They lost my mothers shoes and eyeglasses and we were told that Elizabeth would handle it for reimbursement but after repeated visits and letters, we got nowhere in leaving receipts as we were told. $348.59 was our loss & no one cared enough to do something about this. I WOULD NOT - NEVER RECOMMEND THIS PLACE!

Great staff!! All new management that go above and beyond for the residents. Nurse know what they doing. STNA's are very responsive. Activities have a big calendar easy to read with great activities. I would 100% send my loved one back

I was taken here from Parma hospital after a stroke. I arrived around 3-4pm. I was randomly put into a room that was filthy with urine & human spit everywhere the blankets smelled so bad I vomited. My roommate was an elderly male who was naked most of the time & didn’t understand anything. The nurse came in once took vitals then left. After several hours my wife went out to ask for a cpap that was vital after my stroke & to inform her I haven’t eaten. The cpap came in from an outside source who set it up thank God cause they didn’t come back in until the next day to check on me. My wife made me stayed on the phone all night to check on me & make sure I was still breathing through the night. They also brought food in that was cold dried up pasta that got me sick. I asked to be moved & was ignored. When my wife came the next day with a walker, food & clean blankets & clothes they wouldn’t let her in. She called while in the lobby, no one offered to bring me the items. I could barley walk but struggle to the lobby where they started to yell at me about going into the lobby. So I walked out of that smelly filthy dirty, & unattended healthcare provider place. Please DO NOT GO HERE OR SEND ANYONE HERE. They may not make it through the night.

Falling waters is a terrible place. My husband was the in restpit while i went out of town for one week and they didn't give my husband his meds, food or any liquids. They left him in his room with nothing for 5 days then they either got scared he was going to die or someone came in on that weekend shift which was Fri and saw him like that. Then that's when i got a call asking for permission to put in a medline to give him fluids. He was there 7 days i went and got him he was all messed up and confused. But when he went in he was fine. I took him straight to the hospital and they said there was no way that place was giving him his meds that he needed to have to keep him from being confused. He was hospitalized for 2 months with tubes in him everywhere. He had to be fed through an IV line. Falling waters refused to tell me who was his nurse or care person when I asked. I was highly upset. I haven't taken him to another nursing since I'm truly afraid of what might happen if i do. Terrible place!!

Truly great and caring staff, they go above and beyond. Both my grandma and uncle have been here multiple times, and each time the experience was fantastic. I would highly recommend to anyone looking for both rehab or long term!

I am a current resident at falling waters for rehab and wound treatments. Nursing staff and STNA are awesome, and they truly try to provide the best care but due to COVID and shortages of staff its a challenge for all. I absolutely loved both physical therapy and occupational therapy, the staff is truly compassionate and caring. Not only did therapy get me back to my baseline but they also provided emotional support through these trying times. The DON is caring and involved, with a hands on approach with staff and patients at the facility. I had two concerns with my stay here and the DON addressed both immediately. Activities is kind of lacking right now but that is due to COVID. Once a week Kathy comes around with an activity to do which my granddaughter loves to get my finished project. They also play bingo in the hallways of our rooms where everyone can keep a safe distance. The food is not so good here. By the time we get our tray its usually cold. I think it would go a lot more smoothly if dietary helped pass out trays and they already had the beverages on everyone tray instead of doing it separately. Most of the time items are also missing from the tray, as in condiments that go with the meal they are serving. They also don't offer an alternative meal if you don't like what they are serving, unless you have an allergy to the food. Housekeeping is okay. Angela is a wonderful housekeeper but when she is off we seldom get our rooms cleaned, except for garbage being emptied from our rooms. The rooms are spacious enough. The activity room is also very spacious with a TV area, computer for residents use, and comfortable seating. I would recommend this facility.

If you want your relative to sit in a dirty diaper and urinate on the floor all day, this is the place for you. Only one number, all calls are re routed, if they are answered at all. The most I was able to communicate was when an aide let the patient use their personal phone. Medication took weeks to figure out. Had toiletries, but no access to them. Trying to transfer out, sent wrong paper work to keep them over the weekend. Finally got them out in a diaper and gown in 31 degree weather. Didn't even dress them. Was told they couldn't afford to change her diaper often. Was fully functional before they went in which was a surprise to them. Can't rate the staff, would never call back.

My husband was there for 2 months. I was promised by staff and hospice that when he became worse they would call me to be with him before he died . he died Saturday 31 of Oct. and no one called. On top of that it is now 6 days past his passing and no one still will call me. I have no clue why or what happened. It's been a nightmare and I even called a corporate number and no one there can help.

My mother has had such a negative experience at Falling Water. She has called me crying because the staff has been rude or even outright negligent. She's sat in her waste for hours on too many occasions. They often ignore her call light because her roommate uses it for no reason. Aides have told her that her illnesses are her own fault and that she's just being lazy. Sometimes they flat out refuse to help her with something. And she can't get out of bed or walk to help herself. The dietary staff ignore her requirements and send whatever they want, and it's always cold and disgusting. She's been accused of lying and has had things go "missing" from her room, including a rosary given to her by her favorite grandparent. The business office is sketchy at best; they won't let her do what she wants with her CARES Act money, and we've been trying for months to get her a new phone. The phone often rings for well over a minute before someone picks up so I can ask to speak to my mother. It's a little thing, but it shows their lack of consideration. There are a couple people who've been kind: aide Penny (such a great woman), some nurses (but not always), and the activities coordinator Megan. Everyone else is rude or simply inept. If you truly love your family, never let them end up at Falling Water Healthcare Center.

the staff was always there when needed and always answered questions to the fullest very knowledgable..staee was very caring..the nurses go beyond their job description to help u..the stna's there were so polite and helpful they always came when called..therapy's were awsome.they were always there with smiles on and ready to work ..their thoughts were more on 7us residents..helped me to walk so much better..i could go on and on commending them..i feel like i became family with them..though the food and dietary department could use some help my stay was perfect...ALL OF U STAFF BE PROUD OF THE JOB U DO THERE...Andrea..Heidi..Nicole..Victoria and Thereasa THANK U SO MUCH..Love all of u and tHANKS AGAIN'''' Robert S..iwill keep u all in my prayers

The place looks and smells like a bad nursing home the person who showed me around was as fake as they come The meal was horrible while some staff was pleasant .The person that led are tour was pushy and seemed to be in a big hurry. The residents that we spoke to were confused and in wheelchairs which seemed to be the norm definitely not a place for active seniors.The food was so bland it was inedible while the apartments are nice the community fee was ridiculous way overpriced considering other options in the area.

My grandmother was at this facility 2 different times for therapy and CHF after going unresponsive and being intubated at the hospital. Even at the age of 90, she was able to go back home! Very greatful for the amazing nurses and therapists!!

Staff was caring and compassionate. They really care and always trying to make me happy. Although the facility could use a make over with updated cosmetics and newer furniture.. what the place lacks in new looks it makes up for in the care. Any concerns no matter how small we're addressed and handled right away. Thank you for all you did for me while I stayed with you.

Some good staff, some who shouldn't be working with people at all. According to my loved one, staff gets behind the closed door of a patient's room and trash talks everything. Aides hide in rooms to use their cell phones. No follow through, family not kept up on changes. Building is disgusting- for the love of God, spend some money and spruce the place up. It's pretty from the outside, but that's it. Deficiency free is s joke. Leaves me scratching my head... Oh, but super nice therapists.

Falling Waters was an awful experience for my mom. The place is dirty and the food is atrocious. The staff is unprofessional and while my mom was there numerous family members witnessed aides yelling and becoming overly frustrated with their clients. This was witnessed at dining times and in clients rooms. The administrative staff was unresponsive to phone calls and inquiries.

The care for my dad was terrible, staff tried to convince us to stay, bad choice. Most staff dont seem to know what my dad needed, staff dont seem to care, the food is late. I am sad I let them talk me into keeping my dad there, we are leaving.

Therapy department was excellent, I was able to walk after only 6 weeks. Nursing care also very good, staff was pleasant and truly cared about my injury. Would recommend to anyone searching for therapy after surgery.

hi i was in there after back surgey. i didnt have a very good report for that. they let me fall two times and then i had a stroke. because when i fell i hit my head. but they wasnt going to take me to the hospital .until my husband and daughter made them call the squad.when i got to the hospital the d octor told my family to makearrange because i wasnt going to live.
