The rooms at Provision Living at Canton did have a patio that led outside to a courtyard so my parents could let the dog out and that was important to us. The facility was beautiful. The staff really seemed to know the residents well, like the person who toured us knew the people in the building and they knew her. The residents were coming up and talking to me and telling me about living there. It seemed very homey. They had a salon and a library.

Do not let the clean facility and nice decor fool you! There are too many bad experiences to count, but it all started with a tour of the facility with the white glove service. Many empty promises around the care of our loved one, amount of attention they would provide to his needs, an infrared security system in his apartment that we can view whenever we needed to see how/when he fell and even checking his body temperature sold us to place our father at Provision of Canton. When we moved him in, this technology was not even installed for a long time and when it was, we were never shown the footage we requested and there were many instances that the staff claimed they simply didn't know what happened to him when we saw bumps and bruises or clearly knew he had a fall and our family was not contacted to report the incident. The chef was wonderful and made great food, but he left shortly after and several kitchen staff did as well. Same with the Director, Director of Nursing and many many caretakers. It was a revolving door of staff in the 2.5 years we had our father here. Entertainment was lackluster or not happening whatsoever. Items were often missing that we purchased, including toiletries, clothing, medical items and more. Laundry was often not done to a point we had to wash his own clothes so he had something to wear other than the dirty shirts he would have on for consecutive days because nothing was clean. The care teams were almost never consistent, and there were only a very small handful of those caretakers (we're talking maybe 4-6 total) that we actually knew were taking care of our father properly while being stretched too thin or reminding them on what to do (such as make sure he has water in his oxygen tank to work properly, or to cut up his food per the medical orders so he didn't choke) Basic things.. He was in memory care and we'd never see more than 2 caretakers in his wing at once, to handle up to 14 residents at a time. We asked why there was not more help for residents who needed more attention, and they told us that was appropriate for the government standards. Many of these caretakers barely looked up from their phones, or were too busy putting a puzzle together, SLEEPING. If a resident was confused and wandering around, they would remain seated and call his name while shaking their bangle bracelets as if he were a dog and she was calling him to come back. One of the worst situations we had, was when our father was nearing end of life with stage 7 dementia, and he was taken by a man in a van claiming he was there to pick him up for a doctor appointment and they just let him go with him. He was ALONE without a family member contacted to make sure he should be leaving the facility unattended. There was no doctor appointment and it took them weeks to return our call to address this situation. We NEVER had him leave unattended without family and we are thankful that somehow he made it back safely. This situation and many many others were brought up with the director several times, and they often avoided our phone calls and visits to address these serious issues. My family had several meetings with the team, including regional leaders and they just listened to us, gave us lip service but most of this was never improved, and then something else would happen. They always said something along the lines of "if you have any concerns, please just call our office and we will work with you". It's very disheartening considering the money and effort put into placing a family member somewhere safe, when they can't advocate for themselves. These places are not cheap, especially when we are paying double to be in memory care, and the level of care we were promised was absolutely not what we received. My father recently passed away and not a single person from the facility called to check on our family or to send their condolences, other than the few caretakers who we highly respect and appreciate for caring all while they are burnt out with the heavy workload they have while being severely understaffed. Please take this feedback seriously, and consider sending your loved one elsewhere.

We've moved my parents to Provision Living at Canton. It's been good. Setting it up was good. They are super friendly. There were a couple of disconnects like getting the TV set up, silly little things like that, but for the most part, we've been really happy. It's only been a little bit, but they've been very attentive and very nice. Everybody there has been super to work with. Interaction with the staff has been excellent. Their rooms are really nice. They have a couple of different options. My parents are in a one-bedroom with a little living area and an outside patio. The bathroom is really nice. It's clean and brand new. It's a one-story facility. It is not huge, and it's really nice. They have two-bedroom floor plans. They have rooms that have a door wall with a courtyard view, which is what my parents have, and then they have inside rooms that don't have that view. They have a couple of different layouts for that. They also have a studio, which is a one-room type thing, and we didn't look at one of those. They have activities that go out of the facility, so they'll have meals at a restaurant or they'll go to a park or different things like that. They have movie nights within the facility. They have bingo, games, puzzles, and things like that. They have a beauty salon. Based on what I'm paying and based on what my parents would have needed had they stayed in their home, I'd rate the value for the money five stars.

I would have wanted to know that the staff is constantly turning over, and they are "short staffed" most of the time. The Director and Marketing person "disappeared in the last 3 months. There have been 4-6 chefs in last yr and a half. Quality of food varied with the chef who happened to turn up. LPN who is supposed to be on call 24/7 is NOT. Staff tells me she doesn't want to be bothered when she's at home. Meds are given by "med techs" with minimal training. I ultimately moved my loved one to an older facility with nurses on duty round the clock, good quality food, and resident assistants on duty round the cloth. And the monthly charge was $2000 less a month. Management changed your monthly rates without giving 30 days notice, I found my loved one dirty--body and clothing several times; noone answering his call signal, so I took care of him repeatedly.

I would have wanted to know that their ONLY nurse, a LPN is frequently unavailable to the staff! I would have wanted to know that there is an ongoing shortage of resident assistants! Frequently, I was stopping and getting my loved one out of bed, and to breakfast. Meds are passed by "med techs" with minimal training. I would have wanted to know that the chefs turn over on a regular basis, and currently there are ongoing complaints about the quality of the food that is served to residents. Bottom line is that "IF" your loved one required minimal assistance, this place may work for you. If your loved one needs more than that, you should look elsewhere. I found an older residence, without all the bells and whistle, that has nurses "round the clock, and they pass the meds and supervise the care. The food is excellent, and the price is $2000 less than Provision. I would recomment you pass on Provision/Canton!!

We liked Provision Living at Canton because they had the memory care, I liked the way they trained their caregivers and some of the protocols that they had in place. It was new, so the facility looked nice. The staff was very knowledgeable and they understand memory care and dementia. We met the chef and we had cookies and they were very good. They have a salon, nice seating areas, and exercise classes.
