Do not leave the facility for discharge until it has been confirmed that your home health and other materials have been set up. The day of my mom's discharge my mom was left waiting and confused about when she could leave. We asked days before her discharge date what time would her discharge be and the response was no one had any idea. The day of discharge the response was she can leave whenever she wants. The case worker had told us she would set everything up for her and before my mom left the facility she said everything was done. My mom left the facility to go home. As she was leaving with her brother who had picked her up, I talked to the home health company who said they had contacted Landmark the day before about not receiving all of the required documents. Therefore my mom was on her way home with no home health and no oxygen she needed. Afterwards I called Landmark to ask to speak to the case worker at 11am. They tell me she was in a meeting. I demanded I talk to her. A few minutes later she answers and says she thought she sent everything over and that something must have been wrong with the home health's fax machine or whatever. Some nurses were more helpful than others. When I asked a nurse about meds my mom was supposed to have after my mom had left, her response was my mom was no longer a resident at the facility. I spoke with the director of nursing and voiced my concerns. She told me she would call me back, but did not. It did seem hard to get through on the phone with someone. I had to leave my name and number every time I called to speak with someone. Doctors and doctor's offices complained to me it was hard to get anyone on the phone when I spoke to them. My mom said the food was not good, either too salty or too sweet.

Our experience with Landmark Nursing & Rehabilitation Center of West Monroe started out good, and wound up terrible. The nurses down there would not take care of my mother. They left her down there, and wouldn't check on her. She went into a COVID unit and stayed there for two weeks. When she came out, all of her clothes, her teeth, everything she owned was gone. They couldn't find it. When I complained enough, they came back with a set of teeth that wasn't even hers, it didn't belong to her. They left her in the room without any teeth, so there was no cover, no nothing. We couldn't go see her because of the COVID thing. When I asked them they said, "Well, well, we don't know." They found part of the clothes, not all of them, and never found her teeth. They just left her stuck with a set of teeth that didn't belong to her. Dirty, nasty, thieving. You go back and put new clothes in, they'd be gone, never to be found again. There were only two good ones on the staff. The rest of them were lazy, no good, walked right by, and left water on the floor in a room for her or anybody to just slip on. She fell out of the bed one day, it took two hours for anybody to come and get her up. The rooms were small and non-private. I wouldn't recommend them to anybody.

My mom stayed at LandMark and she was well taking care of and from Administration, Nurses, Aids and staff the people were so caring and treated my mom like family just as well as those visiting her. I will recommend this facility to anyone that has an issue with an elderly family member.

My mother was at Landmark Nursing & Rehab Center, and she had a good experience there. It was clean, and it didn't smell. They had a lot of activities for the residents every day. They had a big dining room, where they served the individuals, and the food was good. They had a beauty shop, where they had their hair done, bingo daily, and church service on Sundays. Almost every evening they had some church or some musical group doing entertainment, playing music, singing, or doing bible study and a lot of religious activities.
