This place is the worst.they are understaffed with the aides.you have to wait way to long for any assistance. Most of them act like you’re asking for a favor. Shortage of gowns ,shortage of socks with the grips. Told to buy my own .residents in hallways half the night in wheelchairs talking and screaming.I caught bronchitis because the air condition was cold. Beds are out dated extremely uncomfortable.food is awful. The only thing good is the Physical Therapy. Otherwise stay away at all cost

The Grand Pavilion for Rehab and Nursing was just as nice as every other experience that I've had. They did a very good job. The place is beautiful. It has everything that anybody would need for elder care. The rooms were very nice and decorated beautifully. It didn't look like a nursing home. The food is excellent. They had a chef and a very nice dining area. Overall, it was wonderful. I could not have asked for anything more.

Our experience at the Grand Pavilion in Rockville Centre was terrible in every conceivable way. The first few days seemed promising, but that was just for appearances. If you spend a decent amount of time at the facility with your loved one you will see what things are actually like — and it’s an absolute nightmare. Overworked and nasty aides who will ignore their patients needs for hours on end. Nurses & admin who are aloof with their patients and uncommunicative with patient’s families. I would strongly caution against this place.

I was at The Grand Pavilion for Rehabilitation and Nursing at Rockville Centre. The therapy was phenomenal, and the cleanliness was phenomenal. However, sometimes the staff were very unprofessional and had a terrible attitude. I love their food though. The place was very clean. They had a lot of activities for them. Because of my pain, I wasn't into them, but they have a lot of activities going on. I had a private room and it was great. Their dining area was nice. They had an outdoor patio as well. It was nice and comfortable.

I strongly caution against choosing the Grand Pavilion for your loved one's care. Last July, my over 90-year-old father sought rehabilitation there after a hospital stay, but what followed was a distressing ordeal. The rehabilitation consisted of either whole day wheelchair confinement or being left bedridden. My father developed a painful sore on his back, requiring surgery. He needed pureed food, couldn't feed himself and was not fed for nearly two weeks. He was given unsuitable food, and essential hydration was either delayed by up to 12 hours, or other days not even provided. The staff readily accepted "pocket money" but failed to provide proper care. Dr. O. was non-existent (never once saw him). He travels to the Bronx and all over Long Island to his other rehab centers and offices, and the other doctors were well trained to evade accountability. At the GP, my father's health rapidly deteriorated, culminating in an emergency hospitalization where he lost recognition of his family, needed a blood transfusion, and was breathing at over 40 per minute. Furthermore, I witnessed other patients enduring similar neglect, spending entire days in wheelchairs, some even begging for bathroom assistance, to no avail. This collective neglect and disregard for patient well-being cannot be ignored. I saw no rehab equiptment, only staff walking with some patients on three occasions. The social worker and receptionists were the sole exception amid this grim experience. In summary, I strongly urge you to avoid the Grand Pavilion. Our painful experience serves as a stark warning to exercise extreme caution when considering this facility for your loved ones' care.

The staff does not care for the patients. They don't even hide their lack of care. My father was a patient there for several weeks on the 2nd floor in April-May 2023. He was told to urinate and dedicate in a diaper all day, everyday which was only changed once daily and with significant attitude from the staff for having to do so. All 15 times I visited I found my father with his face, chest and bed covered in spilled food from his difficulties eating. He was left that way for hours on end every day. There are no medical personnel on weekends and they often leave early on weekends. Don't send anyone you love or care for to these disgraceful scummy people.

my mom checked in at 12:00 and I had her checked out by 8pm the same day. I called 15 + times to make sure she was ok as she came in with blood clots in her lung and was on oxygen. the receptionist sent my calls to the nurses but NOT one picked up. No attention there and the doctor who went to see her .. we couldn’t even understand what he was saying. He also was NOT familiar with the case so my mom was so scared. she felt short of breath all day but couldn’t get the nurse to come. Terrible staff, dirty facility! do not send your loved ones there.!

had to find a rehab/nursing for my 103 year old mother coming out of a hospital. My husband and I made the rounds and decided on the grand. Hearing all the horror stories about nursing homes we went in not expecting the best. From the time we met with Tammy in addmissions we were put at ease. The nurses,AIDS,rehab. were all wonderful to her. They washed and changed her every day , I felt at ease leaving at night knowing she was taken care of. I also know they say when someone is there they take better care, but I was truly comfortable. Sorry to say Mom didnt make it and I could point out things that were not done but all i have to say is we made the right choice

My dad was here rehab for about 6 weeks. At first it seemed to be ok but understaffed. Not the cleanest place but ok. Phone calls were never answered but this is not unusual for rehab facilities. Physical therapists were great. When visiting my dad one day I noticed he was acting very unusual…not at all normal for him. He also had a bright red eye to which nobody knew why. I brought it to the attention of staff and asked to see the doctor. Doctor would not speak to me and I was told that the nurse would call me once the doctor assessed him. I knew I would never get that call so on my way out I stopped by administration office but he was not available. Never got my phone call! About a week later my dad was sent to hospital for being combative. After 24 hours on antibiotics he was back to normal. Clearly he had an infection.However due to the report written by this facility the hospital had trouble finding a new rehab for him. They finally found a lovely facility 40 minutes away. I would definitely look elsewhere when looking for a rehabilitation facility.

My father was admitted there at The Grand Pavilion for Rehab & Nursing. I didn't like it. The rooms were tight. The cleanliness was an issue. I don't recall my father complaining about the food, but the nursing staff was not the nicest. They seemed annoyed when you ask for things or if you ask questions about like the medication that he was supposed to take. My father complained once that he had to ask someone to come and sweep the room. They weren't like sweeping the room every day. The dispensary ran out of soap, and it didn't have soap for a day or two. His room wasn't laid out well; it was very cramped. He went to therapy once or twice and then they had a COVID outbreak.

Don’t be fooled by the positive reviews!!! Let me explain, if you admit yourself into this facility and you are under 75 and can advocate clearly for yourself, then you will probably leave a good review. Our experience was horrible. My father didn’t make it through the week here!!!! He was 83 and was admitted COVID NEGATIVE. As soon as we noticed his condition deteriorating after a few days here, it was clear he wasn’t getting the proper attention he needed, and we felt we had to get him out of this facility!!! As soon as we told the Grand Pavilion we wanted to take him home, we were told he was COVID positive and then passed away in this facility a day later. Dad passed alone at this facility November 2020. I would like to know how this happened if he was placed on the floor that was COVID FREE. How many other patients had the same situation? To add insult to injury when we asked for his belongings they kept alot of his belongings and gave us someone else’s things, which were obviously less valuable. NONETHELESS, this was and still is very difficult for our family. I was finally able to write a review after this heartbreak. This is a facility that clearly needs a complete overhaul, in patient care and compassion!

The facility did its best to keep our family updated with the COVID - 19 protocol and policies concerning nursing home facilities. The staff worked tirelessly to keep our family members healthy and well. The recreation staff provided us with virtual calls which help eased the separation anxiety. The nursing staff also kept us updated on our family members progress. My family and I are thankful for all the care the healthcare workers have provided to our family member throughout the years.

This place is HORRIBLE!! DO NOT send your loved one here! My 88 mother in law was admitted Thursday night. Friday we tried to see her, they refused. We complained and they still would not let us see her. This morning, Saturday, we went back and they wouldn’t even let us in the front door! We called the police. They came and they wouldn’t let the police in!! The police called for back up. Four more officers came and they finally let us in. While we were waiting for back up, they put a microphone in front of her face and forced her to say she was happy there on video!!! No joke!! Which she did because she was frightened! We had an ambulance come and take her to a hospital. She had terrible care while she was there! They made her wear a diaper!! And she was totally capable of using the bathroom if she had help to get there.But any time she called for help, no one would come so she was forced to have to use the diaper! Every time we would call to talk to a nurse on her floor, we were told no one was available because they were “giving out meds”. EVERY TIME! Any time of day! I could go on and on. Anyone that we were able to talk to blatantly lied about the care she was getting, These people can not be trusted! It was a nightmare!

My mom came to live at the Grand Pavillion in February. She had previously been in another nursing home in Freeport which was OK but when Covid shut everything down they were not very responsive or helpful so I had to take her out of there. I had her at home for a while but it was very difficult because she is immobile. She ended up going to the hospital and from there they suggested she go to a rehab, based on some of the reviews I saw for GP, I was not so happy about it but she needed to start rehab immediately after the hospital so I agreed. I have to say my experience with the administration and staff has been nothing but excellent. They have an excellent online site that you can schedule FaceTime visits. They email you a reminder 30 minutes prior and they are generally on time within five minutes. Last week they opened for in person visits and I finally got to see the facility and meet the staff. I could not be more impressed and my mom is so happy there. They are incredibly caring which is something I did not feel with her previous nursing home. Not to say the people weren’t caring but they seemed overwhelmed and here they seem to have a very good ratio of caregivers to residence. All of the staff that I have met is wonderful and genuinely caring for the residents. I have been there a total of three times in person and I am more impressed every time I go. My mom is doing better than she has ever done since Covid lockdowns. While I am of course sad that she had to go into the hospital, I am also thankful that the result was that she now resides at the Grand Pavillion. I would highly recommend for anyone considering a long-term care residence for their loved one.

Dont put your family member here. it is a living hell dealing with them. I can't comment on the inside due to covid restrictions but dealing with social workers, nursing dept, recreation (to set up face time calls) were all terrible. Lack of communication is an understatement. First 3 days no phone calls or return calls. Had no idea what to bring for my relative or what to expect. Knew by end of first week I had to remove my family member. Found out during the coordination of the move that the admin wanted us to wait a week to get "stats". $$$ that's all it's about. It's just disgusting......I wouldn't leave my dog here. I wish I read the other reviews here before choosing this hell hole.

The evening staff is lazy. They leave residents in diapers all day/ night without changing unless feces. Food is not good. David was good & Social workers were nice.

My mother was a patient of short term then moved to long term. Lack of communication between the 2 was terrible. quality of care my mother received during her stay was beyond belief. Wait times of 2-4 hours once pushing the call button to use the restroom, to be turned or re positioned was horrific resulting in multiple pressure injuries to my mother that were so so painful. Leaving her in soiled pads. I would have to call nurses station to get them to go in her room after 4 hours of waiting. Nurses were all nice on the phone then go in and lecture my mother about me calling them. Telling her to tell me to stop calling. Scaring my 82 year old mom . Staff never listening to anything my mother said. Not to grab her arms cause skin was fragile from meds, she even had a bracelet on to remind them and yet they still ripped off her skin leaving multiple on multiple lacerations. unattended constipation care as well. I could go on and on. All this stressed out our mother so and broke her spirit and will to fight. If i didnt attend an upcoming appointment she had shed would have died in this horrible place. They didnt even send her to ER cause of shortness of breath and fluid overload, it took her cardiologist 1 second to look at her and say you need to be admitted ASAP. If nurses were in there like they should have been this should have been acknowledged and addressed to take her in. this was my mothers experience in Grand Traverse Pavilion. I would take warning if you intend on sending your loved one there

This is a first hand observation. We were with my mother day and night. This place is dirty and run down. There is not enough staff at all. At night they leave patients who are unable to get up on bed pans for 25 minutes. I have seen this. The physician on staff did not return phone calls. He orders medication by phone. If we did not ask for a list of daily medications, the doctor was giving my mother two different medications that treated the same issue. A nurse flat out lied about a treatment for my mom and I know for sure she did not get (both we witnessed it and my mother's roommate). We had to ask for results of blood work before the doctor would look at it. I have seen all of this with my own eyes. When my mother was in respiratory distress and not responding, I brought to the attention of the supervising nurse. I was told that they were busy changing shifts. It was not until the aide came in and observed did the supervising nurse come in to look at my mother. The response from the supervising nurse was "well, if it were my mother, I would get her an ambulance to go to the emergency room". She was asking me what I thought!!! Keep your family member at home if you can and get rehab at home. Do not bring a loved one to this place. It is terrible.

This is the 2nd time I was there for pt. The pt and ot staff were great. I went from not walking to walking with a walker. The nurses and cna's were caring and helpful. I would highly recommend this facility for anyone who needs pt. The food was very good also. They gave you plenty to eat.

Nursing care was unexceptionable, no compassion for the elderly, very rough with the patients, not knowledgeable about medical issues, patients left in the hall with only nightgowns on waiting to be put to bed. When a request was asked the response was when I get a chance or yes and it was a half hour later. Rooms were basic, a shared bathroom. I never noticed activity maybe a movie at times.

This place is a pretty sham. I have a client that is here, I was her Senior Helper when she was @home. Although she is here my purpose is to ensure that she eats and is as comfortable as possible. The staff thinks that I am suppose to come and do their job. They are boisterous and inconsiderate. When I visited my client I noticed her room was cold. To my dismay one of the staff opened her window and left it opened, mind u the residents are elderly and usually are always cold. I then witness the culprit open a window where the big bathroom is, consequently making the hallway cold and unfortunately the resident that was sitting in the hallway started to complain. When I alerted the staff, I was told the window was opened to air out the bathroom, which was a lie and the aide still could of done so but should of closed the door. Oh and therapy is a joke, the therapist that was there 12/24/17, was on the phone discussing a resume and left my client just sitting for over 20mins. On top of that she was thirsty and I had to go down the hall to the lobby to fetch her some water. Lastly, I found another resident's blood stained tee shirt and other clothing articles ( underwear) of 3 previous residents in my client's closet. The place is dirty and I had to speak to an advocate in order to get soap in the dispenser in their room. After I asked the housekeeper twice the previous day.

Currently a family member is here for rehabilitation. The PT staff is great and attentive. Food sucks and it is a disgrace how these people are treated. It amazes me how they get away with not helping these elderly people who rely on the everyday care they need. It is worse that a dog locked in a cage!! Place is dirty, don't think the maintenance people know what a mop is!! GROSS

For families thinking about placing loved ones in this facility , think twice , beware . they let you check in for rehab but keep you beyond medicare's alotted days . Then you are charged private around 10000$ a month. . Shame on you Grand Pavillion !

Visited a family member and felt the pain management was adequate. Staffing is always less evenings and nights, this is common in the healthcare industry. As is the norm of LPNs running the units, night and eve shift. Nothing out of the ordinary For the person

My mom was in the rehab for 6weeks. The PT department is great and the nursing/care staff is very nice but the overall care is lacking. They need more staff to take care of all the needs of the patients. The rooms are small and not cleaned often. Their major fix would be to add more staff.

I went therein December for short-term rehab after having a total hip placement. The rehab department is excellent. The staff there were very caring and attentive to my needs. I highly recommend this facility.

My Dad spent 4 months here and it was horrible. The rooms are small and dirty and my dad was hospitalized multiple times for infections developed while here. You have to be on top of them every moment to make sure they are doing their job. I would stay as far away from this place as possible.

My mother was recently in this facility and she was very happy there. They were very attentive to her in all aspects, occupational therapy, nursing, and cleanliness. They are renovating but it is controlled and it will be beautiful. I was very leased with her care.

The staff is a 10 at the Grand Pavilion. But my mom is fully in control of her mind and this just wasn't the place for her. The rooms were small and has completely depressing atmosphere. So we thought that we had to get her out of there, because most of the patients were suffering from some form of losing control of themselves. She was in a 2-bedroom. I went to visit her early evening and the woman next to her, I turned a corner out to the hallway into the bedroom, and into the doorway, she was standing with no clothes on. Mom doesn't deserve to be exposed to that just yet. It's just not the right atmosphere. She didn't participate in any activities. The people took one look at my mom and said she doesn't belong there. And then they did everything in the world to help my mom get out of there. To be fair they're renovating.
