My mother stayed at Life Care Center of Salt Lake City. They were fantastic. She stayed there because she had fallen and broken some ribs. It's a wonderful place for your parent to be when they're recovering. Their staff was so patient, kind, knowledgeable, and gentle. My mother has osteoporosis, so she's been there a few times with very painful situations, and they're so gentle and patient with her. My mother is a very high-maintenance patient. She loves the food. It's clean and it's just excellent care. The food was delicious. I took a few bites as well and it was very yummy. The layout was very nice. It was a little bit more like a hospital facility, but there were beautiful grounds for them to walk. It's not quite as homelike, but it is a rehab center.

I chose The Life Care Center for grandpas rehab stay, following a total knee replacement. Within 5 minutes of walking in to the facility, I knew it was going to be difficult. Someone did help us get him to his room, but once in the room, they left with zero communication in the next steps, even though they are the ones that told us what time he needed to be there for his admission to the facility. We were left sitting in his room, waiting for someone to come talk to us for at least 30 minutes. I ended up pushing the call light and going up the nurses station to find someone after the call light being on for at least 15 minutes. The CNA that came in to do his check in vitals and weight was less than nice. She hardly spoke, was very rude and abrupt and when I told her than my grandpas briefs needed to be changed she argued with me in front of him about the fact that because she couldn’t smell it, he must not need a change. I insisted he did, so she got another CNA to help change him, and both of them complained the entire time, saying it was pointless because he wasn’t dirty, when in fact he was, and they both ended up rudely apologizing. One of the nurses in particular was lazy and had zero desire to help him. After multiple phone calls because he wasn’t getting his pain meds in time, and literal days trying to get them to give him his pain meds on time, did they finally do something. When he was ready to be discharged back to home, they gave attempted to give me less than 24 hours notice to get things arranged for his discharge, they didn’t work with his case worker on getting any of tue supplies like a elevated toilet seat, wheelchair, disposable briefs, etc. set up before discharging him, so he ended up being discharge home without any of the daily living equipment he needed. They no-showed more than care plan meeting. They also called me multiple times to get my arrival time on when I would be coming to pick him up so they could discharge him, and when I arrived at the facility the day of his discharge, at the time I said I would be there, they had absolutely nothing done. I had already set up his home health and physical therapy for when he got discharged, and the a representative from the company we worked with physically walked in to the facility to let them know they would be the ones doing the home health and physically therapy and the facility STILL sent the home health and PT orders to a different company and gave me zero communication on any of his discharge orders. I absolutely would never go there again. The majority of the staff, with the exception of a couple people were beyond rude, lazy, and the complete lack of communication and organization is mind boggling.

My father was here recently before his death at Life Care of SLC. I informed the administration of my three estranged siblings' pending arrival to say good-bye, that they were NOT vaccinated and very demanding. The facility had a positive COVID test the day before, so people traveling constantly (two arriving from Mexico) would seem like a concern. The facility not only allowed my three siblings and two nieces to visit unlimited, they allowed all five to move into his room and also provided mattresses and food! I was stunned that the Life Care administration was bullied by these people, which left the rest of us in town unable to visit or even say our final good-byes. When I went to retrieve his belongings, the admissions director Sharon, complained to me how they were rude to staff, very demanding, and denied my father his requested comfort medication. However, for some reason they weren't told to leave or police called for trespass. Life Care staff appear to have no backbone and cater to the bulldozers instead of the rest of the family.

I was a volunteer here for a while. The staff did know everyone's name, but I felt like they were ignoring everyone. There were wheelchairs lined up all over the hallways. If I mentioned someone needed something, the response was, oh he always wants attention. I was not impressed by this at all.

My father was at Garden Terrace for 2 weeks. He enjoyed the food and thought that the physical therapy and occupational therapy was good. Our only complaint was the administration. NOTHING seemed organized- and when we had a meeting with the head of nursing, the social worker, office manager and PT head and made a plan, nothing ended up being what was planned. I will say that the social worker did return calls in a timely manner but both my dad and I felt like everything was running "by the seat of their pants". Walker was not available (even though we ordered over a week in advance) for when he left. Also even though they arranged showers a few times a week, he stayed in the same clothes for several days. I had to constantly ask them to help him change clothes (he had tubes etc so I couldn't do it myself). The place is very clean however and the staff is friendly (nurses and nurses aids) so not everything is bad. I just felt like the communication could have been better.

The staff was superb! I visited Garden Terrace about six times, and I’ve never entered once without being welcomed by everyone I passed. The receptionist, nurse, physical therapist, or anyone else was just most friendly. They’re always very happy there. It was excellent!

Our experience with Garden Terrace was horrible. The staff was extremely unorganized when arranging his transfer from the hospital. Half of the nurses that helped us were very gentle and the other half were not gentle at all. Many of the nurses were very loud. Two days after my father passed away, Garden Terrace called my grieving mother saying that they would like to discuss plans with her to take my father home! That was very inappropriate. They were very disorganized. The whole experience was a nightmare.

My father was admitted to Garden Terrace after being released from the hospital for rehab. He fell while in the hospital and got a brain bleed, which is why Garden Terrace was chosen for him so they could take care of the brain injury and severe confusion he was experiencing. The facility was very nice and clean with big rooms. But the staff never seemed to be around, was incredibly rude and seemed to just ignore him and his needs. There was one time that his alarm was going off for 23 minutes before someone came in the room to check on him! He did eat the entire time he was there and this was never a concern to them. He actually ended up passing away after a week in their facility. We believe he had a blood clot that killed him but we sure wish he had passed away somewhere that cared about him and his well being. He hated every second he was there. He also had his cell phone and watch stolen within a day of being there!

Garden Terrace is a nice residence. They don't have long term facilities because it is Medicare but their place is clean, sunny and bright looking. The only reason I placed him there was because I was told that they take Medicaid patients. The rooms were large but the staff was not very knowledgeable. They had times where they served ice cream and they had sing-alongs. They take their patients to the doctor appointments and bring them back.

Garden Terrace of Salt Lake City was a very good-looking place. However, it was not a locked-down facility and my mom was confused after the surgery that she needed to be in a locked-down one. I liked the lady at the front desk because she knew everybody by name and greeted everyone in the hall as we passed. She was very personal with them and everybody seemed to get along well. The place was nicely furnished and set-up, very clean and pleasant-looking.

My father spent time at Garden Terrace for about the last 2 weeks of his life. He was afflicted with Pancreatic Cancer and only lived about 5 weeks after his initial diagnosis. So the stay at Garden Terrace was a Hospice situation. My Mother an I toured about 5 facilities in our area before deciding to place him at Garden Terrace. First impression was the cleanliness of the facility. It was extremely clean and bright. The staff was very attentive and friendly. The facility specialized in Alzheimer's patients but they made an exception to take my father into that unit, which was a huge relief to us. From the very start they treated my father with dignity and respect. Whenever I visited I was made to feel like I was important and I could feel the compassion from those who attended my father. He was in a room with another fellow who had advanced alzheimer's. I was also impressed with how the staff handled his needs. He was unable to control his bowels but they just took it in stride, talked kindly to him and cleaned him up more than once each day. My father was soon put in a bed on the floor so that he couldn't fall out of bed. I know he felt a little helpless at this point but all went well. The night that he passed I waited and waited thinking the time was near. I finally went home and we got a call around 4 am that he was gone. I wished that I had stayed but I think he didn't want anyone there. Again the staff was very kind and loving to my family and let us stay in the room as long as needed before they called the mortuary. I would highly recommend this facility to anyone needing this type of service. I even was able to spend time with my father in their cafeteria and was always amazed at the quality of food and again the cleanliness. I hope I never actually have to deal with an Alzheimer patient but if I do I won't be as afraid of it as I used to be. The patients here were treated and lived as well as any elderly person could be. I have nothing bad to say about this provider and I'm sure my family members would say the same things.

Review of Garden Terrace Alzheimers Center of Excellence when I made visits: Ratio of staff to clients at lunch time was two to approximately 10 clients. Other clients appeared to be unable to feed themselves; staff was unable to give adequate attention to make sure each client was well-fed. Clients got up and left room without finishing meal. I saw nothing in writing regarding accountability of what/how much was consumed by my client. Clients were often rambling from room to room without supervision or activities to further treatment goals. My client was often in same chair when I arrived and alone. I observed little interaction between staff and client. My client was clean at each visit, but family reported personal belongings were constantly missing. Room was shared with curtain dividing room. It was tidy with personal belongings on wall. Personal accomplishments were respected. Staff was pleasant at all times when they interacted with my client, they appeared to understand her moods, etc. I was unable to get any answers from nurses on current condition of client. Reasons were that shifts were changed, etc. Recreation activity was bingo. Difficult for alzheimer patient - no concept of what was happening.
