My mom is at Lake Forest Nursing Center. It is nice. I like it and have no complaints. Everybody is wonderful there. My mom is immobile and not really eating. She is receiving hospice care there. She likes to draw sometimes if her hands are working. I know they do all kinds of activities because when I walked through, I saw them doing them. They do bingo, and they go out. Lake Forest Nursing Center is very big. They have rehab for people temporarily rehabbing from the hospital, and long-term care. The room is clean. Everybody there is nice. It is quite pricey, but my mom has Medicare now because she is on hospice.

Rooms are old, furniture is in bad shape, most staff don't wear name tags so one doesn't know who they are or what they do. A drug from home was misplaced for couple of days. Requested fruit for snack, got pudding every single time. Menu read chicken, broccoli, mashed potatoes--got no chicken. The doctor visits 3 days a week. Rehab is spotty to say the least and nonexistent on weekends. My husband is in worse shape now than when he went in there 3 weeks ago and has bed sores due to inactivity. Would not recommend this facility.

This facility has been sold to Sun Mar Health Care. The new company is due to take over the facility on July 1,2022. Maybe things will get better, but currently, the rooms have old linoleum floors, beds still work, but they are old. The linens are old, and have clearly seen better days. The air-conditioning works, but by mid-day, the room can be very hot. There are very few activities as long as there are no patients with COVID-19 at the facility. There have been very few activities in the last month or so due to presence of COVID-19 patients at the facility. The staff are nice but they are over-worked and under paid! The facility is hugely under staffed and sufferers from poor communication and management. If you would like regular and timely communications regarding your loved one, this is not a place for you! The phone system belongs to another century! All calls, including the ones to the patients rooms must go through the front desk! The front desk covers phone calls as well as all people who wish to enter the facility. Good timing is essential if you do not wish to wait! There are few amenities such as a dinning room, as long as you do not mind the state of the furniture, stained carpeting, old paint and wall paper! The food is essentially not appetizing. Frequently, the patients are served the same food for both lunch and dinner. The food has no flavor and at times does not even look good. Would I recommend this place? In a word: NO!

I would like to start by saying that Lake Forest nursing facility is one of the best facilities I believe that a senior can go to for physical therapy, wound care, attention for medical needs the staff are very professional very caring and understanding and informative to family and friends they conduct themselves at the facility like people in healthcare should???? not like some of these other fly-by-night facilities that are only there for the 9 to 5 job. If more nursing homes and Care Facilities would use the etiquette that Lake Forest nursing facility used there would be less nursing home neglect there will be less injury to elderly. I really hope Medicare would give Lake Forest nursing facility a 5-star review I can't say enough about this facility helping my father and the staff. Don't get stuck in some shady facility in Garden Grove????

Lake Forest Nursing Center had been taking care of my father for the ladt five years. They have done an outstanding job of taking care of him. I appreciate all of the staff working to care for the patients there. Thank you all, Joseph Sanchez
