they will accommodate with no problem. They offer daily activities that you can sign up for or just attend, movie night, arts and crafts and faith services and more. Designated timed smoking area is offered. Visiting is allowed Monday-Sundays with weekends are big family days. With that said , Everyone that helped me in my recovery while staying here and the EXTRA steps they took to make me understand how vital it was for me to cooperate with them not AGAINST them, in order for them to do their job efficiently. I was not the easiest person to deal with upon my arrival at this facility. Refusing to understand, listen or cooperate with them.Their diligence, patience, extra steps, and effort for a full recovery was accomplished in my case , so with that, I have to give them a thumbs up. Super Great Job!! Now with that said, this is MY OWN OPINION and EXPERIENCE writing this review. I stayed at this facility from the end of September 2023 to Mid December 2023"

"I'm very happy with The Care Center on Hazeltine. They're taking very good care of my mother. It was recommended from the in-home hospice care that she had. It's very clean. The staff is very friendly and very helpful. They have her on a liquid diet that my mom's not crazy about. For other people on regular diets, the food is a 5. They take her every day to an activity room. She doesn't participate in anything, but she sits there and watches. When I go visit her, there are always a lot of people in there, either playing cards, watching TV, or playing checkers. I'm very happy about that. They have somebody that comes and take care of my mom's hair. She's had somebody come to put nail polish on her. She's not active in anything, but for people that want it, they show movies and they do outings. Right now, we're not paying for it, it's covered by Medicare. After this weekend, Medicare won't pay anymore, and it's just under $195 a day after Medicare stops paying, so I believe that's a little on the high side."

"My mother was at Sylmar Health Rehabilitation Center. She was in a room with two other patients, and it was tiny. The staff was very good, very nice, and very supportive. The dining area was clean. I guess they could improve the ambience a little bit to make it look less institutional. "

"The facility was surprisingly large for what initially from the outside looks like a block of apartments. Trees and grass surround the brick building, on some sides the trees cover the whole building. Adds a lot to the atmosphere of being a quiet peaceful place, the theme is continued on the outdoor areas inside the complex with palm trees in the patio. There is even an option of gardening as an activity program. The hallways were neat and sterile, areas of the complex did feel like a hospital, but still a well maintained one.The entire staff was friendly in person, from the nurses to the directors. Things seemed a bit unorganized at times, but every staff member I passed by with the nurse I was talking with wanted to help. I did not interact very much with doctors. As for the older adults, most seemed understandably tired and depressed but there were some lively people. One older man struck up a conversation with me about his injuries, we talked and despite his need for independence he did not appear to be a lonely person and he seemed to be in good spirits and hopeful. Not that there weren't a fair share of people that did appear to be lonely and sad.There is nursing service available for different kinds of needs for the elderly from psychologists, pain management, individualized care plans, nutritional care, transportation, medication management and education to hospice program. There is care for Alzheimer's patients, sufferers of diabetes and treatment plans that are included in the board and room fee for bowel and bladder training, rehab, strengthening and other restorative programs. There is occupational therapy for day to day living as well as numerous physical therapy treatments and speech therapy. There are also activities like singing, fitness, music, games. They also have socials services for assistance during transitions to and out of high valley for services like dental, vision, podiatry and more. "

"Panorama Gardens was fantastic. I wish the place was more broad, but you got to do what you have. They were very professional. They know what they're doing. The only problem is that they might have to get more specialists from time to time if you go in there, because my mom got better with their treatment, but the underlying health problem that she had, they couldn't see it. For example, my mom had kidney failure and she had pancreatic complications, which sad to say, the nurses don't see it. That's the only problem I believe that can be fixed and improved in time. Other than that, they were nice to me. They had activities. They had multiple types of coloring books, brain activity courses, a lot of socialization, and events as well. They give open access to the patients to reach out to their families and loved ones. They have freedom of movement to go around to the facilities to exercise and stretch out. They did the best thing they can do as a rehab facility. I don't really have any cause against it."

"My mom had lived for a short time in North Valley Nursing Center. We were very happy there. The staff had very professional ethics, and the nurses were very helpful. The room was a double occupancy room, though my mom did not have anyone in the room for at least two or three days, and then someone moved in with her. I would recommend it to others. "

"My husband is at Sherman Village Health Care right now. It's a lot better than the hospital, and they're really attentive at the rehab center. I haven't had any negative experiences with the staff. I don't have any complaints about the place."

"It's a clean center and there's no smell. My mother had been in a different rehab center before but Burbank Healthcare has no urine smell at all. It's very clean and the walls are shiny. There's wallpaper on the walls and it's just comfortable. The staff are very nice. My mom is happier with the place compared to the previous one. I haven't eaten there yet but the food looks good also. "

"My husband was recently a resident at Windsor Terrace for ten days. The care he got was outstanding. The nurses are very caring. The physical, occupational and speech therapists, as well as the wound specialist are excellent. The rooms are clean and well kept. The nurses are responsive, coming quickly after they are called. Previously, my husband was in two other rehab facilities that did not measure up to Windsor. Everyone at Windsor tried hard to make our whole family feel comfortable and at ease. Muty really went the extra mile. She treated us like we were her own family. Demetrius (PT), Erlinda (OT), and Estella went out of their way for us. Administrators Ana Tolossa and Cristina Castro couldn't have been more welcoming. They did whatever they could to bring about a smooth transition. They were kind and empathetic. We are very grateful that we found Windsor. "
