An occupational therapist performs two home visits to complete a home safety assessment, collectss information about the client’s health history and a list of medications for pharmacist review, and provide a balance and mobility assessment and a half-hour exercise regimen.

Heart of the Valley Services for Seniors

This volunteer-run nonprofit organization helps seniors 65 years and older in West Santa Clara County to remain independent in their own homes.
