I am/was a resident of this facility

This is the worst nursing home I been in! Smells of pee and fecal matter when you come in, halls are dirty need painting. Rooms need painting, new blinds and new furniture. Aides run the floor they tell you when to go to the bathroom now or I will not be back to change you until later! Thay leave people in bed , no showers are given to people. Food is the worst people here have ever seen. Some of it is not even able to be eaten. You ask to speak to someone in charge they usually never show up. They have no actives for the residents, no books to read, they can not go out and walk in front of the building those that are able to go out. Do not bring your love one here.

DO NOT PUT YOUR LOVED ONE HERE. After my mother was diagnosed Asymptomatic Covid Positive, she was isolated and ignored. We had to call the police 4 times over 5 days just to get her care. They left her lying in urine and feces for over 4 hours at a time! We are currently working with a detective regarding our abuse/neglect accusations. I believe the corporation does not care about the residents, as I jave left over 10 messages for help with my mother's care, and all have been ignored! Only the Social Worker seems to care and is now assisting us to get the care she deserves.

I am a friend or relative of a current/past resident

My uncle was admitted here and was then neglected by staff and specialists. His care here was so terrible that the state opened an investigation on this center and he was immediately shuttled to a hospital for emergency surgery. As family members, we intervened as often as possible, but oftentimes, got the run around, blame shifting from one nurse/specialist to the other, or excuses for why my uncle wasn't turned over in his bed (which caused severe bed sores) or cleaned properly. The staff never seemed to know what nursing shifts accomplished before they started their shift, his clothes and mail went missing and the center often stunk of feces or urine. This center should be shut down. Please take your friend or loved one to a care center that will actually protect and care for your friend or family member. We are not the first victims of their ineptitude and probably not the last. Incompetence, uncleanliness, lack of professionalism and neglect are how I summarize the experience we had at CLHC.

I do not recommend this facility. My family member was there for about two months and his strength and overall health declined. While there were some caring staff members in the therapy team and nursing staff, the CNAs are the worst we have seen in any facility (and my family member has been in a number of them). Ignoring call lights, leaving full commodes, dirty bedding, unwashed patients and cold food are some of the things you can expect from the staff at Colonial Lakes. In addition, the facility itself is often dirty, smells terrible and does not have any in-door space for patients and their guests. Choose a different place for your loved ones

My mom and I had a wonderful experience at Colonial Lakes Health Care. You must read this! I love this facility and recommend it. This nursing home and rehab cares about its patients and their livelihood. It is a unique facility. My mom was admitted for three months of Rehab. I visited her each day and each night until she was released. I must admit, I read the reviews and I needed to see for myself and in my way protect my mom. As I walked in the very first day, I felt such a peace when the woman with a welcoming beautiful smile greeted me. I was entering in and she was heading out. She noticed me, stopped in her tracks and asked who I was visiting so she can direct me to the right area. I explained it’s my first day and I am going to see my mom. She politely pointed me in the right direction and introduced herself. The Director of Nursing is so humble!!! She is professional, caring and polite. She represents this facility very well. The First impression is the lasting impression! As the days continued, each day and night, I observed the hard work of the Nurses and CNA ‘s and the grace they have for the patients. I always tell my students “WE ARE ALL PART OF THE BODY SO STAY IN YOUR LANE”. When you get in the wrong lane you cause an accident! And someone gets hurt! What I mean by this is that these nurses and CNAs have a grace to do what they do daily. They are not trying to do anything other than what they are graced in and they do it well. (like the feet cannot work like the hand) No way I could do what these nurses and especially the CNAs do! So, I stay in my lane. I have come in all hours of the night and saw the hard work of these professionals and I have such an appreciation for their skill, knowledge and dedication. They tell the patient what medicine they are giving to them and why they are giving it before they give it. I love it because they are showing transparency to the patient. These professionals are also consistent and organized in what they do. I have learned so much from the Nurses and CNAs over the last three months. They are serious about what they do and have the passion for it as well. They kept me informed about everything about mom. I received phone calls during the day about anything concerning my mom. I can call the nurses station and they answer the phone. MY mom phone kept going to voicemail. I called the nurses station and asked to put mom phone on the charger, they put her phone on the charger! They care! They are always available to answer any question. I have laughed with the Nurses, CNAs and Rehab staff when mom was being her funny and joking self. Also, when I visited my mom, I became close to the residences that was in the hallway as I entered the facility. I must say, I like the way the residences can be mobile instead of being cramped up in their rooms all day staring at the walls. They are out and about either walking with their canes, or wheelchair, or just walking. They seem happy and content. I believe the bed will take your strength. So, as I observed these residences traveling safely throughout the building made me feel good. I said wow, whoever thought of that strategy saved a lot of patients from bedsores and sanity, etc. they can be mobile and free. Colonial Lakes Healthcare and rehab was my new family for three months. I have heard the residences curse and fuss at each other etc. just like family! There are good days and bad days for them and for each of us, but we overcome the bad ones. The Rehab staff is awesome!!! They are very positive. You can heal quicker in love and positive energy than negative energy. The rehab area is highly positive. The facility staff and residences were a new part of my life. The social worker and case manager took good care of mom making sure equipment was there to help in her recovery while at the facility. The social worker made sure mom was ready and equipped with everything needed to transition back to my home where she resides. It was very hard to say good-bye to all. We miss our family but are very grateful for the love they showed while we were there. Kudos to Colonial Lakes Health Care and Rehab!!! Keep up the good work!

I am a friend or relative of a current/past resident

Horrible and disgraceful. My father has lost 24 pounds being there. The CNA are a home. They stand around wasting time while patients are in need of help and care. My fathers clothes smell of urine, he has fallen many times, his clothes have disappeared, they dress him in whatever they find including women stretch pants. Roaches in the rooms halls smell like a letrine. This place must close down.

I visited this facility

My brother is there with no phone in his room and no one ever will answer at the Nursing Station. Being out of State I've had no communication with a Case or Social Worker nor a Doctor of his current Heath & Physical status. Messages left with no call back. Night Switchboard could care less about getting me through to a person on floor. Why isn't anyone at the Nurse Station mainly evenings??? Only been able to talk w/ him twice very briefly. Not HAPPY.

Horrible facility. I have 1 positive thing to say the rehab personnel are very good at what they do.the food is not edible. You have no chice in your meals. Everything is always cold including the coffee. The staff argue amongst themselves all the time. There is never enough linens. To properly clean you. They had to use pillow cases. No one ever communicates with you.they say they will come and talk to you and never do. Residence pass away and family members are told they check on there loved ones, but they do not. They are cold and they still provide CPR to cover there butts. They have no camera's like all the other facility's so staff knows they can get away with doing nothing.they need a new director,camera's and a food vendor that knows how to cook healthy hot meals.meetings with staff. That their job is to take care of the residence and not stand around and complain.

I am a friend or relative of resident

This is the worst facility they have no staff. They don't care of the elders. I left my dad for two weeks and I took him out he got hurt no one knew what happen no records they never sent him to the hospital he had a big bruise on his head no one new what happen. The director told me he was going to get back to me never did it's the worst place you can be. A granddaughter found her grandmother stiff in bed no one new she had pass. They don't check them as they should it's the death house.

Colonial Lakes Health Care was not clean. It smelled. Every time we went in there, we would always smell urine and feces. It would knock you down when you walked in the door. The staff was kind of helpful, but sometimes, I just couldn't get a straight answer. We found out that they were keeping my mom and dad sedated. Whoever was complaining of the wheelchair, the would put them in this glass room and turn the air-condition really low which was freezing cold so that people were not moving and then they would put some blankets on them. My mom had actually been there and she said she actually saw the nurses hitting some people and I had some relative from out of town who came and visited and said that it was old slimy nursing home. It was awful. They were understaffed.
