I had lots of questions and concerns and they put together a Care Plan Meeting and everyone shows up and you get to ask all your questions and better understand the plan they have for your loved one. Room was very clean.most of staff are happy to help.

The new renovations look very nice. They have private rooms with private restrooms/ showers. It looks a lot different under the new ownership and they have new managers who care a lot. There are several courtyards that are nice as well to spend time outside in the shade watching the birds or reading.

The food was always late and cold by the time it reached the patient. It had absolutely no flavor. To say that the quality of the food was poor is being quite generous. She was given some medications that she had not taken in over a year and one medication that she had never taken. When she was released they sent her home with the same meds that she was no longer taking. The communication between the staff was absolutely terrible. I have tried to contact the director of nursing four different times to get an explanation on why they were giving her the wrong drugs and I have still not gotten a call back. We are now being billed from their pharmacy company for the bag full of worthless meds. I definitely would not recommend this facility for rehab or permanent living. The place looks nice on the outside but how it is run on the inside is degrading.

My husband is at Seneca Place and our experience has been poor. The person that is supposed to work with the families is very difficult to get a hold of. I called once and I was on hold for so long that it switched over to someone else's voicemail. She never even called me back. So, she knew who was calling her, but she didn't take the time to call me back. There's just poor communication between the family and the person that was there. When I went to his new room, it had this nice big television in there, but no remote. They put all of his clothes in a garbage bag instead of the travel bag that was right there with it. I was trying to organize his belongings when I found that garbage bag with one of their blankets in it that was saturated with urine sitting in the wheelchair underneath his travel bag. There are ants in there, too. His room is very small. There's barely enough room for anyone to slide in between the bed and the window. It's semi-private so there's like a visitor chair in the middle and then the curtain. The person on the other side has just as much space, but the door is right there for them while he is by the window. The blinds won't open though. There's something wrong with them. There's usually not a trashcan in the room. The bathroom, which he's not able to use right now, is also small and there are usually never any paper towels in there. It's a Jack and Jill bathroom and multiple people use it. The food is cold and late and it takes a very long time for them to respond to a call button. However, they were all very helpful. The cleanliness of the place is OK. It doesn't have that nursing home smell when you walk in, but it is not always clean. There were just times when I would go to visit and the floor was sticky.

Food here is late and poor quality. I have seen several other families bring in food for their loved ones. Not enough staff to properly care for the patients and residents

I can't get the images of what I saw in this place out of my head. I don't know how the state is allowing this horrible place to remain open. My Grandmother was first admitted here for rehab, while facilities weren't allowing visitors because of Covid-19. The following is a list of issues during her 2 week stay: 1. The phone in her room never worked. 2. No one ever unpacked her bags, or allowed her to use her personal belongings. 3.She fell and busted her head open. 4. Was sent to the hospital, "for suspicion of pneumonia" but I later found out it was actually for acute respiratory failure. 5. Contracted MRSA 6. Contracted C-dif 7. Developed a stage 3 pressure sore. 8. Developed a blood clot in her leg. 9. Developed pneumonia in both lungs. 10. She was never bathed, and her teeth were never brushed. This resulted in a 2 week hospital stay, where I was advised that it would be best for Hospice to take over her care, and that she go to a long term care facility. Since she needed a medicaid pending bed, Seneca Place was the only place willing to admit her. I was reluctant to send her back there for obvious reasons, but felt like she would be safe since Hospice was involved. Approximately 24 hrs. after she was admitted, I went to visit her, (Thank God the visitation restrictions had just been lifted), and saw what I can only describe as hell on earth. The smell was what I can only assume as being the combination of death, feces, urine, and dispare. Numerous residents were howling in pain, and beging for water and food, while nurses wondered by, ignoring their cries. As I approached my Grandmother's room, the sound of a woman's screams intensified. As the director of admissions opened the door, I heard my Grandmother's roommate begging God to kill her. Once I got to her bedside, my Grandmother told me no one had given her anything to eat or drink since she had gotten there, and begged me not to leave her. She said her roommate had been screaming like that since she got there, and had kept her awake all night long. Moments later the Hospice nurse arrived and was equally shocked by what was going on there. As her roommate continued to scream to be put out of her misery, I demanded that my Grandmother be placed in a private room. They moved her right away, only to place her in even more deplorable conditions. I've seen furniture in a better state left on the side of the road. Mold was along the baseboards, paint was peeling off the walls, and there was feces on the toilet set. The chair beside her bed was left with the impression of the former resident. As I spoke with the Hospice nurse, and she attempted to assess her, I realized something was off with my Grandmother, and the nurse agreed that she should go back to the hospital. Meanwhile, they finally brought her a tray of food for dinner, and when I opened the lid, fruit flies were swarming the plate. I obviously couldn't feed her that food, and the Hospice nurse alerted the administration to call EMS. They refused. At which point I told them that I would be making that phone call myself,and was told that I would have to discharge her from the facility first. I agreed, and waited for EMS to remove her from the building and take her to the hospital. Upon arrival to the hospital, one of the EMT's told me that Seneca Place was the worst nursing facility he had ever been in. Once the nurse checked her blood sugar it was 44, which was caused by not having eaten in over 24 hours. She was shocked that my Grandmother wasn't in a coma. If I hadn't have called EMS, there is no doubt that my Grandmother would have died that night. She will not be returning.

Mom stayed in Seneca Place for a month. The rooms were small but functional. She was never in the dining facility because of her condition, and covid. They brought things to her room for her to do, and so there was some activity for her to do. They were caring, and took care of her the best they could.

The therapy departments (physical, occupational, speech) are great. The nursing department needs work. Not enough CNA's. Long waits for help. Bad communication. When things dont go right, information doesnt get to the department heads. I do not recommend Seneca Place at all.

My mother was a resident a few years back . She has recently passed away a different facility . State came in Seneca Place in June of 2015 and found a unstageable bedsore which resulted in amputation of her leg . When I had inquired about a boot on her foot I was told she had a “ blister “ . There was not even a Care plan for her nor was she being treated . Aides are responsible for 30 or more residents which makes them poorly understaffed . I could go on and on but simply put I wouldn’t place A dog in their care

Diversicare of Seneca Place was a nice, clean, and helpful facility. The rooms were warm and clean. The dining room was very nice, pleasant, and clean, and the food was very good. They had nice nurses and staff.

Twinbrook Nursing & Rehab Center was clean. The staff was pleasant. The rooms were nice. They play cards and bingo. I can't think of anything that they could improve on. It was a pleasant, short visit.
