I saw an ad, and called Horizons, and then they contacted me. They do activities with them. They do outings, and they get my mother's meds for her. It's a very nice little community. The staff is very nice and very pleasant to talk to. If I call there with a concern, they answer my concern. I have no problem. She is very pleased. She likes living there. She has no complaints.

The staff is very nice and very pleasant to talk to. If I call there with a concern, they answer my concern. I have no problem. She is very pleased. She likes living there. She has no complaints.

My brother lived in the Horizon Assisted Living Facility for 7+ years. It was a perfect match for him. He especially enjoyed meal time with the food tailored around his diabetic restrictions. When he was living alone, his diet was processed food and TV dinners eating and taking his medicine when and if he remembered . His driving skills declined and became scary. At the Horizon, trips to the grocery store were provided along with in house visits with Eldercare Physicians and the Physician Assistant. He had his haircut in house with PT and OT support when needed. Having him at Horizon Assisted Living Facility gave us peace of mind knowing he was happy and well cared for. A wonderful, caring staff kept us informed of his health and his occasional falls. We knew that we would receive a phone call from the nursing staff if there was a concern We don’t believe he would have lived to 88 years if he had not been living at Horizon. And we don’t like to think of our life if we didn’t have the blessing of Horizon caring for him. Hugs and thanks to the administration and staff of Horizon.

The rooms are dirty staff unfriendly food not good cold not up to the money you pay

Horizons has been very accommodating in answering our questions. Any concerns and questions we have, they're very quick to respond and get back to us. We live in the area so we're not long distance. We go up and see my mom quite often and they've just been very wonderful to work with. Their staff is consistent. You see the same people day after day and the same staff week after week, and that leads to a nice continuity. The care is very good. They communicate well. We've had to have a few care meetings because my mom's care changes from time to time, or we have concerns from time to time and they're very good about bringing in the folks that can talk to us about her care. We have not had any issues that were not addressed or discussed. They offer the things that my mom needs. They have a doctor who comes into the facility so we don't have to take her to doctor's appointments. They have hair care and hairdresser services. She can go to appointments and everything. My mom is not very mobile, so having all those services at the facility has been wonderful as well. Mom is on a modified diet and they were able to offer her that, whereas most other assisted living facilities would not. That was huge for us. The facility is very clean and well-kept. They have a lot of activities and different things going on. She doesn't participate in very much of it, but they seem to do a very good job with their calendar of entertainment, games, or things for the residents to do. Their price is comparable. We had very few options when considering where to take Mom, because other assisted living facilities would not accommodate her modified diet and we are perfectly happy with Horizons.

Please don’t send any loved ones to Horizons assisted living facility in Cananadaigua! They will promise you the world only to kick your loved one out after 5 months. They did a full evaluation of my dad prior to accepting him, and had a report from a rehab center. Also I was very transparent with them about all his issues. They said no problem, we can work with him. This past Sunday after my dad fell and was sent to the hospital, the director had a nurse call me and say they refuse under no circumstances to let him back in. I understand places are short staffed, I get that but don’t let us think it’s going to be a great place for him, where he will he taken care of, only to treat him this way. A week before Christmas they told me I had 30 days to get him out of there and find him a nursing home, which I applied to over 10 places , but wasn’t enough.

Horizons a assisted facility located in Canandaigua, New York in my opinion sets a great example of how caring the the people who work there should be. When visiting my mother-in-law I met many older people who were very active, and had very interesting stories to tell about themselves. I always remember the first day we took Mom to stay she mentioned that she was 92 years old. The first lady to introduce herself to Mom said, "Oh! I am 93 and I take care of all the flowers around the outside of the building and you are invited to help me." This seemed to be the attitude of all the people I met at Horizons. There were an abundance of activity going on a lot of the time. The food was great and one could have a glass of wine if the so elected. There were bus trips to places around Canandaigua, like church on Sunday and other activities of interest scheduled. I know a couple (man and wife) that recently chose Horizons for the home they are pleased to spend the remaining days of their lives together in. Strict rules about being able to do certain things for yourself are required. You must be mobile, and that means you have to get around on your own two legs or using a wheel chair. The building is very well maintained and has a pleasant atmosphere all around.
