"Major problems: Relative put mom here, horrible place... Family member overdrugged so they don't have to give her therapy or help her get to potty chair,forced to wear diaper when she is NOT incontinent. Warned of allergies to meds, place not listening, Family forced to discuss things with director in room with NO DOOR on it, breach of privacy. Outrageous noise, staff arguing noise complaints by others led to investigation today. Director plays attorney, falsely stating the law, when corrected by attorney refused to listen. Nurses leaning against walls watching TV, while I helped the therapist with the wheelchair myself! Limited food on trays, trays delivered to wrong patients later staff seen munching on bacon NOT put on trays. Way too small, even a dispensation from the govt. on wall saying they can have patients in rooms that are known to be too small.(3 in tiny space) You're loved ones will die before their time here. Avoid at all cost, worst rating in the area."

"My mother-in-law is at Vienna Nursing & Rehabilitation Center. They are amazing. She's clean, she said. They're attentive, and they try to get her involved. She's not in the rehab portion anymore, but they're consistent about trying to get her up. They call me with any concerns. She just had COVID, and she had to go into isolation for ten days, and they called me every day and gave me an update. I can't say anything bad about Vienna. Her linens are always clean. The floor is always clean. Her room is always clean. They have stuff, but she doesn't do anything. She likes to stay in her bed. If you have to be in a place like that, I think they're amazing. The staff is amazing. Even the little gal at the front desk knows my mother-in-law's name. It's almost like a little family. We're just so pleased."

"Fairmont Rehabilitation Hospital was very good. They did a very nice job for my mother. They were very concerned. They checked on her frequently, and they let her order whatever food she wanted. The food that they prepared seemed to be very good. "

"As a patient, I had an excellent six week experience. Food was above average and the two person room adequate. But outstanding was the nursing staff. They were attentive to every need (physical and emotional) and very professional in behavior. And outstading was the rehabilitative program which customized physical and occupational theraphy for each resident."

"Mom was at Windsor Elk Grove middle of January into February. The staff was really incredible, really caring, very dedicated, incredibly friendly, and hardworking. I cannot say enough about the staff. The facility is pretty rudimentary, but it's clean and it smelled fine. The food was adequate. It wasn't super yummy, but it wasn't terrible. But I think the number one thing is, as a child of a parent who's in a stressful situation, it's like you're leaving your kid there, it's important that when you're not there your parents are in good hands. I felt pretty confident that mom was getting good care. I was very impressed with the staff. Mom did physical therapy almost every day I thought they did a really good job. She had a close to broken hip and she was in horrible pain, but she's in good shape now. They really worked with her and got her back on her feet. I was pleased of the quality of the physical care and the skilled nursing. Though they could maybe up the activities a little and have a few more events. "

"Though small in size, upon visiting one is constantly reminded that Bruceville Terrace is part of a much larger hospital campus. Despite staff attempts to create a home for their patients - including daily activities and a teenaged volunteer staff - the smell is overwhelmingly of antiseptic and industrially-cooled air, the rooms dominated not by personal affects but the large, standard-issue hospital bed. This, however, is most likely a result of the unusually high percentage of hospice and end-of-life care at this facility; one thing most certain is that it is not a result of the nursing staff. Nurses here seem capable and caring, and are well-acquainted with both the details of patient's charts, and the idiosyncrasies of their characters. More than most other facilities I've visited, it seemed that the nurses have truly formed caring relationships with their patients, and the population here seems to reflect that - happy, engaged, and energetic to a warming degree. "

"Creekside is very beautiful with a very home-like setting. They have good food and compassionate care. My aunt had been there before, and I was impressed with them, so we used them for the last time my father needed that kind of care. "

"We placed our Dad at the Wagner facilty in May of 2023, and my Dad passed away in Jan of 2024. From the moment my Dad arrived, he was given the most compassionate care. The Nurses, CNAs, Administrators, Front desk staff, Chefs and Custodians all gave so much love and respect to my Dad in passing or when he had a need. We as a family were always greeted with kindness and allowed our spaces. The other residents were always ready to chat with us or my Dad. It was a hard decision to place my Dad in care but the staff showed us that he was taken care of in every way. Even in his last days, our family was accommodated with privacy and comfort in saying goodbye. I have so much respect for the team at Wagner. The facilty felt like a home and the courtyard was my Dad's salvation. Every time my Dad needed anything, he was attended to immediately and given so much patience and kindness. I will forever be greatful for the staff for taking care of my Dad when we couldn't provide the care he deserved."

"I came to Valley Gardens after a PEG tube placement. The facility itself is very clean and well kept. The staff treated me with dignity and respect which is becoming harder and harder to find these days. Once I started my rehabilitation program I very quickly realized that not only will I be able to eat on my own again but I will have a better life style when I return home! I cant show my appreciation any greater then posting this and saying thank you for giving me my life back! "

"The facility was really clean, and they seemed to have gtood activities. My big issue was some of the staff was really mean. One of the nighttime CNAs was cruel to my aunt, a resident at this facility. The CNA called her a nasty lesbian, and would yell at her and berate her on a nightly basis. Other than that CNA, who has since been terminated, the facility is nice. The staff is for the most part kind, especially the daytime staff, who I have had the most contact with. "
