I had back surgery and had to go to a rehab facility. I was given a list to choose from. I chose Jax Nursing and Rehab Center. I made the best choice. Clean and welcoming. The staff is top rate. Now the food needs work, but on a scale from 1-10, I give it a 5. I met the person in charge of the food, and working on making improvements. The entire staff are caring and personable. The physical therapy staff are top notched. I went from a wheelchair to a walker and then a cane. The wound care dept treated me with dignity and respect. If you're looking for a nursing home or rehab center, go by and check out Jacksonville Nursing and Rehab Center, you won't be disappointed.

I visited Jacksonville Nursing and Rehab Center. They were unfriendly. They were not clean. Some things were just so dated and it was too full. There were more patients than people to take care of them. I didn't see how many CNAs there were per person, but there were too many clients sitting in the halls and just roaming and the area was too small and compact. I didn't feel welcome there. The building was a little bit too small to have that many people in it. I didn't smell any smells, but they could have had some updating done.

My mother-in-law is in Jacksonville Nursing and Rehab Center for short-term rehab, and she's going to come home. The facility is nice. Comparing all the places I've looked at, it's nice - landscape, driveway, cleanliness, parking lot, and the room she's in. She has a nice room. She's in a room by herself. The staff for the short-term rehab is is good. They had a nice physical therapy team. They did very well with her. They helped her tremendously. The food is medium, not great, not terrible. It was recommended by another place, so that's how we ended up going and checking it out, and it looked as good as our other options. They could improve on the CNAs, their attitudes, cleanliness, making sure the trash cans are dumped, and responding in a more timely manner with any request or what they're supposed to do. It seems like when you ask for something it takes an hour for you to get it.

Nobody should go to Jacksonville Nursing & Rehab Center. The food was terrible. They just came in every once in a while. I wouldn't see them all day sometimes. They gave me rehab with a fracture. I had a fracture and they said, "No, he's alright." I didn't see them treating anybody halfway decent. They just did not care. I think they had one good CNA, and that was it. I'd give them a five on cleanliness. They thought I had just a bruised hip, but it was a fractured hip. I don't know if they took another x-ray. It hurt bad, so I made them leave me alone for a couple of days. On the third day, they came in and said, "We got to get you out of bed and walking," so I got out with a walker and walked around. The therapist people were pretty good. I have to give them credit. They had one activity in the afternoon, and that was bingo. I went to one bingo, and they put a thing around my ankle; they thought I would run away. This place was not right. Do not send anybody there.

This place stank, literally! The food was actually pretty good. The care of the patients should be reported to the state. My father was in hospice care and we used this facility temporarily for 3 nights, 4 days while moving his belongings, after the sale of his house. He was a fall risk. He requested assistance for the restroom. After 35 minutes, I went to locate staff. The "assigned" employee was on dinner break and apparently no one else would assist. They forgot to bring my father breakfast one morning, then forgot dinner the next evening, even arguing with him!! The poor soul that shared the room kept the TV volume very loud, 24 hours a day. He was bedridden and never, ever, did I see staff respond to his verbal requests for care.

A place (among others) sucking down Medicare and/or Medicaid dollars. There are no interactions with staff, there are NO activities or amenities. Patients are here to provide income. no

My brother is in rehab at Jacksonville Nursing and Rehab Center. It's good and they seem to take good care of him. It seems clean. I don't find them all that responsive when I try to get in touch with his doctor or social worker, but other than that, as a facility, it seems OK or good.

Right now, my mom is in Jacksonville Nursing and Rehab. The staff is great. They do a good job taking care of their patients. They really put their hearts into it. They have therapy for the seniors. There is an area where people can socialize. The rooms they have there are great. Their dining room is also nice. My mom has three meals a day.
