I am a friend or relative of resident

Unfortunately, my mother lived her a few years. She was in a nicer facility previously, but tried to make a go of it on her own and it didn't work out & she was too embarassed to go back so she chose here. Nurse's aides are lazy, my mom's room was always dirty, on four different occasions they sent my Mom to ACMC because she was acting "strange". Of course she was sent right back because the hospital did not notice or find anything wrong. This facility is located right behind ACMC. Instead of calling me to take her, they called Community Care at a $150 after insurance. Now that she has moved, we are having problems with them still debiting her checking acct. even though she has not lived there for over 2 months now. Won't return your phone calls when you notify them that this has happened and now she has a $30 NSF charge and no money to pay the rent at the facility she is currently at. Very unprofessional.

Friend or relative of resident

My grandfather was a resident of this center. It was horrible. He was always dirty, and the people there were rude. I visited him once, and remember that he had the same socks on from the previous week. I asked the nurse why they had not changed his socks and she stated that since he was not active, there was no reason to. I asked them when the last time he took a bath was and they stated about a week ago. I asked why it was a whole week and they stated then that they give sponge baths, so he is clean. He is not active so no need to go overly with it. I was outraged. My grandfather was so miserable there, we moved him to a nicer, smaller place. I have told everyone of this. No one should be treated this way.

Here at Country Club our average age resident is 65 however, we do a handful of residents that are under our medium age group. We provide tons of activities. Residents enjoy our trivia games, card games, our many social events, outings, and movie days/nights.

Here at Country Club we offer daily activities geared towards keeping the mind, body, & spirit healthy. We offer daily exercise, spiritual services, community outings, shopping trips, puzzle stations, library and many other social events to keep our residents entertained.

Here at Country Club we offer "Restaurant Style Dining" Everyday our dietary staff prepares a special of the day for lunch and dinner. If residents choose to have something else; they are provided with a menu where they can order an entrée of their choice to be prepared for them. While they wait they will have an option between two different soups, salad or a few other "starters" that's on the menu. Our menus are changed every 4 weeks to continue variety, quality and overall happiness with our residents.

Here at Country Club families are always notified if any changes of behaviors occur, health situations, and/or concerns that may arise. We also have an open-door policy so family is aware that they can call or stop by anytime to speak with staff to answer questions and/or address concerns.

Here at Country Club residents are allowed to have animals on our Independent/ Assisted Living. All animals must be under 15lbs, all vaccination records must be up to date. Residents are responsible to care for their fury friend, and make all future appointments for their fury love one while residing at Country Club.

Here at Country Club we pride ourselves in allowing our residents to know that this is there home. We make sure their our several common areas though out the facility; where residents can lounge and mingle with their loved-ones and/or friends. Our residents understand that our entire campus is their home, not just an area they sleep in.

Our residents safety is our priority we make sure that we provide fire drills, emergency evacuation drills, & flood drills.
