This place was absolutely disgusting. Full of cockroaches so bad my aunt was afraid a cockroach was going to crawl in her mouth while she sleeps. They didn't clean her bedding for a week and hardly ever checked on her. Terrible.

My dad is still at Consulate Health Care of Port Charlotte. They're working on Medicaid for him. As far as I'm concerned, they seem to be treating him very well, and he seems to be happy there, so right now, it's fine. He is in a shared room right now. He is great with the staff. The staff takes good care of him. The place is clean. They can go outside, and there's a courtyard there.

My mother was admitted to the Port Charlotte Consulate Health Rehabilitation Center from Fawcett Memorial Hospital after recovering from Pneumonia and Heart Complications. My mother was in Consulate for 2 1/2 days. On day two she was calling us complaining of severe abdominal pain, sharp in nature and vomiting. I called the Director of Social Services, then called the nurses station requesting someone help my mother, my brother said he called the nurse's station requesting help for my mother. That night at midnight we learned she had been transferred back to Fawcett Hospital in Port Charlotte where she underwent emergency surgery for a perforated gastric ulcer causing peritonitis and putting her in Septic Shock. My mother died the next day from Multi Organ Failure, Septic Shock. I feel this could have been prevented. I live very far from here and was only able to visit my mother for 3 1/2 weeks before her horribly painful death.

My wife was here more than a year ago and she had some difficult problems and the staff gave her terrific care, I am now here and I can say that most of the staff know me from when my wife was here and they are very intuitive about my needs and likes. The place is kept clean and needs are met almost immediately, I have found that most meals are good and when I have a problem with something they serve they go out of their way to make something I'll like. Please remember that this has been a very difficult time especially for a facility such as this but I would say they have come through it with flying colors...Bravo Consulate. Mr.Scott

First I would like to thank the CNAs that work at the Consulate. They are very dedicated, caring and passionate to the care of their residents. They have an activity room which they seem to always be doing something to keep the residents occupied and above all happy. They are beaming with pride showing off what they have done that day. They also have group outings where they may go and get something to eat and a movie. That is just fantastic for the residents and family members knowing they go beyond the facility in their own Consulate bus. I was pleasantly surprised to see the giant Halloween inflatables, different sizes, in place by the side of the building with a large bowl of candy for trick a treaters and of course guest and family members. I can't wait to see what they do for Christmas! This facility has worked hard to obtain the status of JCHO Gold Seal of Approval. I've worked at a hospital and know how much work is involved to obtain this very important Gold Seal of Approval. It is a very clean facility. I have to say this, NO ODORS, which is the norm in some facilities. My husband is a resident and if I have any questions or concerns they are immediately addressed. I would recommend the Consulate of Port Charlotte to any family member. It's like a small community within itself Thank you and keep up the wonderful work and interaction you have with all your residents.

Do not use this home if you or your loved one needs constant oxygen. Apparently they do not find it necessary to make sure the oxygen line reaches into the bathroom. AND they don't seem to care if you're left in the bathroom withOUT oxygen for over half an hour at a time. How's THAT for good care?? And don't be surprised if the staff hangs up on you if they don't like what you say on the phone, either.

I was in the Consulate for a month for rehab. It was a park setting. I ate what they gave me, and it was not good. My daughter had to bring meals in for me. I didn’t like that, and it was important that I could eat what I want. The service was great. I really didn’t have any complaints with the nursing staff. They also had a bus.

When mom first had her stroke, she was in Consulate of Port Charlotte. I had some problems with billing and administration. She fell when she first went in there like two to three times and had to be rushed to emergency. She didn’t realize she couldn’t walk and tried to get up. She had several accidents there. The dining area was clean, and the food was good. The nursing staff and the social workers were really good and kind and did good things. The beauty shop was good. In the end it was really good; I just moved my mom as it was more convenient to where I worked.

I visit Consulate Health Care in Port Charlotte on a weekly basis. I can always find an empty parking spot, the grounds are well tended, and the waiting area is clean and inviting. They have a coffee / infused water bar right at the entrance so you can get a hot drink or cold lemon water coming or going. The receptionist is always polite, too. There is an activity room painted with underwater scenes. The rooms are clean and the staff is always helpful. The dining room could use a little redecorating; it's a bit bland, but it's clean and serviceable and the tables aren't crowded together too tightly. There are little common rooms near the nurses stations where residents can watch TV or visit. A lot of residents just seem to be parked in their wheelchairs by themselves, but that appears to be typical of nursing homes. Personal items also have a tendency to walk off, so it's important to label your stuff so it stays with you. Overall, it's a good facility.
