I didn't like Park Avenue Extended Care Facility at all. They were very impersonal. It's one of the places where I didn't think that they really care too much. The place had a very bad smell to begin with, which tells me off the bat they're not really keeping up with the cleanliness of the place. I didn't like when I went in there and we were seeing diapers on the beds and the beds were unkempt. I wouldn't put any of my family there. I'm Puerto Rican and I'm 72 years old, and I know what racism looks like, and I saw it in the lady's face. I didn't feel very comfortable with her at all. That kind of mentality stuck out very strongly in my mind, and I choose not to have my family anywhere near that to not be given any attention because they're Hispanic. They need to change the whole managing staff because they don't know what they're doing or they are just turning a blind eye. They've got to get people there that are more sensitive to other mixes of people because I've noticed that they have a large mixed population, which is good, but from what I'm seeing, they're not being taken care of properly.

The place is well run. Good rehab and the nursing staff was very caring. They had lots of activities which kept my aunt busy during the day after her rehab appointment. She was only there for 3 weeks and was able to come back home. I would recommend this facility

After a recent fall my mother needed physical therapy/rehab close to home. This was her first time in a hospital/facility in over 50 years. The care received at Park Ave was attentive, supportive, and caring. Alana, the social worker works incredibly hard for her patients to secure after care and ensure long term needs are met. Colleen, in PT is excellent. My mother was eager to attend PT every day, worked incredibly hard with her and she gained confidence quickly. The facility itself is maintained nicely, very clean. There were days the aides were clearly short staffed and overworked, but they did their best. Health care shortages exist everywhere but I would say Park Ave is doing better than most.

Given my husband (63)was diagnosed with FTD we could no longer give him the care he needed at home Park Ave has taken wonderful care of him. My children and I leave from our visits feeling good and happy that he is in a facility that understands him and is condition. The staff keeps him clean and presentable for when my children visit (which is so important for them). I can't say enough about the wonderful care he is getting compared to the first facility he was in.... Thank You Park Ave...Rosanna Palette

I was recently at the facility for rehab. The physical therapy was excellent and got me up and walking quickly. My room was clean and the staff on my floor were very nice and checked in on me often. My social worker Victoria was very kind and made all my arrangements in order for me to go home . I would recommend this facility . The care was great !

I had the pleasure of having my grandfather here for subacute rehab. The rehab team was phenomenal. Very patient, encouraging and engaging. In between therapy the recreation department had activities to keep everyone busy. Grandpa who was always refusing to participate in recreational activities developed a new found love for Melisa in recreation. With such a lovely spirit who could refuse her. I want to thank all the staff at Park Avenue because together they made it a great experience and in the end Grandpa was able to go back home safely and happily!

What a wonderful skilled nursing facility. My cousin was there and not only is the building immaculate the people caring for him showed such kindness and compassion, it was impressive. We would pickup coffee and treats on the corner at Starbucks and sit in the beautiful lobby and catch-up. He was on the top floor and we also enjoyed looking at the Ocean and the bay, such beautiful views! If you have to have rehab what a beautiful place to do it in.

As a Long Beach resident it was important for me to choose a facility for my mom close to me and my sisters so that we could visit her often. We were recommended to Park Avenue and it was the best place for her where we saw her thrive in such a friendly caring environment. Very different from the previous facility where she seemed to decline mentally and physically.quite rapidly. During Covid although we were unable to visit her we were updated on how she was doing and we could FaceTime with her on a scheduled day and time. We know she looked forward to these visits and she got to see her children and grandchildren. It was wonderful for us to see her laugh and smile. We are so thankful that we made this choice for her as her new home where her caregivers really care for her in a very nurturing environment.

This place was clean and the staff were kind and caring. They often called me to give me updates on my aunts care. She didn’t participate in the activities they had because she’s a loner but she often said how nice everyone was. She came home with homecare but if needed she would definitely go back.

My grandmother was there for several months during the hieght of the covid pandemic. While we were angry we couldnt visit we couldnt say enough about how great the care was. My grandma had nothing to complain about. The social workers helped me speak to her with facetime and the nurses called me regularly to keep me up to date. I would recommend this place.

I dislike giving negative reviews, I really do. I have been rooting for this place since before my gma was placed there. The minute she arrived, nothing good has come from here. They are always inattentive and aren’t strict with their COVID regulations as they pertain to their staff. They restrict the families from coming to visit as a safety precaution, while their staff is inside infecting the vulnerable patients. I have a list of complaints and I wish I had atleast one thing good to say. I am so heartbroken that this is the only place where I may be heard. I hope things change. I really do. Too many of us are trusting that our loved ones are being taken care of properly, when in reality they are often being neglected. I find myself calling the front desk at least once a day because my gma calls me telling me she has to us the bathroom and no one will take her. It’s sad.

I had a relative here and he was very happy. He was sent home after he finished rehab and was walking and able to function at home. Only took two weeks. The care and attention was great. Room was spacious and place was very clean

The only thing I can say about it, is do not send your family member there. In past years, it was a excellent place and better than many out there. Recently, leadership has changed and they do not care about the patients at all and only care about the numbers. They do not take proper precaution, you cannot get through to the floors, doctors don't call back and the administrator does not care at all. Negligence is the best word i can use to describe the recent care of my family member.

Run from this place. One star is too much! Everything about this place is disgusting. Julia, the social worker is useless, did not advocate for my mother at all. They called a "family care" meeting after 4 days to let us know my mother would be discharged in one week. That was the meeting! My mother could still not stand up by herself or walk. The nurses are nasty. A few nice aides, Elizabeth and Mrs. Puka. My mother was in the dining room (if you can call it that), asked to go back to her room to go to the bathroom, they kept telling her in a minute, she then had an accident, and then the aides argued in front of my mother over who was going to change and clean her. A table was dirty in the dining room at lunch time, when it was dinner time, that same table still had the same dirt on it from lunch time. Some residents were still in the same position from earlier that day. The food is gross looking and horrible tasting. Breakfast one day was hard boiled egg on a dish still in its shell. That was it. The rooms are ehh.... the first day my mother was brought to her room, the floor was dirty, took 3 days for the floor to be cleaned. Like I said.... Run from this place.

My mother is adjusting to Park Avenue; they've been nice. She is getting therapy there. I chose it because it was extremely close to my house, so I could pop in a lot. She did rehab there before, so I was familiar with the place.

MY MOTHER HAS BEEN IN THE FACILITY FOR 3 WEEKS. IT SEEMS THAT THIS IS THE ONLY REHAB WITH A HYPERBARIC CHAMBER ON LONG ISLAND. SHE IS 89 YEARS OLD AND CANNOT WALK WITHOUT A WALKER. THE FACILITY IS SHORT ON MAINTENANCE STAFF AND CLEANING STAFF. THE FOOD IS AN ABSOLUTE DISGRACE. I AM THERE EVERY DAY BRINGING IN 3 MEALS. I AM GOING TO WRITE THE STATE OF NY TO INVESTIGATE THE MEALS SERVED TO THE ELDERLY IN THIS FACILITY.

I enjoyed my stay at this facility, the room was very clean and bright. The nurses for the most part were helpful. I really liked the director of social work she was very caring and helpful. I would recommend Park Avenue Extended Car

There is no staffing. If your family member is unable to take care of bathroom needs they will be sitting in their own filth. I feel one hour is way to long to "hold it" when you are 89 years old. Also you are unable to call. The only number is a switchboard that is apparently not always manned as we have called for hours trying to reach someone. Also have left numerous messages for social worker with no call back. If cleaniness is all you want this is the place.

The staff during the week and weekdays at Park Avenue Extended Care seems to really care about the patients. I cannot quite say the same about the weekend. They kind of just do what they have to do and no more. We have seen a few of the rooms because my in-laws have been moved around to different rooms, but they are shared rooms. There is one other roommate, decent size, they are pretty large, they all have windows, and if you know Long beach at all, it is on the water so they usually have a nice view. There are some residents who have been there a long time, and have some personal furniture in there and have made it really cozy; other rooms that have nothing personal in them -- they just look very hospital like. I have not eaten there myself, but whatever they are serving must be decent because my mother-in-law is eating it, gaining weight, and getting healthier. The dining room is a pretty big space. It is probably the size of four or five of the rooms, it has lots of windows, big open space, lots of round tables where people can sit together, there is always bright light, and it is just welcoming. It seems to be a pretty safe place. They don't have security guards, but I have never felt in danger of walking near there or walking here.

Caring, compassionate, and competent describes our experience at The Park Ave Extended Care Facility. After suffering a stroke at the age of 86, my mom lost her ability to walk. From the moment we arrived at the facility, we where greeted be a kind and courteous staff, sparkling clean facility, and top notch medical assessment and treatment teams.The talented and skilled men and women , assessed her condition and proceeded to address every one of my mothers medical conditions, identify and treat all of them simultaneously and it worked! She recovered her ability to walk! Every aspect of her stay there was carefully planned and customized to my mom individual needs. The entire staff worked as a team to provide the best care possible. From admissions to administration, the treatment team, housekeeping, all of these professionals are the best in their field. I truly thank all of them for giving my mother her life back.

WORST PLACE EVER. DROPPED STROKE VICTIM ON THE FLOOR. HAD TO BE RUSHED TO HOSPITAL WITH ACUTE BROKEN HIP. TRYING TO GET HER OUT OF THERE TO A BETTER PLACE CUT LOTS OF PAPER WORK.
