"Plainfield Healthcare Center was a very neat, clean, and friendly place. It's also spacious. The staff were very friendly and helpful. The facilities were very neat and clean. It seemed like they're in order. They had a courtyard and a barber and beauty on the inside. It's very close to restaurants, grocery stores, and convenience stores. There are a lot of different things over there."

"Having my dad in Chalet Rehabilitation And Healthcare Center was great. The facility was clean and the staff was friendly. He's got a shared room but they also have an option for a private room. He does some occupational, physical, and speech therapies as well. They have games and I've seen where they've had get-togethers, where the celebrations for the guests that's there, like maybe birthdays and stuff like that."

"I would like to begin my review with how overly pleased and blessed I am to have my mother at a place she calls her home. My mother has been a resident at Washington Healthcare since 2018. She suffers with many health issues. As a single caregiver it became overwhelming to try and handle her on my own. I made a visit to this facility due to a program they were having for the residence in 2017. The moment I walked in I knew if I ever needed a place for my mother this would be the place. Sadly, I had to make that decision after caring for my mother for 8 years and each day becoming overly concerned about her well being, safety, and health around the clock. Since my mother's stay I have seen nothing but growth and love. I realize it has been very hard on everyone since the pandemic hit our world. I began to really worry how this would affect my mother and our visitations. Although visitations were stopped during the time the pandemic began the staff made sure I was able to see my mother. A window visit or face time call, she always knew I was there for her. The staff made sure of it. Staffing did lower when the pandemic hit, but I must say the staff that remained showed excellence during their demanding hours of work. Double time, over time what ever the time they never missed a beat. With new staffing at Washington Healthcare, I remain super pleased and it's now the year 2021. Shout out to the Social Worker for keeping me informed and hearing all my concerns if any and attending promptly to whatever is needed. Shout out to nursing staff who also hear my concerns and help me understand my mother health changes. Shout out to the new head nursing director for the kind open door policy she has given also for any concerns or questions I may have. Shout out to the housekeeping staff. I have never seen floors shining and glowing as much as I do here at this facility. Shout out to all food services,CNA's, activity department. To ALL who give their time to make sure the residence are taken care of. I can leave my mother here with ease and assurance that she is in the best hands even during the pandemic. I know this review is very lengthy, but I must give this review because it is very important that people see this is a place where you can leave your loved ones and go home in peace knowing they are in the best hands. "

"This is such a sweet and wonderful place to visit. My grammy felt right at home here. "

"Although this facility may be cosmetically appealing to the eye, I can assure you the care my loved one received here was appalling. My loved one stayed at Aspen Trace for rehabilitation following a hospital stay. This family member was found several times connected to an oxygen machine that was either not plugged in to the wall or attached to a portable one which had run out of oxygen. My loved one also experienced a genital rash due to lack of bathing. My family was told bathing and physical rehab is regularly scheduled and often neither were completed. It was extremely difficult to request a chest x-ray, etc. when it was obvious our loved one was suffering from a chest infection or pneumonia. There was an extremely long wait once the call button was pushed, likely due to lack of staffing. The food was horrific and was usually delivered whole (if at all), not following the mechanical soft (pureed), physician ordered preparation. When my family requested help from the front desk staff, nurses, or assistants, it was as though you are personally inconveniencing them and an annoyance. My family is tight-knit and believe in being each other’s advocate. A member of our family visited every day, and it was the epitome of stressful. My family and I can’t caution highly enough to refrain from using this facility for yourself or a loved one. "

"Basically cheerful atmosphere in facility and staff. It was clean and well kept. I got good physical therapy. The staff was helpful and most were friendly. There does need to be a rule about the patients turning off or at least down their tv after 11 pm unless they are in a private room. I enjoyed being able to wash my own laundry. Activities were plentious but I was so sick that I could not participate. The nurses tried to do a good job keeping my meds straight. I got prompt responses to my call light. If I could not go to the dining room, I did receive meals in my room at meal time. I liked the patience and encouragement from the therapy staff. Physical therapy and occupational therapy was excellent. The social worker, Phoebe Lemonds, gave me helpful follow up advice for when I went home. She helped me get in home help set up. This has been an answer to prayer."

"I live in ManorCare Health Services. Maintenance is excellent; some housekeepers are good, and some aren’t. Sanitation is a little above average. I do a lot to make sure that if I drop something on the floor, that I clean it up. We have a council as residents, and today is the day we link together once a month. Anybody that has a problem, they bring it up there. Usually it’s either the laundry or the kitchen and sometimes the nursing staff because they don’t have enough people here. They’re currently under a remodeling phase on one of the hallways on the north end of the building. It’s like 3 hallways so they had extra patients from this wing and from the south wing. We are packed, and I think they need more aides. Depending on their outlook on their jobs, some staff are just here for paycheck; you can tell by their attitude about things. On the rehab side, half the hallway is private rooms and on the other side are semi-private rooms. The rooms are big enough even in the semi-private rooms. We have a good manager in the kitchen, and she does her best to take care of things. I’m on a mechanical soft diet because I have no ridges for the dentures to cling to. The evening cooking staff is not as good as the ones in the morning and at lunch time. Most of the time, I get cold food and sometimes the vegetables are not cooked or steamed enough so I cannot bite into them. Activities are well scheduled and are pleasing. If you want to play games or if you need an educational thing for the older staff, it works out pretty well. They’re very diligent to do the best they can with the activities. "

"I was in a rehab at Countryside Meadows, and they have excellent rehab. I was in there after a fall with a stretcher, and I was able to walk after two and a half months. I would recommend the therapy and rehab department to anybody. It's excellent. They were strict as far their therapy, and they were all dedicated. There was no sitting around and looking at each other. The nurses were just outstanding; they know just what to do. They answer the call light, and they just don’t let you lie there. Most of the time, the food was very good and too much. I enjoyed my stay."

"Lynhurst has been a real blessing to my family! They may not be the prettiest place around but they make up for that with such caring and loving hearts! Staff to resident ratio is wonderful. The Administrator and the DON are really nice people. I love Lynhurst for everything they have done!"

"My mom is at Wellbrooke of Avon in the nursing section. It's clean and nice. The people here are great and they seem to provide a lot. She can't do much right now, but they came around saying that they're going to have like this one night or something like that. She can't do anything, or she can't eat anything. She's just on a feeding tube. She can't be in the meals together or anything like that yet. The menu looks good but she's just on a clear liquid diet period right now/on a feeding tube. I just know they had set up to have some kind of night, like Mardi Gras or something."

"Wife spent two months here. Newer facility. Like most, I always say nice place to visit but not to live. No place like home. Some staff went above and beyond, some just do their jobs."

"The care that our mother received was above and beyond our expectations. The staff treated her as though she was their mom. They took great care in ensuring she was comfortable and that we knew her health concerns on a daily basis. They would spend quality time explaining things to us. It was like we were family and we personally knew 12 of the nurses who cared for her. Even during her passing the nurses gathered at her bedside until we arrived. The staff is top notch. "
