The rehabilitation area is kept very clean on the first floor. The nursing station most of the time has someone there. The first floor they keep clean. My mother fell out of bed the 2nd day and hit her head on the 1st floor. I had to demand to speak with her to see how she was doing. I wanted to come and check on her and was told I could not. She is not their property and I have 24 hour access to my mom. I went to the facility and called the ambulance myself and demanded they take her to the hospital. They don't call the ambulance, they call their private ambulance service and that takes 30 minutes or more. They do this so the call is not recorded and no public record. Before I could get her transferred out, they took her to the 2nd floor. Don't be fooled the 2nd floor is dirty and too small. You are not given a tour on the 2nd floor. They don't answer the call light. You have to walk the floor looking for help. The Cna's don't want to change her through out the day. I come at 8am only to find her wet from head to toe. They didn't comb her hair, my sister had to do it., they didn't do mouth care , we had to do it. We also constantly had to tell them to change her clothes. What a hot mess this place is. There are a few compassionate Cna's that don't walk pass the door when they hear residents hollering. They are over worked and short staffed. Its not the residents responsibility for staffing but they suffer because of it. The management there is worse than the employees. They turn a blinds eye to what's going on. My mother was given a laxative and I told the Cna to change her. The Cna left my mother and could not be found. I went to her supervisor and she said my mother would not let them change her. My sister had to come to the facility and demand that they clean her. Finally, after 3 hours of a family fight a compassionate Cna took care of her from another floor. This place is a hell hole, I'm not sure why the State of Illinois keeps this place open. The name Elevate Care is a slap in the face, Its a dump. They need to elevate there level of care and get that hell hole in order, I was horrified. I'm even more upset that I trusted them with her care.

My sister is at Elevate Care South Holland. It is okay. It is sort of different to move from independent to dependent care. So far, this facility has met expectations as far as making sure that the residents are taken care of. They have activities for them, and they provide the services that they need. The food looks pretty decent. They're standard meals. It is OK. The staff members have been very nice and friendly. They're OK. For activities, my sister said that they get them up, and they participate, and they do some fun things. She sounds very happy about that.

I visited this facility

My brother will be moving into Elevate Care South Holland. The whole place was clean, and I liked the personality of the staff. I just liked the setup of the facility. There were 2 people in a room, but they had private rooms, too. I saw them doing physical therapy in the physical therapy room downstairs, and they were also doing physical therapy with some of the residents in the bedrooms. They also had activity rooms. The family could come and be with them down there. Everything looked clean and organized. They could have their meals in their rooms or they could come down to the common area and eat. I did see some of the residents sitting outside in the front garden area in wheelchairs.

My mother has contracted an infectious fugus that is not curable and NO ONE has given the family any proper information on how, when, where the residents have been exposed. What is the nature of the fugus. How long they will be quarantined. NOTHING but we are free to research it ourselves.

I visited this facility

I toured Elevate Care South Holland. I liked how clean it was, and everything seemed to be done in a proper fashion. The main thing that I like is that they have dialysis downstairs on the premises. It is also state-of-the-art, and it's very nice. They have equipment for showering and taking a bath. They have equipment that would lift my mother, so that I wouldn't have to worry about her falling. And if my mom has a day where she can't feed herself, they'll feed her. The staff member who toured me was patient, very thorough, and anything that I asked her, she didn't mind showing. No food samples, but I was told they do three meals a day.

My father-in-law is at the Villa at South Holland. It's nice. They've got nice people there. They have single rooms now, but he has a roommate. They have very nice facilities and a very nice dining area. The staff is caring, attentive, and very hands-on when it comes to him. It's really good for him. He's always in the dining area. He likes to hang out there. Every time we look for him, that's where he is. He doesn't need constant care, but he likes it. If you're looking for that, they're really good. They're an alternative. If you have a parent who likes attention, they give him that. He likes to socialize and he likes to talk to the people that work in the place. He's very happy. It's a smaller place, so the cleanliness was good. It didn't have an odor. It smelled fresh and clean. Overall, it was pretty good, and he chose it. He had pretty good taste.

My mom was in a nursing home called Villa at South Holland to have physical and occupational therapy. She was there a month ago and the experience was terrible. Their staff, both the medical and other staff, were very slow in responding. My mother's roommate had to dial 911 to get oxygen because no matter how many times she rang for them, no one ever came. My mother was in excruciating pain and they never came so I sent her to the hospital. They wanted to put her in hospice but the hospital said she was fine. She's thriving and doing well in her new nursing home now. The physical appearance of the facility looked okay. Her room though had 2 beds and it was a tight area. It had 2 TVs competing against one another. There was a curtain that separated them but when there were visitors, they would be right there at the curtain. It wasn't spacious and pretty tight.

ABSOLUTELY TERRIBLE! The only good thing I can say about this place is the physical therapists are wonderful. The CNAs are 1000% disrespectful and condescending. (If you can get through...more often than not they do not answer the phones for hours.) They do not care enough, and do not care to take care of their patients or keep track of their belongings. Do not send your loved ones here.

Staff not properly trained. Nurses making treatment mistakes. Rooms need repair Holes in walls plaster coming off walls My mother developed scars and bruises on her body. Terrible place. I’m seeking help from state licensing agency. Please don’t put your love ones here.

Terrible facility. If you want your love one to be unkemp and disheveled when you visit then send them to The Villa at South Holland. My father was there for respite care. During his short stay he developed a pressure sore and half his clothes were lost with no explanation. Some of the staff has such a terrible attitude. One nurse in particular spoke in a very condescending manner. And good luck with ever receiving a phone call in regards to ANYTHING. I've left multiple messages in regards to a billing issue with negative results. Their medicare.gov rating is subpar for a reason. DO NOT send your loved one to this facility.

My advice is “DO POP UP VISITS?!” My dad is currently residing at The Villa and I KNOW that he received the Care he deserved because my family and I SHOWED UP Everyday Periodically! There are a Few Professional staffers, on the other hand, there are Many Non Professionals! I have watched MANY patients try to get the attention of a nurse/aid and they walked by and TURNED their heads! DO POP UP VISITS?!! Make sure that your loved one is not wearing a SOAKED Diaper? The first week my dad was there, I POPPED UP and he was Soaked! He told me that he had rang the bell and asked for help several times. He wasn’t able to change himself after having a Stroke. Also, if your Loved One knows how to use a cell phone, Provide It? My dad couldn’t figure out how to use the phone in his room. If your Loved One has a Roommate, when you call them BOTH phones rang which is Disturbing to the Roommate! Get a FLIP PHONE if you have to?! More Importantly, if your Loved One is Not Capable of feeding themselves, look out for drastic Weight Loss. Again, there are a Few Professionals working at The Villa but MANYyyyyyyyy UNPROFESSIONALS!! DO POP UP VISITS?!

My mother was so pleased with the pretty birds flitting around in the cases in the main reception areas. However, we were terribly disappointed in the actual care she got when she ended up here. She underwent such unprofessional care and suffering that it was unconscionable. Her overseeing medical doctor told me that she had a psychological complex that prevented her from recovering from her inability to stand or walk after a hospital stay. I saw through this villain’s snow-job and forced the Villa’s administrators to get her admitted back to the hospital, where tests showed that she actually had a tumor inside the vertebrae of her cervical spine! Unfortunately, removing the tumor only caused her to become even more paralyzed. Villa’s staff was supposed to feed her but refused, saying that she needed to be motivated to feed herself in order to recover. Instead, lacking family close by, my poor aged mother starved and suffered immensely, including public humiliation. She was unable to hold the phone by herself, and she couldn't call me easily, but when my mother finally told me about this appalling situation, I was enraged. When I tried to resolve her difficulties at The Villa, I was given the run-around by multiple staff members. As a result, I arranged to have her moved to another nursing home far away where extended family could see to her needs every day. I wouldn't put my worst enemy in The Villa.

My Mother was in this facility in August 2017, where she passed on the 21st. I paid for the entire month's stay (thru the end of the month). Tomorrow is December 1sr. No refund, although EVERY time I call I am told, you shoukd have gotten the check. Checks are cut every Tuesday, i was honest with them. I feel they should do the same, instead of giving me the run around.

If I could give this place a no stars I would They are very understaffed the staff has nasty attitudes even the social worker. My granddad has been here for 4 days he has already started to have bed sores he did not have them before we got here they leave these patients in the chair all day he was in the chair yesterday for 12 hours from 8 to 8. They put him on the same clothes for 2 days someone else's clothes he had clothes in his drawer that they didn't use why I don't know. He's on thickened liquids the nurses giving him regular water she said oh I didn't know he was on taking liquids. When I was calling for a to help put them to bed I couldn't get any help for an hour or so I can put them to bed myself. I wish I had taken heat to these reviews because I will have never sent my grandfather there please I beg you I really I beg you do not send your loved ones here if you really love them don't send them here anywhere but here

My Uncle is currently here. He has been through so much in his life. He has no children so I am his advocate. Villa isn't perfect, but they do correct there misfortunes in a timely matter. It is difficult to have a loved one live in any facility, but I am grateful for the staff here. They keep me informed about my Uncle's care. There has been at least two occasions when I thought I had made a mistake in his placement, but I now know he is in the best place I could've chosen for him.

One word...horrible. I am very disgusted at the level of care my dad has received. The majority of the nurses are rude and complete buttholes..so I can only imagine how they treat my dad and the others when family isnt around. They leave the residents grouped up in a huddle in their wheelchairs, most of them slumped over. My dad requested NOT to have a diaper put on him (he's continent after 2 heart attacks and a stroke) and they put one on him anyway because they don't want to put him on the bedpan. A CNA had the nerve to tell him that no one wants to help him and they purposely ignore his call light. I'm sick of this place and my dad will be leaving it as soon as his medical supplies get to the house. My suggestion? Do not put your loved one in here....run far far away. And this is not the review of a daughter who doesn't get her way for her father. This review is sincere and all facts.

I would just like to mention the hard working Nurses and CNA staff! My loved one was admitted and I will be honest I had my apprehensions about this place okay!! But after I have the staff a CHANCE, they went over and beyond to make sure my mother's stay was all that and some. It's easy to find everything wrong with ANY facility, but even harder to find everything RIGHT!! I give credit where it's due and they have earned MY STAMP IF APPRIVAL????

Over the last several of visiting a family member at the Villa at South Holland, I am no longer going to keep silent about the lack of patient care at this facility because of the fear of retaliation. I leave the Villa at South Holland on a daily basis ‘shaking my head’ in disgust. The lack of care for their patients brings tears to my eyes for this the patients aren’t unable to care for one reason or another. It breaks my heart to see how helpless they are and how uncomfortable some of them are. During my visit, I have observed some things that I feel are unlawful. I am sure after reading them you will feel the same way. Please read some of my observation to get an idea of empathy and sympathy for their patients. I hope someone reading this can ‘HELP ME’ get the right attention/media coverage to stop prevent this from happening at this facility and across others as well. Observations: - Profane stench through-out the facility - Patients are sitting in his/her feces for long periods of time - Littered mattresses that are placed on the floor next to the patients to break their fall rather than having beds with bedrails to prevent falls - Patients have bed sores/wounds that aren’t being attended to causing very foul odor and discomfort - Patients being left unattended and sitting on the toilet for long periods of time without supervision - Patients aren’t being bathe on a regular basis - LACK of communication between the RN’s, CNA’s, and other staff when it comes to patient - A family member witness the fall of one of the patients - Staff not following the proper diet guidelines when it come to the patient care - The overall rating I would give this - Staff under paid for the services care provided - LACK of quality care from certain staff members - VERY ‘HIGH’ turnover rate with staff - VERY short staffed resulting in lack of care for each patient - Broken equipment that is being used such as the “HELP BUTTON” which limits notification to the nursing station when help is needed by patients - The temperament of some of the staff is just OKAY…BUT can be improved by implement, better processes, procedures, and employee code of conduct In my humble opinion, there should be a full investigation of this facility and staff due to the lack of quality care for patients and all of the preliminary observations mentioned above. And for other reasons that are listed below: Assessing the accuracy of the internal controls or business systems and operations. Testing the internal controls to ensure proper operation. Lack of communication finical disclosures between the social worker and the hospital staff An audit should be conducted of the staffing credentials to make him or she has the proper Note: There is video footage and recording of some of the certain incidents to support the above.

Do not let the pretty first floor fool you! The stench of urine hit us as soon as we stepped into the elevator to go up to the second floor only to realize the smell was worse when the doors opened. The residents were just bunched in the middle of the room, and the hallways were littered with mattresses, carts and all types of other equipment, but we couldnt find the basics like gloves in the patients room. I am truly disappointed in this facility. I have had family members stay here years ago and it never looked or smelled like this! What a shame.

My mother was a resident here for almost 2 years. Less than 6 months after she became a resident, the facility changed hands and became "The Villa". It has gone downhill in many ways since then. It's understaffed, especially on the weekends. We found the aides on the 3rd floor chatting away at the desk while residents sat clumped together asking for water that never came. They ignored the overwhelming smell of poop while my mom sat in her own feces asking for help. Mom suffered from at least 4 UTI's in the time she was there and each one helped to deteriorate her brain a little further. They said that mom "slipped" out of her wheelchair but assured is she was fine, only to discover 2 days later (after she kept crying out in pain several times) that she had broken her hip. The last and final act of negligence and stupidity came on the day she died when a very ignorant aide tried to feed my unconscious mother pudding. They didn't tell me that when they called, but instead said that they needed to suction her throat due to mucous build up. We found out about the pudding because someone let it slip and my brother saw pudding coating her mouth...she wasn't even able to swallow and was on morphine at the time this ignoramus thought it was okay to try to feed her. The only reason I'm giving 2 stars is because of a precious few who are there for the right reason and who actually care.

This facility was not the best out there, because I have seen better. There were some truly nice staff members that I can relate to, and talk with about my grandmother's needs. They did make sure that she was well taken care of. When it came to the dietary department, they did not pay too much attention to the food that my grandmother was not allowed to have. She was given the wrong food on numerous occasions which resulted in complaints. The front desk staff was very nice and respectful. They made sure that my grandmother was happy and comfortable, especially when I came to visit. I do think that there is a little bit of room for improvement, which is why I gave the facility only 3 stars. Overall, my grandmother survived the short stay until she was brought back home with us. I guess you really can't be 100% satisfied with every facility.

Excellent Facility For Nursing Care

My 92 year old mother has been a resident of this facility for the past three and a half years. I have been very satisfied with the care that Mom has received - the staff has been extremely caring and compassionate, attending to Mom's every need. I have always been notified whenever there was a problem and any questions I have had have always been answered. I could not have made a better choice of a nursing facility. Mom will be leaving the facility this coming Monday to live at home where my husband and I will care for her, as finances do not allow us to have her stay there any longer. But I would seriously recommend this facility to anyone looking for the best quality care for their loved one.
