My grandmother was cared for by one of the nurses at this facility and received excellent care. My grandmother told me she was treated like family by this particular nurse and we so greatly appreciate it. My grandmother has since past, but I just wanted to give a huge thank you to this facility for caring for her like she was family.

I am a frequent visitor of this facility and every time I visit it’s always fresh smelling and all staff great you with a Hello and a smile, and in today’s times it’s even more important to see smiling happy faces taking care of many residents. The nursing staff is professional and always helpful in answering questions. The rooms are well lit and very spacious. They have a wide range of activities. With things opening up now we can even be reunited with our loved ones. I can’t say enough about the entire staff and how they go above and beyond to ensure great quality care. I’m a hard one to impress and NO ONE is perfect and I only trust two people taking care of my loved one, me and the fine nurses at Clark Rehab

My mother was placed in this facility. This facility took very poor care of her both physically and medically. She came home with bedsores and other issues related to the extremely poor care received. Visitors were not allowed due to Covid restrictions so the family had no way of seeing what was going on and helping outside of phone calls. Her doctors and nurses were appalled at her condition upon her return home. She has spent times in several facilities in the Southern Indiana area in the past few years and this was hands down the worst. I would never recommend this facility to anyone who cares about their loved one.

This place is trash!!!!!!! My father is currently there. Bandages have not been changed. Called to ask why and the nurse stated “ we were just to busy today to change his bandage”. His belongings have came up missing... They do not shower him properly!! Do not ever place your loved one at the facility!!! Int he works of getting him out of this terrible place!!!!

Was told before placing my relative here that “great” care was given. IT WAS NOT! Housekeeping is practically nonexistent. When asked about cleaning the room was told they were short-staffed. In my opinion,whatever their issue with being short-staffed is NOT the paying resident’s problem. My relative’s clothing was taken. Even with her name being on everything she owned. Chargers for electronics were misplaced,never to show up again! She was taken to hospitals,not calling me,even though they were supposed to. I am in the process of finding her a different facility. If you’re smart,you won’t subject your loved one to this horrible place.
