I would like to thank the rehab staff. They worked well with my 100 year old mother. She enjoyed them very much. They tailored her exercises to her needs and abilities. She enjoyed them all. The thing that she disliked was the pureed bread. I tried for 10 weeks to get it off her plate to no avail.

Rooms are very dirty, had to complain about my father's sheets not being changed after wetting himself, trash over flowing, dirt all over the floor. Quality of care is horrible. Staff left my father with his meds unattended in the courtyard in sweltering heat. His face was sunburned and he was so weak we needed to bring him inside in a wheelchair. No one took any accountability. Please don't send your loved ones here...

My mother recently had a two week rehab “respite” stay. We paid privately and was told she would receive at least 5 days of OT and PT therapy. This did not happen. This facility is not managed properly and has insufficient staff. My mother experIenced long wait times or not at all response times to call light. My mother experienced a fall while there because a staff member went to answer a call light in a room they were passing instead of making sure my mother was being assisted to her therapy session. Most of the nursing staff seemed stressed and tried to be pleasant. Social Services Director is a rude and unprofessional individual. There were no care plan meetings or a safe discharge plan provided by the facility. There were no return telephone calls either from the social services department. A call received late afternoon the day before discharge to ask me what time my mother is leaving and the Director stating that “oh the social worker for that section is leaving in 7 days” is not an acceptable excuse as to why my telephone messages weren’t being returned. The family had to come up with their own safe discharge plan including setting up home health care services and medical supplies. Good thing I had previous experience in this field and knew what steps to take. I do not recommend this facility.

They have a very friendly and cooperative staff ready to help. Cassandra is concerned with keeping relatives of residents informed with events at Glade Valley through emails and zoom call on a regular basis which is very helpful. We really appreciate the zoom visits with our sister on a weekly basis. It is reassuring to visibly see the type of care she receives.

It is a great center for your loved ones. They are caring and great with the residents. They have a lot of activities for your loved ones. If you have a concern, they will be happy to help you with it. The facility is clean and the staff is friendly

My mother has been a resident at the Walkersville location of Autum Lake for about a year. The food is horrible, the rooms are dirty, the whole places smells terrible. The CNA's for the most part are uncaring and most have broken english that you cant understand. It takes forever to get a response to the call button. I cannot wait to get my mother out of there. This place is just not good.

My mother is a current resident, and can’t get bathed 2 times a week. It’s once a week because they are too busy or short staffed, so she now has a terrible rash. Call button? What a joke! It’s a minimum 45 minute wait. It has taken up to 2 hours at times. Good she wasn’t having a heart attack! Administration knows and has done nothing. Basic hygiene should not be neglected! Not for the money they charge! Shameful! The room walls have been patched and not painted. Terrible!

I was a Rehab Resident at this facility for a month starting 5/2022. I was very pleased with the facility. Rooms were spotless. Nursing Staff was very kind and were always helpful. Loved Haylie on Catoctin Unit!! The Physical and Occupational therapists were excellent. They really helped me to get home. The Food was really good. You can always request something different, and they have a list of alternatives!! The Beauty Shop did my hair and I was very happy! The stay was pleasant.

My wife is at Autumn Lake Healthcare at Glade Valley, and I think she is being very well taken care of. The facility is one of the best. She seems to be pretty content with the food. We haven't been in her room because of COVID. The have a lot of restrictions in place right now, but I could do an in-person visitation by appointment. The dining area is excellent. I am very pleased with them. The staff is wonderful, and I think facility is great for her. They have the coverage she needs. They've got nurses and doctors on staff. They are in transition because they were just taken over by another company.

My brother stayed in Glade Valley Nursing & Rehab. It was pretty good, but he just wanted to be home. I actually liked it. The place is pretty good. The staff is very efficient, and the rehab staff is excellent, in my opinion. They work with the patients very well, and they give the family updates on the patient. They didnt have a problem with my sister-in-law calling and asking how he was doing and things. His room was clean, and he had a window where we could speak to him from the outside. He was able to see his daughter and his wife. He actually liked the room. He had a private room, a private shower, and a private bathroom. The food was decent. It's clean and neat, and all the common areas were clean and sanitized. They made sure there were enough staff to attend to the patients' needs.

My mom was a resident for a bit a couple years ago. Dr Gilson changed her prdnisone from 15 mg to 10 mg and then mocked her in front of me. Nursing staff was intimidating, always overworked, left her ac on in the winter, urine in the bedpan when she moved into the room, claimed blood clots were bowel movements, NEVER put her support hose on properly, numerous bruises, forgot to feed her, just to name a few of my horrifying memories. Dr. Gilson should not be in charge of anyone that is in a compromised position.

My mom is doing rehab at Glade Valley Center. Everything is just my perception because I'm not allowed in, but the people there have been very helpful, very nice, and very professional. I don't really have anything negative to say, but I also don't know what the facility is like, since with COVID, you are not able to go in. There are no activities because of COVID. She said the food was unimaginative, but I assume they have to just keep repeating things because it's an institution. She did not say it was bad, though; she just said it was not creative.

My Dad was in Glade Valley for several weeks following a surgical procedure. I think experience depends on the staff involved in care. Amy the social worker was so helpful. Topper and Erin were great nurses and did anything they could to help and care for my dad. Several of the staff were integral in providing dignity and kindness. Food was ok to great. Facility could use remodel, but great overall.

Waiting 2+ hours and pressing call button several times just to be taken to the bathroom or get pain medicine after surgery. Also They charged me for a service I canceled and never happened! When I called to ask for help they didn’t care or if want to help.

Glade Valley is a nice organization. I do not have any issues with them. The staff seemed to be very helpful. The facility smelled clean, and it’s in a typical residential area.

My dad was in Glade Valley Nursing & Rehab. I didn’t have a good feeling for it; it was very institutional in nature. The food was terrible; the rooms were sufficient. My dad was in a double room, and the staff was just average. They didn’t go out of their way too much to care for him. They had some activities, but my father refused to join in.

Glade Valley is the other one in Frederick that is a big dive. I mean they sent my father in there with people that had MRSA and people who had pneumonia. I know that is typical, but it just didn't seem like a very good place. I actually asked my father to go to the emergency room because he started to get sicker. There was nobody that acknowledges him there. They have double occupancy, and nobody wants to be in a double especially if they are old and sick.

My husband was there for rehab (Stroke). Everyday I came in and asked about his therapy. Half the time they couldn't give me an answer and several times was told that they weren't able to get to him so they would get to him the next day. Occupational therapy was in the dinning room where we were rushed because of setting up/cleaning up for lunch. Very noisy and the employees did not respect people in therapy. Family members are not able to participate with the therapy. The physical therapy room is entirely small. Most living rooms in homes are bigger than this room. Therapists were very friendly but I know he didn't get the therapy he was supposed to get. The gentleman in the room with him had to use the restroom. He pushed the nurse button and after an hour of waiting and no one showed up, I walked up to the nurses station and told them. They said they would send someone down right away. Half hour went by, still no one showed up. I helped the man to the restroom. Two hours after the man pushed the button for the nurse, someone finally came. I told them that I helped the man to the restroom. She got an attitude and I told her the next time she needs to use the restroom, she should wait 2 hours. Over a year later, they sent me a bill.

My mom has had rehabilitation about 30 days ago in Glade Valley and the people were really good. It's a nice and comfortable place. Everybody was very good and very knowledgeable. She seemed to like the food. She could either have her food in her room or go to one of the dining areas. They do bingo and they have people coming in with music.

2 years ago I spent a few days in Glade Valley for rehab. after a knee replacement. At that time, it was awful. The food could not have been worse, the sink in the bathroom was pulled out from the wall, I needed a raised toilet seat and in order to do that I had to pick it up from the floor and put it on myself, all the while manipulating a walker, often at night my meds. were shorted ... when I asked about it, they said, "oh yes, I'll get the other one." The topper was one evening I was sitting on the bed talking to my roommate and across the floor ran a "grand-daddy" centipede." The furniture was obviously changed from what it had been prior to Genesis taking over, no pictures on the walls, no curtains. After 3 days, my doctor got me out. I will never recommend this place again to anyone. This is everything you hear about a bad, poorly run nursing home.

My Mom has been in this facility for over a year. This is the most wonderful place, clean , good food, loving caring staff. The house Doc is great as well. Mom has severe dementia following a stroke two years ago. She has been in two other facilites that were also quite good, but this one is the best. A bit pricey, but you cannot beat this place! Someone from the family visits every day and we are always welcome at any time. In my work, for a hospital in another state, I have visited at least 50 nursing homes ,and assisted living facilites and NONE of them hold a candle to Glade Valley. There is also a rehabilitation section and a regular elderly section, not just for dementia patients.

My grandmother has been a resident of Glade for a couple of years. I visited her once before they transferred her to an area for patients with mental difficulties. She shared a room with one other person. She has a room with a window and they allowed her to put up a bird feeder. This is one of her favorite things to do and I thought it was really nice that they could do that for her. The staff was very nice to me when I was there. They knew my mom well and joked around. The lady cleaning my grandmother's room commented on a flower that my grandmother had received. We ended up talking about it for a minute. It was just small talk, but it helped to break the silence. Sometimes it's hard to know what to talk about with my grandmother. My grandmother needed eye drops so my mom asked the front desk if they had any. They didn't, but took the time to tell her how to get to the closest CVS to buy some. My grandmother isn't happy there (probably wouldn't be anywhere), but they seem to treat her very well. She participates in the activities that they offer as a group and I think there is some benefit to her. The only negative experience has been with the dentist they have in their facility. This person did not fix my grandmother's problem and there were some billing issues. We discovered that several guests had similar complaints and we were told that they were looking into finding a new dentist. Overall, this facility seems like a good fit for my grandmother's needs.
