I visited this facility

I recently toured this facility with my husband while searching for a place for my mom, and we were truly impressed. From the moment we arrived, every staff member we encountered was kind and welcoming. I toured with the Admissions Director, Rachel, who was incredibly helpful in guiding us through the process of determining the best level of care for my mom, who was just diagnosed with dementia. It has been an emotional and overwhelming journey trying to find a place that feels like home, but this facility genuinely stood out. I had the opportunity to meet the CNAs and nurses who would be involved in my mother’s care, and they were all attentive, compassionate, and knowledgeable. The facility itself was very clean and, importantly to me, didn’t have a clinical or hospital like smell. There is a beautiful, peaceful courtyard with a walking path, as well as a charming resident maintained garden that adds a charming touch. Knowing that my mom’s needs may change over time, it was reassuring to learn that this facility is equipped to adapt and provide the appropriate level of care as those needs evolve. Rachel and her assistant took the time to thoroughly explain everything, including insurance coverage and out-of-pocket costs, which gave us a clear understanding and peace of mind. After spending several hours with Rachel, both in person and over the phone, I feel confident that we are making the right decision. I strongly believe that employees reflect the heart of an organization, and based on every interaction I had from admissions to nursing, kitchen staff, maintenance, and housekeeping I feel reassured that my mother will be in caring and capable hands. I left feeling relieved, secure, and hopeful. I highly recommend this facility.

I visited this facility

My mom was recently at St. Jude and they referred us to a skilled nursing facility, so l came by to take a tour. I had a walkthrough with the admissions director, who was very kind and empathetic while answering all of my questions. The building looked clean and well cared for, which gave me a lot of relief during a stressful time. I really appreciated how patient and understanding they were while explaining everything to me.

Provider Response

Thank you for your kind words, Brenda Ortiz. We are thrilled to hear that our admissions director provided you with the support and understanding you needed during this stressful time. Your feedback on our clean, well-cared-for facility reflects our commitment to quality service, and we appreciate you taking the time to share your experience with us. Reach out to the team if you need anything else during this time.

I am a friend or relative of a current/past resident

My wife broke her right femur above the right knee replacement. The room my wife stayed was well suited for her condition and clean. The meals were good with a variety of different meals. The center catered to early or late meals to accommodate her doctors appointments. My wife has physical therapy almost every day giving her a sense of going forward with her life and accomplishment. The staff was available when she needed them. The kicker is that Park Vista Health Center had some form activity every day. This kept the patients engaged with other patients.

Provider Response

Thank you for sharing your experience, Albert! We are thrilled to hear that your wife found her room well-suited for her condition and enjoyed the variety of meals we offer. It's wonderful to know that our daily physical therapy and engaging activities helped her feel a sense of accomplishment and connection during her stay. We wish you both well.

I am/was a resident of this facility

I'm in skilled nursing at Park Vista Health Center for about three weeks now. It smells clean. The staff comes in in the morning and says, "Good morning. How are you? It's a beautiful day." I said all they need now is the Disney music and the birds singing because it's really, really cute and happy. There's constant upkeep and maintenance. There are constant people vacuuming the floors and cleaning the bathrooms. I've actually never encountered something gross. I've never even encountered pee or anything in the toilet. They're constantly cleaning. They have really great food. They have a chef that cooks fresh food, not frozen junky food. They have activities for people that are here, like movies and live music, sometimes two or three times a week live music and sing-alongs. They have PT and OT and they have a whole room with equipment for the PT and OT. They will do anything to make you happy. I never encountered a staff member who wasn't supportive even if they weren't in charge of your room. If they saw you doing something weird, they'd say, "How are you doing today? Can I help you with anything?" It's like everybody's always there for you. It's an older facility, but I still give it a five because of the cleanliness, the smell, and the upkeep. If you don't like the food, they'll bring you something else. They'll bring you something any time of day. If you get a hankering for ice cream, you could ask for it and they'll get it for you. Compared to regular nursing homes, this place just shines. They deserve a five in everything. They saved my life. When I first got here, I couldn't move anything except for my big toe. I was so sad because I thought I'll never walk again and now I'm walking with the walker up and down the big hallway and all around the building and I'm supposed to go home tomorrow. Most places don't give you as much time in therapy as this place does and that's important to get your strength back and being able to function again.

Provider Response

Diane, we're so glad we exceeded your expectations and delighted to hear we delivered such a great experience. If there's anything else we can do for you, we're happy to help! We will share all of your kind words with the team. Thank you so much for your feedback! We hope you have a great day!

I am a friend or relative of a current/past resident

I went to visit a friend at Park Vista Health Center. She is recuperating from a broken hip. It's a lovely facility, but I hate computers, and I think they should just have people signed in. They have everything that I have noticed other facilities are having. The staff members are very helpful. Her room looks clean, and she seems comfortable. I didn't see a dining room, but there must be one. She's not able to get out of bed as far as the broken hip is concerned. I did notice some kind of physical therapy going on. The facility is very well maintained. I haven't had a meal and she doesn't have an appetite, but it looked good. There's a library to the left of the front desk. She seems to be doing well. The other reason I noticed that it might be a good idea to just have people signed in (and I realize with Covid they have to take your temp) is because her son has epileptic seizures and he was with me, and it takes him forever to use the computer to put his name and phone number and so forth. They helped, which was very kind of them. I also noticed a gentleman that had probably come to see a friend or a relative (he's about my age and I'm 80), so sometimes it would be better if they could speed things up, so people don't have to stand there.

Provider Response

Kathryn, thank you so much for taking the time to leave us a review. We’re glad to hear that you are happy with our care, staff, and facility. As far as the screening process, we have implemented several measures at Park Vista to maintain our infection control practices and maintain the safety of our residents, guests and staff. We strive to follow CDC guidance and recommendations from state and local health officials, which includes a mandatory health screening and check-in process for each visitor. Our staff does all they can to assist with technical difficulties and provide support of any kind, which I am glad they were able to assist you with. As always the comfort and safety of our residents and guests is always our number one priority. Thank you again for the review, from the team at Park Vista.
