Oak Pointe of Carthage provides staff for the Memory Care Neighborhood 24-hours a day. The neighborhood staffing depends on the number of residents who make their home there. Our regulations dictate the number staff required for a memory care specialty neighborhood. Oak Pointe of Carthage ensures that we meet or exceed all required staffing for our Memory Care Neighborhood.

Accepted Answer

Oak Pointe of Carthage is not directly affiliated with any hospital system. However, we do have a strong relationship with Mercy Carthage, located less than a mile from our community.

Oak Pointe of Carthage has a variety of shops near by to include, nail salons, banks, and a shopping center.

Oak Pointe of Carthage provides complimentary transportation to and from medical appointments with the city limits of Carthage.  We also provide transportation for our planned shopping days and scheduled outings

Regular exercise classes, interaction and socialization with other residents, offering ancillary services such as massage therapy and medical pedicure.  Planned daily activities, animal therapy and music therapy, outings to promote community and involvement.  A restaurant style dinning to promote healthy choices.

Our dinning offers restaurant style meals and are prepared individually.  This is an opportunity for the residents to enjoy their meal with friends and family. The attention to detail, quality of ingredients and the most important part; all meals are home-cooked.  We work very hard to maintain that the residents choices and preferences are carried out.  Every meal consists of a “Special of the Day” or they may order a selection off the menu.  We also offer a soup of the day.  Specials include dishes from other cultures and good home cooking.  Menu items include sandwiches, salads and soups.

Oak Pointe of Carthage has a diverse and dynamic resident population. Residents come from all walks of life from teachers to bankers and nurses, professors to housewives and military spouses. These diverse backgrounds give our community a rich and robust group of people who quickly become friends and find common ground.  Our residents average between 85-92. Favorite activities include: coffee chat, sewing club, anything with music, reading with youth, crafts, Bingo, and chair exercises.

Oak Pointe of Carthage begins the communication process for our memory care with each resident’s pre-admission process. Our team spends time learning about each resident’s individual needs and preferences to ensure a person-centered, individualized plan using resident and family input. This plan is reviewed on regular basis and as memory care resident’s needs change We encourage our families to ask questions, visit with our leadership team and staff to ensure we are sharing information effectively and providing for the varying needs of our memory care residents.  We provide support and education to families to help further their understanding of the dementia process. Family members are invited to our Family Council meeting where they can communicate with the community leaders and share their experiences with other family members. Our full-time activities director shares photos with families of our residents through our Oak Pointe of Carthage Facebook page!

Oak Pointe of Carthage ensures that our caregiver-to-resident ratio meets or exceeds required State of Missouri requirements. Our ultimate goal is to ensure that each resident receives the care and supportive services they need to maximize their independence while living in our community.

Oak Pointe of Carthage is a pet-friendly community and we understand that our residents’ pets are family. Dogs and cats are welcome; however, must remain inside the resident’s personal apartment. Unfortunately, not everyone enjoys having pets in their lives and we must respect their rights as well as those of our pet owners. All pets must be leashed when outside the resident’s apartment, in common area, or outside on the grounds. We request that each resident and/or a family member ensure the care of the pet. Pets are not allowed in the dining rooms at anytime. We have a yard on all sides of the building with big open areas, as well as an in-closed courtyard for the pets to enjoy.

Upon becoming a member of our team, each staff member is provided with an extensive orientation process that includes both State required training and additional training required by our community. Both new hire training and our ongoing continuing education programs include topics related to Dementia, Fire Safety and Emergency Preparedness, Resident Safety, Confidentiality and Safeguarding of resident information, Safe Medication Administration Techniques, and Customer Service. In addition to our trained caregivers, Oak Pointe of Carthage’s director of nursing and wellness manages the assessment of each resident using monthly reviews, regularly scheduled individual service plan updates and ongoing communication with staff and families. Our team establishes a baseline cognitive level for each Memory Care Neighborhood resident during the admission assessment process. Residents are formally re-assessed and their individual plans are established upon admission and updated every six months or when changes are observed by staff that require a new plan to ensure resident needs are being addressed.

Oak Pointe of Carthage’s Memory Care Neighborhood is designed to engage residents and encourage interaction between residents and caregivers. The neighborhood is intentionally designed to provide “activity” stations that appeal to residents who are may wander or have repetitive behaviors. The neighborhood offers an activities table with puzzles, memory games, adult coloring and painting supplies that residents have access to around the clock. The space is open and conducive to those who enjoy walking and offers activities stations for sorting and folding, sensory stimulation, organizing and tactile stimulation. Reminiscing activities often feature a topic and encourage residents to share their experiences from a past time in their lives to stimulate long-term memory.

We are located in the middle of town with easy access to the highway.  Oak Pointe of Carthage is on a quiet street off of the business district of town.

Oak Pointe of Carthage is designed to the latest in fire safety standards and life safety codes with a fully integrated alarm system that includes smoke detectors in all resident apartments and throughout the community and a remote connection to a central dispatching system. Our community is fully-sprinklered ensuring even greater safety for residents. Residents are provided with an emergency call system that is monitored in a central location and alerts staff when a resident needs assistance. All entry and exit doors are alarmed and provide a greater measure of security. Our front entrance is locked at dusk and requires staff assistance to enter the community after hours.

Oak Pointe of Carthage is designed with a Memory Care neighborhood that caters to the needs and safety of residents with memory impairment. Residents with a known history of wandering are referred to our Memory Care Neighborhood. This secured neighborhood is fully secured and designed for residents who are wandering and/or exit seeking. All entry and exit doors are alarmed and those alarms are linked to staff pagers.

Oak Pointe of Carthage provides each Memory Care Resident with choices and options for meals. Working with families, we identify “likes” and “dislikes” related to food choices and ensure each resident receives nutritious, well-balanced meals that appeal to the resident’s tastes. Caregivers take orders for each meal and residents join each other in a family-style setting in the dining area of the neighborhood to share the meal. To assist our dementia residents in making their meal choices, our caregivers often use our picture menus to help them decide what they want to eat. Caregivers often share the meal with residents providing a modeling opportunity that encourages residents to eat. As resident’s progress through the varying stages of dementia, caregivers may request certain types of foods, for example, finger foods or sweet tasting items that appeal to the resident. To encourage residents to eat, our neighborhood Bistro is set up daily to offer drinks and snacks throughout the day.
