I am a friend or relative of a current/past resident

Used to be a wonderful place. The care was good and the DON and Administrator really cared about the residents. Over the past 3 1/2 months, it is awful. The DON has since left and now there is a new DON that is not approachable, and the administrator just doesn't care. I have called numerous times and left messages for the administrator and have heard nothing. I bet if I didn't pay my mom's bill, i would hear plenty. The staff doesn't change my mom, she had a fall and I was never notified. My aunt was also in this facility and fell and died from lack of sending her to the ER,

I am/was a resident of this facility

I was placed there after a 2 week bout with flu covid and pneumonia. Response to call bells very slow. Food was ok if you like sweet potatoes and collard greens. Most wasn't enough for a child. Star has been exceptional and caring.Weekend crew is even slower to respond. Facility was very nice airy and clean. Carpet in room smelled like urine. I would rate therapy a 4 of 5. Time spent was to short and not enough to get me on my feet or back to normal. I will say knowing the facility prior to me being here it is one of the best in burke. However less time at nurses desk and more time checking on and spending time with patients. If I need help again would come back but bring a hot plate and food...

I am a friend or relative of a current/past resident

My mother was a resident of this facility and she was provided with the best of care. Her PT and OT team were phenomenal. the nurses were very caring and prompt to respond to her needs or request. The room she was in allowed the space for her children, grandchild and great grandchildren to come and visit all at one time. The care she received sped up her recovery time and she was discharged sooner than expected. I would like to thank all the staff for the service they provide. It is much appreciated.

Private rooms are very nice and the one my family member is in has recently been redecorated with new furniture. The floor is carpeted however which doesn't provide for a very sanitary environment in a Rehab Facility. The carpet also results in additional difficulty for staff while attempting to move non-ambulatory residents. Dietary needs/requests are not reviewed appropriately with patients. My family member has had stomach problems for the past few months. She cannot eat saugage nor tolerate sweet food items mornings. The only meat items for breakfast this week have been saugage in some form or sweet rolls.

My brother was at College Pines Health and Rehabilitation Center, and he got along really well there. He was there for 100 days, and they helped him a lot. They were very attentive to him, so I thought that was good. I liked them. With COVID, I could not go and see him for a long time up until the last two weeks; then visitors were allowed to go in. He also said he liked some of the food, and some he did not like. He always liked his breakfast.

I am a friend or relative of a current/past resident

My 77 year old brother came to CP from a hospital. Initially he was in a very small semi-private room, but was moved the next day to a much larger, albeit semi-private, room which was very nice. The staff was extra attentive, including the nurses, CNAs, PT and OT...I couldn't have been more pleased. My brother was not able to do much due to his advanced disease but I was able to see how many activities they had planned for other residents every day. Cosmetically, the College Pines building is beautiful. Upon entering they always had coffee available with their Kureig machine, which I took advantage of on some cold winter days. I could tell their rehab room with equipment was well appointed and I found the PT staff to be very well trained and worked well with both my brother and us. If you are in need of either long term care of short term rehab care, you will not be disappointed.

I can't believe these terrible reviews. My best friend's mom, my aunt and my dad all here for months and years. All have had very good care. All treated with respect and love. My dad is currently a resident and has to have everything done for him. They do a wonderful job feeding him, bathing him, and transferring him from his bed to his wheelchair. He likes his roommate and thoroughly enjoys the Sunday morning services. The staff works very hard to make sure he has not only what he needs but also what he wants. I am eternally grateful to and for College Pines Health and Rehab Center.

Where do I begin. My mother was in Frye hospital because of congestive heart failure. She was 88 years old. One doctor wanted to send her for rehab and the other heart doctor suggested Pallitive Care because her heart was only working 25%. The doctor sent up the Palliative Care admn. from the hospital to talk to us and try and get her moved to the Hudson facility. However the other doctor already signed orders to have her moved to College Pines for rehab. Big mistake. The first night at the facility my mother woke up confused and went to the front desk and they put her in a wheel chair and she sit ALL night at the front desk. We told them that was not acceptable, because of all her health conditions, and spoke to the doctor there. He gave them strict orders to put the bed alarm on and look in on her every 15 minutes. They only did that one night. She had a stroke 4 days later. The 2nd shift RN said she didn't. We requested that an ambulance be called and they wouldn't. The following Monday she went into distress. Her BP was 40/14 and we had to demand twice for an ambulance. She was then taken to Valdese ER then Hospice and she passed away on 3/16. Terrible care. They are only good if you stay with your loved one 24/7 and demand care. Believe me I am filing a formal complaint.

Words fail me with this place. Incompetent doesn't begin to describe them. My mother was there for rehab after back surgery, getting her pain medication was always trouble, her c-pap and oxygen were hooked up wrong and when the staff was asked about it they tried to argue before they even looked. I just said the come explain to me how this is supposed to work the way it's hooked up. It was changed in a few minutes. When she was released again her pain meds were again a problem area. The list goes on and on. It seems strange how much trouble they had with only the pain medication it makes me wonder.

My father in law was in this facility shortly before he died. He was transferred to this facility from Mission Hospital and on the first day they failed to feed him. He was somehow left off of the cafeteria list and was not fed. We came to visit him one day and his socks were saturated in urine from an accident that he had earlier in the day. We had brought him a whole bag of brand new socks so there was no excuse for leaving him in filth. The staff was indifferent. I felt like there was something wrong with him several days before he was finally sent to the hospital. He ultimately died of a kidney infection that led to sepsis. I would not recommend this facility.

I visited this facility

My father passed away after being in this facility. He contracted MRSA and pneumonia while staying here. I was not impressed with the staff or cleanliness. Do your relatives a favor and DONT put them in College Pines.

The main reason why I went to College Pines Health and Rehab was that it was close. I had been in there for a month. The food was very good.
