Summit ElderCare, Fallon Health's Program of All-inclusive Care for the Elderly (PACE), offers complete, coordinated and individualized care for adults 55 or older in our Massachusetts service area who want to live at home and in their communities instead of a nursing home or long-term care facility.
The program's team of care professionals works with participants and caregivers to address each individual's specific needs. This Interdisciplinary Care Team is an essential part of how we provide care.
Care at Summit ElderCare may include: Medical care:- On-site medical services, including Primary Care Provider (PCP) visits and vaccinations
- Specialized dementia and Alzheimer's care
- Indoor and outdoor activities for social, physical and cognitive needs
- Transportation to and from our sites and off-site medical appointments, when needed
- Assistance with medication administration
- Help with meal preparation
- Light homemaker services
- Assistance with care planning and decision-making for plan participants
- 24-hour emergency telephone access to care staff
- Support group meetings with Summit ElderCare staff and other caregivers
- Physicians and nurse practitioners who are geriatric specialists
- Staff who have extensive experience working with older adults
- Physical therapists, occupational therapists and registered dieticians
- Behavioral specialists
- Social workers

My grandmother was a participant in this program. She was there for over 4 years. Within those 4 years not once did her doctor recommend her be on dialysis or follow up with my family's concerns. After pulling her out of this program because her health was failing drastically and seeing a specialist she was diagnosed with late stage kidney disease. A month later she passed away. Absolutely no communication, no concern, and no compassion for human life whatsoever. It was a fight anytime we brought up our concerns. They are quick to sign you up but after that the person going to this program is just a number.

CARE? there is no care for my mom in this place. No communication, Unprofessional, UN Knowledgeable staff. I am sitting here tonight, crying because I don't understand how this program can state such misleading information. I don't think my mom saw a doctor twice in the 2 years she has been going. I am so upset and frustrated. I guess my being selfish in wanting help caring for my mom so I could work is paying me back 3 fold. They do NOTHING for her. I've got to get her out and back to a genuine geriatric doctor that will help us straighten out her meds as she takes way more than is necessary. And just help her feel better. She is 82 years old and is good shape for her age. She likes having her own place so I am doing my best to keep her on her own while insuring she is well. it's not easy and I thought for sure Summit was the answer. I dont know about the other facilities but, the one on grafton street in worcester is just aweful.

This organization is awful!!! After dealing with them, I realized what a HUGE mistake it was enrolling mu grandparents. This organization is concerned with money and NOT the well-being of the elderly!! It's an absolute disgrace!!

Trust this place with any family member. Know that they will be safe and taken care in a loving environment.

It seems that this is a $$$ first program. From our recent experience we have come to the conclusion that they are more interested in receiving fees than they are with the actual needs and well being of the family member being enrolled. Highly disappointing!!! Our intentions for our loved one to remain in a safe environment with supervision during our routine work absence and the hopes for stimulation and the opportunity to maintain hygene as well as dignity were clearly not their priority!!!

My family member has been very unhappy since she started receiving care through Summit Elder Care. They are very difficult to work with. Very poor communication. And, although they say their goal is to help participants remain independent, they've eliminated services that were in place before she joined. I do not recommend them.

I also have problems having to use their Drs when we had our own for So Many yrs..Lack of Communication is Very poor and no follow up to almost everything. My friend does like going there, however I think there is a better place and they disagree. Once your in its hard to get out..

Checking into concerns of an elderly relative - rate 0 Waiting time for seeing a doctor - up to 2 hours Forcing to use their doctors under this plan when they have to leave their doctors that they have seen for years
