I am a friend or relative of a current/past resident

One night was one too many My mom was recovering from a bacterial UTI and was too weak to return home. In addition, she contracted covid almost 6 weeks prior and was still testing positive. Because of that, her options were very limited for nursing homes who would accept her. She arrived at the Pavilion at Creekwood on a Friday at 4 PM. Her room had not been stocked - no blankets, towels, hand towels, washcloths or hand soap. The bed wasn’t made. The gentleman who transported her from the hospital put the only sheet on the bed. She didn’t receive towels and a blanket until 8:30 PM. There was zero orientation. And the “food” certainly can’t make anyone feel better. The room was disgusting and hadn’t been cleaned. The bedside A/C was caked in filth, blowing directly on her. There was even a shopping bag left behind with trash and personal effects, proof the room wasn’t prepared. At my first visit, the receptionist reprimanded me for not following procedures instead of offering guidance. Her only help was a dismissive wave of the hand when I asked for directions. While I moved items into her room, not one staff member acknowledged me. Most were on their personal phones or computers. Even the halls were disturbing. Residents were left to wander aimlessly with no stimulation or dignity. That still haunts me a year later. Her admission paperwork allowed her to opt-in to manage her own medications. However later, a nurse actually yelled at her stating otherwise. When I arrived Sat afternoon, her only trashcan was overflowing. As I removed the bag, I noticed there was feces inside the small bathroom trashcan. Obviously this is not an ideal situation if you’re recovering from a bacterial infection! If that’s what was visible, I can only imagine what wasn’t. This was the final straw. We decided to leave. I made multiple trips moving her belongings. Less than 24 hours later and not a single staff member noticed. The real kicker is the final pass. I escorted my Mom on her walker, straight out the front door. No one even noticed! When I called from home to say we checked out, the nurse said they had been looking everywhere for her. If that’s the case, why didn’t either of us receive a phone call? The staff was rude, unhelpful and indifferent. This place is hell on Earth. I cannot fathom living out my years in these conditions. If you have any shred of dignity for your loved ones or even your enemies, don’t send them here.

I am a friend or relative of a current/past resident

My husband stayed at The Pavilion At Creekwood for rehab. It was a very nice, beautiful ground. The room is average. It's a room with a bed. The people who worked with him daily, the nurses, the therapy people, and all were great. Communication from the business was not always fantastic, but the rest of it was good. He didn't like the food. I've never seen anybody anywhere in those places that did.

My 96yearoldmother is in a private room and it is very spacious and she is able to move around in a wheelchair and bathroom is huge and also wheelchair friendly. The staff has been so caring for my mom.( I feel they are doing over and above for being short staffed) The morning nurse- Hilda and evening nurse Carolyn Farmer and the others( medication nurses,CNA,housekeepers and dietary workers) have been pleasant.Mom has enjoyed the activities- Bingo, music, and art.We also used the salon for her hair( nice lady Donna). The food was ok alittle under seasoning and was cut in bites for her.The P.T. was excellent. The social worker,Sydney and Tamara was so sweet and caring. This is my mom and my experience.I realize everyone has their own opinions- ours was very positive .

The worst care ever. I can’t begin to tell you all the horrible situations including falling, baths supposed to be 3 times a week. I don’t think she ever had 3 I a week. The last two weeks before we moved her was 1 bath in a week and a half. They had to change doctors in order to get medication provided for a U T I. Staff was rude inconvenienced if she asked for anything. Food was cheap and horrible. My mom stated that she thought she might die soon if she was left there. We love her new place with lots of care and compassion. Do not let the pretty facility fool you!

My husband was in rehab at The Pavilion at Creekwood. Their physical therapy department was excellent. The nurses or aides were very good. I was very pleased. The facility was beautiful. It was fairly new, and it was very attractive. The food was not that great. They should have better food and better selections. They served the food in Styrofoam, and that didn't keep it warm as it should have. The facility was very modern, very attractively decorated, and very clean. There was no nursing home smell or anything. It was a beautiful facility.

The Pavilion At Creekwood is fantastic because of the nursing attention, the nurses are just all over the patients in there. Any time there's any question or any problem, within just seconds the nurses are in there, or a nurse assistant is in there to assist. So they have been superb and the quality as far as covid is concerned, everyone has to put on a gown and mask, and they're limited to some people that can come in right now. You can only come in like so many people a day in the entire building, but you have to schedule an exact appointment to come in and see your relative. You're always welcomed and you're able to modify those hours, and they are fantastic. My mother-in-law thinks their food is fantastic, other than that, the pavilion has been a blessing, a true blessing. The facility is super clean, super sharp, super modern. It's not that old of a facility. I think it's only like five years old, and it is excellent. I mean, you look at the surroundings when you pull into the pavilion and the grass is all mowed and trimmed, the trees are all nice, and the bushes are trimmed, and it just looks manicured.
