I visited this facility

I removed my mom from Clare Bridge because they weren't well staffed and they didn't have nursing care on the weekends. At first, we thought it was a nice facility. It wasn't too big and I thought it would be a good place for my mom. However, as it turned out, she didn't get the care that she needed there. There was a problem with her medication and I felt like they weren't paying attention to her. The sanitation of the place wasn't that great and their rooms were fine but they're small. Also, they didn't have the activities that the other facility has.

I am a friend or relative of a resident

I really like what I saw at Clare Bridge of Farmington but it was not geographically desirable for us. The price was competitive. The people were good, but it's a little bit further out so it didn't quite work for us.

I visited this facility

My mother had alzheimers and was at this facility for 16 months until she passed in Dec. 2011. My mother was given excellent care. They have a wonderful caring and compassionate staff. They not only supported my mom but supported me during a very difficult time. I always knew that she was being well cared for and it put my mind at ease. There were plenty of hugs for my mother and always a hug for me during the difficult times. They had a support group that met once a month that I attended. Being able to talk about this terrible disease with others going through it brought comfort to my heart. I highly recommend this facility.

I am a friend or relative of a resident

This facility has been a God-send for my family since my elderly Aunt with dementia went to live there about a year ago. The staff is kind and caring, Their "movement is medicine" approach keeps the folks active and they have fun activites that draw out the otherwise reclusive residents. Excellent security and beautiful grounds.

What ongoing training does the staff receive in dementia care?

Our associates undergo extensive dementia training prior to employment and receive additional training often - sometimes daily! Brookdale offers numerous resources and support such as a Divisional Dementia Care Specialist. Each member of our management team - ED, HWD, and SM - has trusted the care of one of their loved ones to the team at Brookdale Farmington Hills North at some time.

How many staffers are on duty in the daytime and overnight?

Staffing is based on resident care needs. Currently we have 4 caregivers during day shifts and 2 overnight caregivers.

What programs or activities does Brookdale Farmington Hills North offer to keep residents engaged? How does the staff engage residents' long-term memories?

Our programming is rooted in a person-centered approach that helps sustain feelings of belonging and purpose. Renee, our Clare Bridge Program Manager, has been with us for 15 years. She was a caregiver prior to becoming program manager. Now, she is a trainer and mentor for new program managers. She is especially good at creating purpose for our residents. For example, Renee realized that one of our residents, Sherri, loves to set the tables every day. Sherri's sister says she hasn't been this happy in years!

Who assesses residents' health and cognitive functioning? How often is that assessment repeated?

Our clinical team is second to none. Diane, our Executive Director, has been a nurse for 35 years. Felecia, our Health and Wellness Director, has been a nurse for 20 years! Our geriatric primary care physician visits weekly and is available by telephone daily. We also have a geriatric psychiatrist who visits every other week and is also available by telephone for consultation daily.

What techniques does Brookdale Farmington Hills North use to support memory care residents during mealtimes?

Everything in the community stops at mealtime, care staff and managers are present in the dining room to ensure close attention is given to all residents. Our In touch is used for appropriate, soothing, mealtime music. Brookdale home health speech therapist is able to consult regarding any swallowing difficulties and recommend any assistive devices needed. One course is served at a time, plates are a different color than table cloth for contrast. Glasses are smaller for easier use but refilled often.

How does Brookdale Farmington Hills North keep residents' families informed about their loved ones' health?

Brookdale Farmington Hills North communication is second to none, we always receive high customer satisfaction surveys. Any fall, skin tear, weight loss/gain, means a call from our nurse. Care conferences are conducted quarterly or upon change in condition. Our doors are always open to families. During the last few months, our director, Diane sends out emails to update families on how are community is. Our business office coordinator also conducts monthly family calls.

Our community is located in a residential neighborhood. Most of our apartments have a neighborhood view with families and children playing in their backyards. conveniently located close to Twelve Mile Road and Water P Reuther Freeway, tucked into a quiet, residential area

Is Brookdale Farmington Hills North affiliated with a hospital if more care is needed?

We are not affiliated with a hospital. Our only discharge criteria medically is a need for an IV, ventilator or continuous feeding tube.
