This place is amazing. The care is the best. They are beyond thorough. Their kindness is so appreciated. My grandmother can be a challenge and they lovingly overcame it all, and give constant reports on her well being. I thank God for them. Because of them I can sleep at night. She is kept clean and well fed. Extra precaution is given because she is a fall risk. The bed is even lowered to keep her safe. I can't say enough kind words. Even when they are overwhelmed, they never change their kind professional disposition and show kindness to the rough tenants. I wouldnt change a thing. The place is beautiful, clean, I have no idea how they keep it so pristine. My grandmother can be antisocial and yet they get her to enjoy the activities which keeps her mind strong. So grateful. Send your loved ones here.

I visited my love one there by the name of F.B she is a trach patient there on the second floor. I had encountered with nurse glenda, Cna april and their manger meiata. They were all helpful especially the manger. I could go to the manger and she would stop what she was doing and assist me whenever I needed her. She was so kind, this is my first time having my love one in a facility like this. I was nervous at first because I didn’t know what to expect. But the manger made me feel welcomed! My love one is doing great! Everyday she up and dressed as if she would be on a regular. The staff there is polite and professional. april is a great cna and glenda is a excellent nurse! Kudos to there manger and her team on the second floor!

This place looks nice in the lobby, but the floor that I visited made me sick to the stomach! the carpet was nasty-I mean NASTY. Dirty, stained with GOD know what. Staff was rude as all get out. Relative said the food in not good.

Health Providers (Dr., RNs , CNAs, Therapists, Administrators) poor communication and coordination to provide adequate care for patient. Poor cleaning of patient causing infections. Little housekeeping (dirty and spills in floor and furniture). No plan of care established or updated provided to patient and POAH. Very short of RNs and CNAs. Main floor us beautiful; patient floors and rooms totally depressing in halls and unsafe with unmonitored patients entering someone's room. Very slow response to patient's call button. Elevate is not a care facility for short or long care. My relative, a short-term patient for rehab, only wanted to stay because of the care of the therapists and her physical improvements. She died.

My sister is at Elevate Care Country Club Hills and she needed assistance for everything in the beginning. Now, she can get up, go to the bathroom by herself, and walk around a little bit. The place itself is nice, it's clean, but within the first seven days she was there, the medication that she takes as part of her cancer treatment weren't didn't given to her for five straight days. Then in the second week, for a day and a half. So, she missed treatments. There are a lot of things that they don't do. She was having diarrhea because of the stool softeners they were giving her, which they didn't know that she didn't need. I have to take food for her a lot of time because she said it doesn't taste good and a few times she ate something there and it gave her diarrhea also. Recently, I just saw for the first time a calendar of activities and it looked pretty full. I know that they had someone to do their fingernails and they were having some type of group thing getting ready to start when I was there another time. You would also see some residents hanging out in common areas, talking, and doing something. I just didn't get the time to read the calendar, because I had to attend to her. There are a lot of things that they don't help her with, that she needs help with. When she first got there, she needed assistance with cleaning herself up, and they wouldn't do that.

We moved my mom to a nursing home at Windsor Estates Nursing and Rehab. We've only had communication with one care nurse. That was it at the beginning and nothing else. We don't know how she's progressing. They have not communicated to us. As far as the facility, the downstairs lobby is beautiful. That's all we've seen so far. We haven't had any interaction with the staff, except for the receptionist. When we go there, the receptionist has been very pleasant. We saw the food for the first time on Sunday when they brought her down to visit with us. They brought her tray. She ate everything on her plate, so I guess it was okay with her. They didn't discuss her care plan. We want to know what type of care is being done for dementia. What they were doing to maintain her current state? What is she doing during the day? We're concerned. Nobody has discussed that with us. Lack of communication on their part.

My mother is in Windsor Estates Nursing and Rehab right now. I think the care is pretty good, but I can't go upstairs right now to see the actual living situation. They have a room where family can visit, and it's convenient. To make the appointment for visitation is a headache. She said the staff is pretty good; but she's suffering from dementia, so she has issues remembering stuff. They're doing physical therapy. I haven't been upstairs to see her living condition, so I have no idea. I haven't been able to interact with the staff. It looks well maintained though. I've seen an exercise area, a place where females get their hair done or get their hair cut, and an outdoor area.
