I can't even give this facility 1 star but had to to complete this review. Anyone who has a loved one needs to avoid this SNF. Rooms are outdated, in need of paint and furniture is dirty. The smell and upkeep of the facility is beyond poor. The staff had poor response time in attending to my loved one and sustained a life threatening injury during the stay. We were in the process of finding another SNF when the injury occurred. Families please do your due diligence by visiting SNF's before hospitals rush you to decide on any facility.

My father’s room wasn’t cleaned for at least 10 days, I visited him on 1/4 before going on vacation, returned from vacation on 1/14 and the same trash plus more was left on the floor, the floor was nasty when I walked my shoes was sticking to the floor. The food wasn’t tasteful per my father always cold, small portions, no presentation The Administration is rude, disrespectful, unprofessional, dishonest The Business Office Manager rude, demanding, wasn’t consistent with daily rates I wasn’t aware My Father wasn’t receiving therapy from 12/14-1/14 until he told me therapy hadn’t been in for a few weeks . I called on 1/15 to inquire from the Director of Therapy, she informed me that the Administrative told them to stop therapy because he was afraid they wouldn’t get paid because my father’s insurance had ended service, after I had given the information that he had won the appeal to continue service, she was very caring and concerned saying she would call the Administrative. I informed her that he wasn’t in per the reception due to me just calling and had to leave a message and I asked her if she got in touch with him to ask him to call me, or I would be calling Corporate. I received a call from him within minutes, still rude and disrespectful. I was voicing my complaints about my father’s room not being cleaned, 4 sets of soiled laundry left in plastic bags that I literally had to throw away informing him that my father isn’t be checked or changed during the day, he rudely told me your father know how to ring the bell if he needs help, and my staff do not have time to check every patient every two hours or frequently per my request My Father missed his transportation to and appointment, due to staff not having him ready, after notifying the staff the night before and appointment posted on his wall, I wasn’t notified I had to call to see if he had left because he hadn’t arrived On weekends my father would be in the bed after 2 because the staff would tell him they would be back to get him up and never come back The phones aren’t answered after 7pm business hours I called corporate with my concerns on 1/15 I have until 1/29’ to hear back from someone, at this time I haven’t received a call back. There were a few caring staff and I do appreciate the care they providing to my father, if they were there I knew he would be well taken care

Room equipment (beds, stands etc.. are outdated, but serviceable). electrical outlets are few in number and need upgrading, beyond these things, I find the staff pleasant to patients and guest. the care my relative receives is excellent and my relative is comfortable and content. I don't think I could have found a better placement for her.

Facility is very dated, and rooms are just okay. As in all places, some staff is wonderful and some, not so good. One head nurse was very rude and impatient both to patient and family. Food was brought to the patient with few attempts made to help patient get situated to eat. Medicines not given in a timely manner. PT was okay, but did not encourage patient to do more each day. Patient left facility with little improvement after being there almost one month. Case Mgr was cooperative and caring right before discharge. This facility is not one for any rehab other than PT.

I am/was a resident of this facility

I was a resident at The Laurels of Bon Air after breaking both ankles. After surgery I needed physical therapy. I could not put weight on either leg when I arrived at the facility. I was dependent and needed assistance. I experienced kindness and helpfulness by the staff. I enjoyed the meals. I got used to the routine. The physical therapy was my way back to regaining strength and ability to walk again. The recovery is ongoing. I am especially thankful to the occupational and physical therapists who worked with me. They have patience and help to motivate you to appreciate every milestone you make. All the staff were pleasant and encouraging. The rehab staff is a great team. My son came to town to support me. I had encouragement from him and his willingness to support me was amazing. Friends and family can make such a difference while you are getting rehab. I am grateful for the care I received. The medical staff helped me and all associates made my stay a great learning experience. I had a private room on the first floor which helped navigating the unit. I am thankful for everyone who contributed to my ongoing recovery.

I have a friend who is a long term resident at the Laurels. She is the happiest she has been in years. Great care. Can’t say enough good things about the staff that takes care of her. She has a large privat room that she has set up as a “little apartment”. Loves going to bingo and has actually Gained some needed weight since she has been there. I would recommend to anyone needing placement for a loved one.

The place was clean and kept up well. Room was Hugh. My mom was there for 5 weeks for rehab from Covid and pneumonia in both lungs. She also has cancer. She really liked the food which was a plus. However, we were sent to an outside doctor appt. with an empty oxygen tank; blood was taken for an emergency stat that sat on nurses station for five hours so had to be taken again; had to wait 1-1/2 hrs for someone to take her to bathroom in middle of night, among other things. Her 3-11 shift CNA’s and all nurses were fantastic. Left multiple message for doctor and NP to call and never talked to them. Director of nursing was not apologetic or very professional at all when I finally got to talk to her. Our family stayed with her 10 hours a day to make sure she received proper care. The receptionist up front and Allison in admissions were fantastic

The insurance stopped paying. I was told and had 4 days to find care for my mom. I was told by the director they have no control over the insurance. I get that, but as a director that’s what you say? It snowed the day I was moving her. She was very anxious so we cancelled til the next day. I received a $500 bill! No exceptions given here. The nurses and PT teams were awesome. The director and administrator are awful. Really lack empathy. Who charged $500 when it’s snowing and not safe to transport? The administration here does! Sad stuff. I feel bad for their staff as I’m sure if the management was awful to me as a worried daughter, I can’t imagine how the employees are treated.

My father is currently staying in The Laurels of Bon Air. It's adequate, and it's acceptable. I think they're lacking a lot of necessary things to communicate. They don't have individual extensions for their contact. If I want to reach the social worker or the admissions people, I've got to go through the operator, you can't call anybody directly. All you do is leave messages, and maybe the next day someone could call you back. I don't know if that's the facility or the staff, but they have very poor communications about technology. I haven't tasted the food, but my dad has, and I don't think he would rate it high. Some of their staff is extremely good, but it only takes one or two that are just horrible to bring that number down. They have one nurse who's just not good, but all of the aides and the other administrative people I've worked with have been good: but they're correcting that. Once I shared with them the details, they are making those corrections.

I visited this facility

Father was here for about 2 years. The people that work here are nice enough but no where near enough people with actual qualifications. Witnessed an aid slap my father, reported it. They sit the food down and whether they eat or not they do not care. They just come get the same plate later and keep on to next meal. Very robotic without concern of nutritional care of clients. Father got COVID, was tested and 4 days later it came back positive. No one in family was called until an hour before he passed away. They let my elderly mother go in to see him after 8 months of no visitation. She was not protected at all nor was my sister who was allowed to visit but only when he was on his death bed. No documentation was done as Nurse told me she was the ONLY ONE on the floor. I think they mean well but severly understaffed for a place that costs about $12,000 a month for my dad through medicaid.

My mother-in-law went to a nursing home for rehabilitative care after being hospitalized with a urinary tract infection. She was transitioned to a skilled nursing facility right from there. She is at The Laurels of Bon Air. It has been fine. They're doing as best as they can considering COVID has made their lives pretty crazy. We have been satisfied with the care that she has received. I haven't met the staff because of COVID.

When my sister went into this facility she was very sick and I didn't expect her to survive. 4 months later she is better than she has been in years. The proper meals, attention she received in health care (diabetes, COPD, obese, fluid retained, improper eating at home) has been the key. Her private room was an asset. Now, she may have to share a room and I am afraid this will put her in state of deep depression. I do not like this about the facility. She is a people person and is in her right mind. Sharing a room will be a setback.

My mother is a current resident at the Laurels. She has been there over 2 weeks, and I pray we can get her moved soon. There is not adequate staff there & they are rude to the residents. My mom sits in urine and poop for an hour at a minimum before being changed. One night she remained in a soiled diaper for over 11 hours. Multiple people in my family have called and complained. I will call the Administrator daily if needed, and I will be filing a formal complaint with Medicare.

Don't put anyone in this place. Mom sent here after broken hip. Not enough help!!!!! She received 15 min. of therapy when she got it. I could have done this for her myself. She would call after I left crying how mean the people were to her. On top of that when you tried to get any information from the people at the desk they would all disappear. I took my mom home and they called at 8 o'clock to ask me if she was with me. Needless to say they are very neglectful and not organized.

I would like to take a moment to express my appreciation for the overall caring process regarding my father who has been a patient at the Laurels of Bon Air Senior Living facility on different occasions. For the past couple of years, my father has undergone multiple surgeries that affected his physical capabilities as well as his inspiration. Although my family and I collectively made an effort to encourage my father during his healing process, I also have a profound sense of gratitude to the staff the Laurels of Bon Air for their professionalism, cooperation and understanding pertaining to my father’s physical and emotional wellbeing. Furthermore, I acclaim the “above and beyond” efforts of the Admissions personnel (Angel and Catherine) in ensuring a smooth transitioning process that my father underwent from the hospital to the Laurels facility. I also appreciate their patience and willingness to assist me with handling multiple issues concerning my father’s business in a prompt manner and with an ensuring attitude. For these reasons, I feel very comfortable recommending this facility for short or long-term medical/health necessities.

Unfortunately, there is not enough staff to manage the residents at the Laurels of Bon Air. Clearly the staff is under paid and there is no incentive to keep staff and for the staff to be motivated to give good healthcare. I don't believe that most of the aids realize what giving good healthcare even is these days. With that said, some of the nurses in white have poor HIPAA compliance. There is absolutely no patient confidentiality. The language that the women use their is appalling. Who taught them that using profanity in the work place was acceptable? Pay them more, get more staff, and give incentives to keep them there. Now on to my mom. I get off work and go see mom almost every day. There have been 3 occasions when I've found the toilet backed up with feces. On all 3 occasions there was no sense of urgency to clean up the situation. Mind you, mom has to go to the bathroom every time I WALK THRU THE DOOR and she's already gone to the bathroom on herself when I take her. Complaining only seems to make it worst for mom. I'm determined that they retaliate by not taking her every 2 hours as ordered. She has bed sores now. I'm hurt!!!! Please Help this place.

I visited this facility

I was recently released from the laurels after 21 days of impeccable care the staff was excellent, they went over and above their duties. I was in the Huguenot section I don’t remember everybody’s name but anyone who worked in the Huguenot was awesome I ws pleased with the food, the activities persons truly doing what she loves. the rehab room is fully equipped that the staff there was so professional and knew what they were doing and focused on helping you regain your Anyway been h three days and I truly miss being there I know that sounds funny but I live alone and the staff there was so fun to be around and they were so helpful and really miss being thereThe Sellitti’sanyie ere I want them to And recognition that they deserve I’ve been to a lot of facilitiwith when she was was ill and I ....know how they can Give credit to the Huguenot wing and this is an excellent place I would recommend it highly Sincerely, Ms Turnbell

This place is awful and neglectful. Basically the reason our loved one passed away. After they had a fall in the middle of the night due to no one coming to assist them, they broke their hip and were sent to the hospital and the staff waited 12 hours to tell us what had happened and that she was at the hospital.

I as one women said would rate this place zero. My. Mother-in law was there last Jan. with broken hip, I have to say there were really no issues. She a few weeks ago fell and broke the other hip and this was the first place we wanted at with no doubt. Well WRONG! My wife was with her mother and hit help button twice and they turn off. So finally she goes to nursing station and ask that they put Mom back in bed. they responded we are eating and will get her back in bed when we are finish. They never came so, my wife and niece put her back in bed. When they left out the back door the 2 women were outside smoking a cigarette's. Then she had family visiting from out of town and rang nurse as she needed to go to bay room. Well they never came and she ended up a mess before the aids got to her. She had to clean her self up in bath room. Ok there's more! My mother-in law call my wife and said the aid had the waste pail ready for her and she needed to get up into wheel chair and go wash, then she left. I know this is rehab but, she should have stay to assist if she need. Then dress her self and get back in bed wit no help. there's more but I think it speaks for it self. Today we call the assisted where she lives to evaluate her progress to see if she could come back to the home. Good news she out of there tomorrow. The person from CAL was concerned that see went in and saw no one and was ask now questions. Anyone could walk in and cause harm to anyone. One last thing the staff ratio to patents is should be 1 nurse and aid to 12 patents. It is 24. They have claimed they have government has no regulations for them. Our Aunt who words for Dukes has done every nursing position and now administrator now inform of this this evening. Mother-In -Law is an angel and they have treated her mean as result of our complaints. Stay Away From This Place!

Just a follow up about the trials and tribulations of dealing with this place. My Mom passed away September 27th. The funeral home sent the necessary paperwork to the Laurels so we can get the death certificate. Went by today, October 3rd and they couldn't find it. Eventually the information was found under a stack of papers. But the person who is required to fill it out is not there. Maybe tomorrow? Very inept rehab. Talked with a friend of my Mom who has been there two weeks and she hates it. She is supposed to have certain medication before breakfast and they have only done it twice out of 14 days.

Really upset the care my mother is getting here. She is 87, broken bones in neck, back and wrist. Require 2 liters of oxygen continually. She has to wear a neck brace because of the broken bones suffered in a fall. Today she was sitting in bed with no neck brace. Asked a nurse why and she said it was off when she came on. Doesn't anyone look at a chart?! THEN she doesn't know how to put it on. Had to get help and they still didn't do it right. A piece of it was not put on. Then they didn't put her back on the oxygen. When sitting up and sleeping she will tend to let her head fall to one side. Without the brace who knows what could happen. I had taken her to Virginia Eye Institute for a follow up Friday. Went to the nurses station to give them the new instructions for her eye drops and she said I don't need that. I asked shouldn't that be put in her file? No, give it to her nurse and pointed down the hall. This place is not where you want to bring your loved ones. Very inept staff.

I only have one star but honestly they do not deserve any. My grandmother was left on a bed pan for over an hour and when my mother finally took her off of it herself the very rude aide berated her and blamed her for the mess. As if this wasn't enough, my grandmother lost her balance and fell into a commode except that we were never informed that this occurred. After she collapsed we learned that she had sustained severe injuries that have now rendered her immobile. They're obviously understaffed and hire unqualified aides to step in due to their nursing shortage. She was constantly left in her bed, soiled for hours on end. Thankfully she was just released but the facility is so unprofessional and so oblivious they failed to submit orders for the basic requirements for her care - medications, a hospital bed, and a wheel chair. When we tried to push them to do their job and fulfill their duty we were given the run around. Now being released a day - they have still failed to submit the orders for the wheel chair and the bed. This facility is not acceptable on any level. There are two amazing nurses who stepped up and did what they could to assist us - Jeremy and Sonia. Aside from these two we have constantly been met with attitudes, inadequate care, and patient neglect.

The staff leaves clients on bedpans and takes a very long time to respond to the call button. They skip showers. They dont provide a therapy schedule. You don't know what to expect. Procedures are for the staffs convenience, not client needs. Breakfast is always late and lunch is always early. Very disappointing experience.

I would recommend the Laurels of Bon Air to anyone seeking post hospital care for them self or loved one. The entire staff seem to enjoy caring for others. My grandfather was in one of the many private suites, which included a flat screen cable television, wifi, phone, and little sofa. We were encouraged to visit and bring in personal items to make his stay more homelike, which we did daily! He met many other patients with similar medical troubles, which seemed to help him cope with his new limitations. The care and attention he received was wonderful, and he is now back home enjoying his grandchildren!

We were referred to the Laurels of Bon Air by several church members after my mother had fallen and broke her hip. The staff at the Laurels were very attentive and made sure she was comfortable and free of pain, which was very important as she has dementia and didn't understand fully what was happening. The rehabilitation and nursing staff were wonderful! They spoke to her, not at her and spent the time to get to know her, which made all the difference in the world. When she had a set back and had to return to the hospital for a few days, a representative came to visit several times and brought her a get well card to cheer her up. Mother was made to feel truly special. I would highly recommend the Laurels of Bon Air to anyone in need of therapy or nursing care!!

My wife has been at the Laurels for almost two years. There are people who do their job, and then there are people who do it just to get it done; it depends on the shift. There is a good bunch of nurses and RNs in there. They play bingo, they have pastors that come in, and they take them shopping sometimes if they are capable to leave the building. They take them to Walmart or to visit some places. There were better places, but they were too far away. Where my wife is, I can go in the assisted living, and it would be convenient; I wouldn't have to drive or anything. Overall, it is pretty good. They have made some big changes right after we got in there. The food has improved and everything.

I visited the nursing home facility. Is it true that that patient can't have a phone in their room? The rooms are nice size so why can't why can't they have some of their own furniture? Some personal items would make them feel like home.

Dad was at the Laurels of Bon Air. They didn’t bother to bathe him. They said they didn’t have the facility to bathe him. It's just disgusting. They just don’t take care of these older people. It was pathetic. Everything is clean and there's no problem with that. It didn’t have a smell when you go in there, but he got the same meal every night. He was on pureed diet and they never change his menu. He didn’t get the treatment that he needed there.

Mom was at The Laurels of Bon Air just for strengthening, an after-hospital stay to get her back feeling comfortable on her feet. The staff were very nice. Her room was fine, not luxurious, but it was clean. She didn't have any issues with the food there. She was happy with it. It is a bit institutional feeling. As far as everything else, it was fine.

A horrible place, my mom entered post heart surgery on a friday, no staff, no nursing care. Said no professional care until Monday? No one to help her in & out of bed so she wet her bed over and over with no response from staff, did not have doctors orders for meds or other critical care, in one day she degraded with lack of care to vitals crashing, and needing to call 911 to return to the hospital
