Very decent rooms. The food always looks so good! New facility. The activities are keeping my grandmother super young! My grandmother is being treated with the most respect and care that I finally get to sleep at night. She loves her nurses and every staff there. Thank you!

Rooms are clean. Most of the staff are very nice and there were a few that didn't seem to want to be there. My relative is in great hands and I'm very happy with their needs being met.

Rooms are dirty, They have cleaning staff members, but they expect patients to clean their rooms even if they are in walkers!! Some aides and nurses are rude and have little to no compassion, or empathy for patients. They lack respects, how to talk to sick people, they lack skills on how to care for weak and frail patients. They have very limited exercise or activities. In order to get any kind of therapy you basically have to fall and get hurt before they will do therapy. (Medicare rule). Very little amenities. Patients are left for hours laying in their soiled undergarments. Please, do not put your loved ones in this facility. They make everything sound so wonderful during the initial interview and once your loved one is admitted, it's pretty much downhill from there for their care. It's very very sad. I didn't have a choice when my sibling was placed at this facility. They placed him from another horrible assistant facility and i was basically told this was the only facility with openings. Trying to get call backs from the staff when you leave a voice massage needs a lot to be desired. it could take hours, days, or no call back at all.

This facility is not worthy of sending your loved one. They are massive users of insurance money. My mother was sent there from the hospital and she was to have therapy daily. I called and spoke with the social worker to see about her length of stay base din insurance. I was the length of stay is based on the treatments received. So I asked how many days of therapy she was getting and they didn't know. But said her insurance has ended and now they were using her Medicare insurance. Which can extend her stay for 100 days. My sister went in to visit and had to clean the room, change the sheets and empty the garbage. They are using therapy to extend her stay and bill the insurance. If you have someone in this facility PLEASE go visit on the regular and just show up.

An absolutely HORRIBLE place to send your loved one. The staff sucks, they’re always too busy to attend to any concerns I have regarding my brother. He has scheduled medication and it’s always late. He has to wait hours to be changed, he could walk when he got there 2 months ago, now he can’t. He lies in bed all day, they won’t even sit him up in a chair. I have to find somewhere else to take him. This is ridiculous. The place is FILTHY also.

my brothers room on the third floor is absolutely disgusting he is being given pills but they don't care if he takes them because they are all over the floor he had no bed sheets on his mattress that was just about deflated the place is a night mare

If you are thinking of sending your family member here.. Please don’t. My grandfather has only been a resident for less than a year and he not only fell down over 10 times and had to be sent to the hospital every time, He is now hospitalized with septic shock, influenza, and has severe brain damage or has had a stroke and the facility has either hidden or covered it up or did not report it to the hospital or to us. They intentionally kept us from him the whole time and would not allow him to call or reach out to us or put a phone in his room as we requested. When we heard he was in the hospital and went up there I was blown away by what he looked like. He looked like he just came from a Concentration Camp. He looks starved to death. He can’t talk anymore only can reply ONE word and can barely do that. He stares off into space and can’t get up or down from his bed anymore. He has scars all over is head from the falls that he had there clearly that caused severe damage to his mental sand physical state. I am so devastated that what had initially was supposed to be a few week stay in the Buffalo Centers rehab turned out to be a full time stay in their nursing home and now because of that my grandfather is suffering every day in pain and I don’t know how much time he could have left In the condition he is in. When he arrived there from assisted living he was fine he had dementia that was it and it wasn’t that bad enough that he couldn’t take care of himself or talk to us call us every day. This place is literally neglecting residents to the point of death and I am not the only person with a story like this from this place. Please do your research before you put your family here or any facility I didn’t and now it’s costing me my grandfather.

Make sure you double check your insurance before you even consider this place. Make sure that all your clothes are labeled with your name BEFORE you send them down to be washed. DON'T RELY ON LAUNDRY TO LABEL THEM they lost all my new clothes that was brought up to me. Be very careful of what you eat if you have allergies because I was served mushrooms when the ticket clearly listed my allergy to them. I was forced to lay in pain for hours even though I was due pain medicine. This is not a place I would recommend to my worst enemy. The staff was nice most of the time when they were fully staffed. They were usually short staffed having to have nurses and aides pull double duties and work more than one section. Someone needs to step in really give this place an overhaul from the kitchen all the way to the top.

This facility is horrible! Please avoid it at all costs. My sister spent 2 months in there recovering from a brain injury and the level of care was extremely inadequate. She was admitted the day it was announced that visitors were not allowed in nursing homes due to COVID-19. Therefore, my sister, who had a temporary cognitive impairment at the time, was admitted with no family member to speak for her and no one to help her get settled. We could not get an update on her condition and was told numerous times she was "fine". Once my sister gained consciousness and was able to speak, she described to us the lack of care she was receiving. Unable to walk, she would ask for water or ring for pain medicine, and her request was ignored time and time again. The meals were terrible and there were days when meals were skipped. One day, she told us that no meal was provided at all - no breakfast, lunch or dinner. The staff was uncooperative and lied to her (and to us) over and over again. They argued with my sister over her getting her meds, and on too many occasions she didn't get them. She laid in her bed for 23 hours a day with hardly any human contact. She ended up leaving against medical advice, but wasn't receiving sufficient medical care there any way. In fact, she developed complications that could have been avoided if the staff was cooperative. When she came out of that facility, she looked like she came out of a concentration camp. She went down to 86 lbs but had gained some back before her release. The only positive thing that happened there, was the physical therapy team was very nice to her and helped her get walking again. But I can go on and on with the horrible experience my sister went through at that place. I am writing this review so that no other family has to go through the nightmare my family went through.

Arrived with my father, after discharge from the hospital, for a sub acute rehab stay. It took less than an hour to realize that facilities like these are what keep the nursing home lawyers in business. The gentleman at the "front desk" provided a room number and pointed the way to the rickety elevators, leaving the impression that we disturbed him with our arrival. After getting off the elevators onto a dark and dingy unit full of staff congregating at the nurses station, deeply involved in personal conversations, we were again pointed in the direction of a room where we found a very narrow bed in a very small shared room. My father is a big man and has trouble re-positioning himself in bed so we asked for a larger bed, pointing out that the roommates bed was much bigger. The response was a huff followed by "we'll have to see if anyone from maintenance is still here but I doubt it". The final resolution was that they could put mats on the floor in case he falls out of the bed. We asked for a urinal which was a huge production, apparently they're hard to come by. Wheelchair or walker? Not today. Pain pill? Not today. Pitcher of water? About 20 minutes later. Needless to stay we brought our Dad home where he's at least safe and can have some water. Honestly, I don't understand how places like this are legal.

Cigarette smoke accosts you as you enter. Very drab interior. Many employees sitting/standing around chatting or watching TV while patients are calling for help. Some employees ARE diligent, though. Rehab room is good and therapists are very good. Food choices can be better. Transportation service can be more timely and professional. Social work staff are quite responsive.

Incompetence from the top down. Medication errors covered up, nursing will fix any errors they don't care about the residents. State needs to look at them a little closer. Don't put you loved one here if you care about them. Everybody acts nice to the family but then talks trash about them. I would not even put my dog here. Management needs to go. People need to get fired. Nurses and aides sit around eating all day and on the phone. Worst place ever. Residents are bored nothing to do if you are not able to move on your own your stuck on the drab floors all day. Dining room has not been used in ages. A disgrace. Oh and that is just the day shift, off shifts are even worse.

I could not put into words how terrible this place is. Staff must of been deaf, especially when my family member kept kept requesting help and no staff came to assist her. The administration was invisible, completely indifferent to the needs of the patients. Calls were never returned, and when confronted with a social worker, they were defiant in attitude and demeanor

Do not, under any circumstances, let your loved one stay in this home!! My husband was sent by the V.A. for long term care/rehab and ended up back in the hospital with numerous infections due to the haphazard, inappropriate and complete lack of infection control for his bedsores (which were almost healed when he got there). On more than one occasion they allowed the aides to take off his bandages, then left him laying there with open wounds exposed. One time this happened in the late evening, the nurse was supposed to come back to re-dress his wounds and didn't. My husband knocked over his urinal, then fell asleep while trying to change his own ostomy bag (he could never get any help with that and had to do it himself). So he laid there all night in urine and feces, with his open wounds. I could go on and on about the severe neglect and abuse he endured. In addition, the place is filthy, extremely understaffed, and staff are not trained properly in any procedures that I could see, and I was there at least four times a week. The experience was so horrible for both of us that I won't allow him to be in a nursing home again and am bringing him home to take care of him myself. I have turned this placed in to the Department of Health. It needs to be shut down, immediately!

Worst place ever !! Monitors go off for over a hour before they check them. They could not get a feeding tube running properly. Never saw any hand washing or sanitizing done! Rooms are hot as hell the window screens are too dirty for any air to get in. The man in next bed was stealing all my Father-in-laws belongings even his teeth!! When my Father-in-law got there his RN asked what his feeding tube was and looked at it like she never saw one before where did she go to school?? When we said he was leaving they were very nasty and unhooked his feeding tube and let it run out of his stomach all over the bed!! Then they refused to let us use a wheelchair or help with him leaving. Please do not ever let a loved one go there

Extremely dirty, smells of feces, staff outright ignore residents asking for assistance even in front of visitors. Makes me wonder how bad it is when no one is around. This place should be investigated immediately.

I would not recommend this facility to anyone. They're service is bad and their staff was so unprofessional. Their treatment of their residents were horrible. It takes them a while to get to the room when you need assistance. I actually timed them a couple of times and t took at least 20 minutes for them to show up. And there's already an instruction for them not to leave my grand-dad alone while he's in the toilet and they left him alone couple of times. They really don't show compassion and care.
