Do NOT bring your loved ones here. Our family has experienced extremely concerning issues with the care provided at this facility, and I feel compelled to share what happened. My father is now being admitted to St. Anthony’s Hospital due to what we believe to be clear negligence. He was left in his own urine for over an hour, and the floor around him was saturated. It wasn’t until I arrived and involved management, social workers, etc, and my sister called in that staff finally responded—despite my mother repeatedly notifying the nurses and using the call button. Additionally, he was not provided a single meal the previous day. The situation escalated even further during a meeting that the facility itself scheduled with my mother, administration, and nursing staff. In the middle of that meeting, a nurse received a call informing her that my father had fallen. He is a documented fall-risk patient, yet he fell because no one assisted him to the bathroom and no one was monitoring him appropriately. This level of negligence is completely unacceptable. It is heartbreaking to think about patients who have no one to advocate for them receiving this level of care. And the list of negligence from this place is much longer. We will be formally escalating this matter in full & taking legal action

This is a horrible place. Do not put a loved one in here. There is absolutely no supervision on staff during weekends and apparently no supervision during the week because the poor nursing care remains the same during the week days as it does during the weekends.. My sister is in there because she has an aggressive cancer throughout her body. She lost use of her legs and arms. The staff didn't keep her on her pain med schedule unless a family member was there. My sister would be without water because the staff wouldn't go to her room very often. Her medication kept her mouth dry so she needed water. She developed a diaper rash and a UTI. They left her in dirty sheets and gown until my sister brought it to their attention. The facility is not clean and poorly supervised. Fortunately today she is being moved to a Hospice care center where she will get the 24/7 care she needs to be comfortable in her last days of her life. Horrible place. Don't go there. I feel sorry for those patients that do not have a friend or family member to be their advocate.

The food was good during my wife's 5 day respite care stay but there are some issues I have that were never addressed. My wife as to get a shower on Thursday which never happened even though several nurses said they would mention it to her CNA. When I visited on Friday she had not been provided a shower. I spoke with Lashairi, a very positive and pleasant nurse there and she said she'd ask the CNA, Avanti to make it happen. I also noticed that many of the patients are not checked on routinely. In fact I do not see regular supervision at all. Many rooms had a distinct urine smell coming from them. Cannot say I'd recommend this care center to anyone

I was for the most part pleased with this facility and it's cleanliness. Also, the friendliness of the staff was quite good. They took fairly good care of my grandfather, and provided sources of entertainment and amusements for the seniors staying there. They seemed to genuinely care about him and passing all pertinent information on to our family about his care. There were some instances where I was disappointed in the manner in which he was treated or left in a dirty linen for too long, all in all I would say they did a good job, aside from those instances mentioned above. When he passed away they were very kind and expressed a deep sympathy for our family. The price from his stay was partially covered under his Medicare insurance I believe. I appreciate all of the care they did provide for my Grandfather in his last months of life.
