My Uncle lives here and it is great! the food, the activities and the wonderful staff. they are always doing fun events with the staff and the patients

I am/was a resident of this facility

I stayed at Autumn Care of Portsmouth. It's a rehab that's also part of a community with people who live there. The people who worked with me were great. My experience with them was all right; it was good. Their food was very good. I just saw my room and their rehab gym, and everything was very clean and nice. My room wasn't that great. It just had a TV and a bed, but that's all I needed because I wasn't very mobile. It's like an army barracks or something, but they kept it clean.

I visited this facility

Autumn Care has a wonderful rehab center. My mother was a resident for seven months before she passed away. The rehab center staff is tops. The facility is clean and some staff are the best and others need to find another job. My worst experience was with the lady who handles billing. She failed to return phone calls, answer emails. She even blocked my emails. I was away for three months and asked her to email me my Mom's bill since my mail was being forwarded and I would get the bill late. She didn't respond several times to this request and then blocked my calls. I had to send a relative who was looking in on Mom to go to her office and ask her to unblock me. Everything I am telling you can be verified through my kept emails.

My mother was at Autumn Care of Portsmouth. They took care of her wonderfully. The nursing staff, the therapy people, and everybody else was concerned. They're still helping me because every once in a while, I have a question. She had been there for a month. They're under new management and is one of three facilities that we found within 50 miles that were rated a 5-star by Medicare; their rating is well deserved. It's kind of depressing, and there were people there in really bad shape sitting in wheelchairs, but the nursing staff was great and took care of them. It looked to me like she was fed well. I saw some of the food that they brought to her, and it looked good to me. She was not able to go down to the dining room, but every once in a while they had a seafood buffet. It's institutional food, but they did the best they could. Her room was like a hospital room.

My father is a patient at Portsmouth Autumn Care.. I am trying to have him transferred elsewhere. My opinion, they are very unprofessional. The place is old and not clean. I have experienced seeing large cock roaches, and flies quite often.. The hallways are always crowded with other patients walking or in a wheelchair. The staff is not that friendly. The hallways always smell like urine and doo doo.. The laundry department is always losing clothes that belong to the patients. The aid once washed my father's cell phone and the facility would not reimburse me for it nor his lost clothing. The patients are constantly getting moved around to different rooms with less than a days notice. I was asked by the social worker if I wanted to move my father to a private room and my father and I turned it down. They claimed in so many words that he was stinking up the place when he had a bowel movement and that he needed to be moved further away from the main entrance. Within less than two weeks later another private room became available. My father and I was once asked again if he was interested in this private room. So, we decided to take it. Before we took the room, I asked the social worker if he would be ever moved again.. She stated no. I also asked her if he or his family would have to pay any extra for the room. She stated no. I asked her is she was certain that Medicaid would cover everything like before when he had his semi private room. She said yes and not to worry about anything. I said ok lets go for it. He has been there for almost 3 months now. He and my family really like the room. The administrator and the social worker are no longer there. My dad had taken a fall and hurt himself badly. After a long period of time of being on the floor without being seen he managed to get up himslef. He mentioned it to the nurse early that next morning. he was in a lot of pain. he had chest pains and was sore for days. It hurt him to reach for anything or even cough. I complained to the state ombudsman. When I did that everything changed. I received a call from the facility letting me know that my dad will be taken out of the private room or I would have to pay and extra $400.00 a month. I am fighting to keep him in the room. I feel like I am fighting a losing battle. They lied to me and when I tried to talk to them about it all I got was a too bad for you attitude. I feel that this was done out of spite. I am still fighting this. My dad has been grouchy and depressed since they are in the process of moving him. He wants to move to another company. I told him that I am working on it. He came from a semi private room before he was asked to move to a full private room. They are now trying to move him to a semi private room with two other people. After I talked to them he now may move to a semi private room with one other person. I was able to see the room he will be moving to. I do not like it at all. It does not have a restroom like his 2 other rooms. The sink and the floors are badly stained. it is smaller and will be harder for him to maneuver his wheelchair around. he is very incontinent and moves slowly in the wheelchair. The bathroom is across the hall which means he would have to go across the hall to use the restroom. Sometimes he leaves a trail of mess on the floor before he evens gets to the restroom. This is definitely not going to be good! I called the compliance line to file a complaint. I hope they are able to help me. This is totally unfair and sad for my dad. I can not believe I am going through this ordeal. I feel like no one cares. I still have other phone numbers to call and other people to speak with. They have lied to me about a lot of things since has been there but this is was the last straw. I am not going to give up without a fight

I visited this facility

I went to Autumn Care. It seemed to be professionally run. The staff was very accommodating. I had a very good tour of the entire facility including the kitchen, as well as the physical therapy area and the dining area. I saw some rooms. It was clean.

I am a friend or relative of a resident

My father was at Autumn Care for two months. We were happy with the care he received. We didn't have any problems. The staff was friendly and helpful. Although at times they seemed understaffed. The food was pretty good. The room he was in was semi-private and adequate. The physical therapy he received was excellent. The office staff seemed very organized. I live out of town, but the communication with the family was excellent on how he's doing, constant updates, and they even did conference calls with me at work for assessments for his care every few weeks. They'd have meeting with the physical therapist, doctor, the nurse, nutritionist, social worker, and I could ask whatever questions I needed. They all gave me full report every few weeks on his progress. That was wonderful. My father said the food was really good and they had a good variety. If he didn't want to sit in his room, he could go down. There were certain days of the week that they had lunch or breakfast buffet. They did have family buffet dinners where they send invitations to us. They did movies, bingo, a lot of music, Bible study, board games, card games, and it seem like there was always a group in there singing.

I am a friend or relative of a resident

My husband was transported to Autumn Care from the hospital for rehabilitation due to shunt surgery. Upon his admission, nobody informed me about anything: dining room, laundry, etc. His stay was for 20 days. He was given physical therapy each morning then taken back to his room where he would spend the rest of the time until the next morning unless I would help him into a wheel chair and push him around the halls which were very crowded with resident in wheel chairs, linen carts, RX carts, trash carts. I found out later by word of mouth that he could eat in the dining hall. He was admitted on May 7 and finally on May 22 I was able to get the admissions counselor to go over papers which I had to sign. While there he fell hitting his head and I wasn't notified until the next day. It was when he seemed warm and weak that I told the nurse I thought he was ill. She was extremely rude to me! But after my insistence she finally checked his O2 and it was 80. He also had a fever. She called an ambulance and another employee informed me that he was going to the E.R. in 10 minutes. He was admitted with pneumonia for 5 days. I never heard from the house doctor who supposedly examined him. I believe his stay there only set him back. I will say this though, there are many caring workers there. I think the owners should invest some money to bring the place up to date. There are 4 A .C. homes in this area...check on inspections...all the other 4 got 5 stars, Portsmouth only got one. I would research online before I admitted anyone to a nursing home.
