My father stayed at South Mountain Post Acute. I thought it was great. He was there for about five weeks. When I went in every time, they were very good. I like its friendliness. It makes a difference when it seems like they care. When you're not friendly, you come off right at the top as just doing their job like a robot. The staff was very friendly and helpful. The food that they had came in separate Styrofoam containers, which is nice because of the whole COVID and everything that is going on. It's almost like a take-out where everybody gets their own Styrofoam container. He has a certain diet he has to keep, so I thought it was great because that's what he needed at that time. The food they were giving him was within his guidelines, so that was good. His room was like a regular hospital room; you share a room with somebody. They have a front area where they can go any time. It's just a front area right there where they do the rehab for everybody depending on what they're going there for. They have a different area down the hall for a shower. Each room doesn't have a shower because everybody has (for safety reasons) different situations. The shower is down the hall, and the rec room is in the front, which has like a TV and stuff.

Since being here at this place, I have lost weight. The kitchen is consistently messing up my food. Never enough. I have double portions. But they don't always do it? The menu this morning said there was to be milk, toast and jelly and margarine. I wouldn't have known that if I didn't check the menu. I sent my clothes to wash. It was lost. I have been washing my clothes by my own with hand and one foot. Not cool. I wrote a grievance 3 weeks ago. No response. Got corporate involved. The only reason I came here they were rated 5 stars? The only reason I haven't left yet. The staff is wonderful. But laundry and kitchen.....

Generally, the facility is clean and care is taken to provide a good "first impression" to patients, families, and visitors. As a patient, it didn't take long for such a good face to erode. Staff are generally personable, but not necessarily friendly, and were often accommodating and patient, although there were significant exceptions likely due to personality conflicts or my having to great an expectation of what good customer service looks like in a skilled nursing facility. I soon realized that I never want to be placed in such a facility again as I prefer interacting with staff and individuals beyond providing basic information. I was told numerous times that staff are there to address immediate needs and then move on, and that my family and I would never find a better facility in the state as all are staffed similarly and share the same care philosophy. That attitude left me feeling demoralized, so I simply accepted the status quo and prayed to be released soon. I engaged in activities and, as predicted, was released at the exact time medicare benefits were due to end. I left the facility without any 'happy' message from staff as one member told me that all were glad to see me leave.

I was there a few weeks ago for a broken hip. I have been to a few rehab places, and this is the first that did not smell. It was a nice facility, it looked like it was updated recently. Therapists were top notch. Food was good, not the best I have had, but it was good. When it was not hot enough, aides were nice and reheated. I would highly reccomend this rehab center.

Would not recommend! My grandmother was placed here due to covid and she could not do anything herself and they released her when she was no where near being healthy again! They said if we couldnt find another home they would have to call a transport to send her back home and they lied on paperwork saying she was healthy and now we are faced with having to take turns from work watching her when she has no strength to do anything herself! Such a terrible case manager to send her off like this!

I have had a knee replacement and my family toured a few rehab facilities, south mountain is the only only that did not smell. The staff actually care about the patients there. Therapy is really good at helping you recover. They had a girl visit while I was at the hospital and answered all my questions. I stayed there 2 weeks it was a pleasant stay and I will return if I need to future for therapy. I used the barber shop when I was there too. Food is hot and I like that they have an alternative meal if you don't like what they serve. I would give a 5 for overall rating.

My mom is there for rehab. I find that she is getting good care. Most of the workers are very nice. They could be a little more professional. I give them 4 stars because my mom is not the easiest person to get along with and they do a good job. Also I have myself worked in assisted living before so I understand some of the challenges. No pee smell. Rooms are clean. Facility clean

The facility always smells like urine, patients are constantly yelling for help which they don't receive and the nurses have no bed side manner. Do not recommend to assign a love one to be a resident there. If you have a choice to choose somewhere else, do so.
